A cholesterol-rich diet accelerates bacteriologic sterilization in pulmonary tuberculosis.

BACKGROUND: Hypocholesterolemia is common among tuberculous patients and is associated with mortality in miliary cases. Some in vitro studies have shown that cholesterol is necessary for the good functioning of macrophages and lymphocytes. STUDY OBJECTIVES: To determine whether a cholesterol-rich diet could accelerate sputum sterilization in patients with pulmonary tuberculosis. DESIGN: An 8-week follow-up, randomized, controlled trial carried out from March 2001 to January 2002. SETTING: A third-level hospital for respiratory diseases in Mexico City. PATIENTS AND INTERVENTIONS: Adult pati

Twenty years of pulmonary tuberculosis in children: what has changed?

OBJECTIVES: To compare the frequency, clinical and radiologic manifestations and source of infection of pulmonary tuberculosis in children treated in our hospital during two decades (1978 through 1987 and 1988 through 1997) and to evaluate the influence of the emergence of HIV infection (since 1985) and the effect of the discontinuation of Calmette-Guérin bacillus (BCG) vaccination (since 1987) on childhood tuberculosis. METHODS: We reviewed 324 children diagnosed with pulmonary tuberculosis in our hospital during the 20 years (1978 through 1997). The data from 2 decades, 1978 through 1987 an

Surgical outcome of mycobacterium other than mycobacterium tuberculosis pulmonary disease.

Between January 1995 and May 1999, MOTT were cultured from sputum, bronchoalveolar lavage or resected lung specimens in 110 cases. 17 patients with MOTT pulmonary disease underwent pulmonary resection. Preoperatively, 5 of 17 patients had been diagnosed with MOTT pulmonary disease. The diagnosis of others was based on positive cultures from surgically resected material, and organism identification was successfully performed by the microplate DNA-DNA hybridization procedure. Surgical resections performed included wedge resection in 7, lobectomy in 6, and segmentectomy in 4. Antibiotics were gen

Pulmonary tuberculosis in HIV-infected patients with normal chest radiographs.

SUBJECTS: Three HIV-infected patients with active pulmonary non-disseminated tuberculosis and normal chest radiograph at clinical presentation and during follow-up are reported. Patients had cough and fever but no other specific symptoms. Löwenstein cultures of specimens from bronchoalveolar lavage in two cases and induced sputum in one yielded Mycobacterium tuberculosis. CONCLUSIONS: The diagnosis of tuberculosis in HIV-infected patients depends greatly on clinical suspicion by the physician, because of its atypical presentation. Failure to perform appropriate diagnostic tests in HIV-infecte

Apical opacity associated with pulmonary tuberculosis: high-resolution CT findings.

To elucidate the nature of the apical opacity that is commonly seen in patients with tuberculosis--usually referred to as an "apical cap" or "apical pleural thickening"--18 patients with upper lobe tuberculosis were studied with high-resolution computed tomography (HRCT). All had a homogeneous apical opacity at least 1 cm thick on chest radiographs. Fifteen of the 18 had a history of pulmonary tuberculosis of more than 5 years duration, and nine showed evidence of ipsilateral pleurisy. HRCT scans at the apex of the thorax in all nine patients scanned at this level showed that extrapleural fat

[Osteoarticular tuberculosis without pulmonary involvement. Description of 2 pediatric cases].

Tuberculosis of bone and joint is becoming uncommon in childhood, overall in absence of clinical and radiologic pulmonary findings. The Authors describe two cases: the first one had typical radiologic picture of the hand localization; the second one had tibial localization. Both the children had none clinical and radiographic findings of pulmonary tuberculosis. Chemiotherapical treatment and bed rest led to complete recovery, without anatomic and functional sequelae.

Solitary pulmonary nodule due to Cryptococcus neoformans and Mycobacterium tuberculosis.

This report describes a rare combined infection of Cryptococcus neoformans and Mycobacterium tuberculosis that manifested as a solitary pulmonary nodule in a nonimmunocompromised patient. Transthoracic needle aspiration biopsy was initially nondiagnostic, and histopathologic and culture confirmation of the diagnosis was eventually attained after wedge resection of the nodule. An extensive review of the English literature failed to reveal any reported cases of such a combined infection with a similar clinical presentation. The variable morphologic features of C. neoformans in tissue sections an
