The micturition reflex pathway is a supraspinal pathway. Anatomical tracing evidence is compatible with an involvement of the periaqueductal gray (PAG) in the ascending limb of this reflex. We tested the involvement of the PAG in receiving urinary tract- or perineum-related information and attempted to characterize this ascending path in terms of what type of information is being conveyed. Fracture of the penis is a relatively uncommon form of urologic trauma. We report an unusual case of rupture of both corpora cavernosa associated with partial disruption of the urethra. The patient underwent surgical repair using a Foley catheter as a urethral stent. The patient experienced excellent results with preservation of all functions.

In this study we examined sonographically the position of the bladder neck in healthy control persons and patients suffering from stress incontinence before and after pelvic floor exercises supported by Kolpexin. 14 of the 20 patients are satisfied after performing the Kolpexin exercises. We found by using perineal sonography a significant decrease of the angle gamma after the pelvic floor training. The perineal sonography allows good informations about the position of the bladder neck after conservative treatment, too.

Urethral pressure profiles (UPPs) were recorded in ten adult healthy male cats before and after administration of either phenoxybenzamine, diazepam, nifedipine or xylazine. A significant decrease (p less than 0.05) in urethral pressure at the level of the prostate was observed following treatment with all drugs. Primary localized amyloidosis of the bladder and urethra is a rare condition. Two cases of isolated primary amyloidosis, of the urethra in a man and of the bladder in a woman, are reported. The clinical specificity of the location and the specific therapy are discussed. We draw attention to the extended, palette of diagnostic investigations now available and the new classification made possible by the introduction of immunohistochemistry and immunoelectron microscopy.

An endoscopic, transurethral suspension of the female urethra and bladder neck is achieved by placing sutures through the anterior urethral wall up to the anterior abdominal fascia. The sutures are placed under direct endoscopic vision using a specially designed suture passer. In 8 female dogs a mean increase in urethral length of 1.81 cm (SEM 0.21 cm) was obtained immediately postoperatively. A new method of performing urethral pressure profiles and fluid bridge tests using computer technology is described, demonstrating the advantages over standard equipment. The fluid bridge test has been used to detect bladder neck incompetence in the male both before and after bladder neck surgery and has been adapted to investigate sphincter weakness at the distal urethral sphincter. This study suggests that the fluid bridge test is useful in the urodynamic evaluation of retrograde ejaculation, post-prostatectomy incontinence and vesico-urethral neuropathy.
