Multicenter Analysis of Patient Reported Outcomes Following Artificial Urinary Sphincter Placement for Male Stress Urinary Incontinence.

PURPOSE: Patient centered data are lacking regarding functional and quality of life improvements after artificial urinary sphincter placement. We analyzed the degree of benefit from artificial urinary sphincter placement using ISI (Incontinence Symptom Index), a validated patient reported outcome measure assessing the severity and bother of urinary incontinence, and IIQ-7 (Incontinence Impact Questionnaire-7), a validated patient reported outcome measure assessing the impact and emotional distress of urinary incontinence. MATERIALS AND METHODS: We performed a retrospective review at 4 centers…

Persistent urinary incontinence after a robot-assisted artificial urinary sphincter procedure: lessons learnt from two cases.

This case report describes 2 cases of persistent urinary incontinence in the beginning of the learning curve of robot-assisted bladder neck implantation of an artificial urinary sphincter (RA-AUS) in men at risk for erosion due to neurological lesions. Among a series of 4 RA-AUS, 2 patients still experienced urinary incontinence after surgery. A complete urological workup was strictly normal and did not show any device malfunction. However, during an AUS revision in these two patients, exploration revealed that the cuff was not tight enough. This issue was resolved by placing smaller cuffs. Af…

Ectopic Ureteral Orifice: An Unusual Cause of Urinary Incontinence in an Adult.

Vaginal insertion of an ectopic ureter is a rare anomaly causing continuous incontinence despite normal voiding. We report a 30-year-old woman with lifelong urinary leakage that remained unrecognized until adulthood. Computed tomography urography revealed a left duplex collecting system with an ectopic ureter draining the upper renal pole into the vagina. She underwent successful ureterovesical reimplantation using the Politano-Leadbetter technique, achieving immediate and complete continence. This case emphasizes the need for meticulous evaluation in adults with chronic incontinence to allow

Clinical Tools to Diagnose Frailty in Women With Geriatric Urinary Incontinence.

IMPORTANCE: Establishing an efficient and accurate frailty measure in older women with bothersome urinary incontinence (UI) is necessary because frailty increases the risk of UI treatment failure and postoperative morbidity. OBJECTIVE: This study aimed to primarily observe associations between the clinical frailty measure of gait speed and the electronic frailty index (eFI) to determine if the eFI may be a proxy in determining presence of frailty in older women with moderate-to-severe UI symptoms. STUDY DESIGN: This was a secondary data analysis of a prospective cohort study of women, older …

Sexual Function in a Randomized Trial of Pelvic Yoga for Urinary Incontinence.

IMPORTANCE: Urinary incontinence (UI) can negatively affect sexual function in women. Although many women with UI seek complementary strategies such as pelvic floor yoga techniques to improve sexual functioning, evidence of their efficacy is limited. OBJECTIVE: The objective of this study was to evaluate the effects of a pelvic floor yoga program versus a general physical conditioning program on multidimensional sexual function in women with UI. STUDY DESIGN: Ambulatory women aged 45 and older with daily UI were enrolled in a multicenter randomized trial in 2019-2022. Participants were ran…

Prevalence of urinary incontinence in patients with chronic cough: a systematic review.

Chronic cough-related diseases increase the risk of urinary incontinence (UI) due to repeated intra-abdominal pressure affecting the pelvic floor. Existing studies focus on specific diseases rather than UI as a broader consequence. To determine the prevalence of UI in patients with chronic cough-related diseases and compare it with the prevalence in healthy populations. A systematic review was conducted using Medline, Embase, Cochrane, OVID, Scopus, ProQuest, PEDro, and EBSCO. Two independent reviewers screened studies using predefined criteria in a Microsoft Excel spreadsheet. Only prevalence…

Sarcopenic Obesity as a Key Determinant of Urinary Incontinence in Older Women.

BACKGROUND: Urinary incontinence (UI) is a common geriatric syndrome that adversely affects quality of life. Sarcopenia and sarcopenic obesity (SO) may increase vulnerability to UI through the interplay of muscle weakness and excess adiposity, yet their contribution in older women is not well established. This study examined the prevalence of UI across women with sarcopenia and SO and explored its associations with other geriatric syndromes. METHODS: In this cross-sectional study, 535 community-dwelling women aged ≥ 60 years attending a geriatrics outpatient clinic underwent comprehensive ger…

An Overview of Novel Devices for Tibial Nerve Stimulation in Urinary Incontinence.

Urgency urinary incontinence (UUI) and overactive bladder (OAB) significantly impact quality of life in those who are affected. While initial treatments including lifestyle changes and pharmacotherapy often fail, second-line options such as posterior tibial nerve stimulation (TNS) offer relief. This review focuses on implantable TNS devices, a recent development in the field. Implantable TNS devices show varying degrees of effectiveness in reducing UUI episodes and improving patient quality of life. Most devices are minimally invasive and offer longer-term solutions with fewer surgical interve…

Heavy DIY Activities as a Potential Preventative for Stress Urinary Incontinence.

Physical activity is associated with stress urinary incontinence (SUI). The genetic causality of this association remains unclear. This study used the Mendelian randomization (MR) method to explore the potential causal relationship between physical activity and SUI risk using heavy do-it-yourself (DIY), light DIY, strenuous sports, walking for pleasure, and other exercises as proxies. We selected single nucleotide polymorphisms associated with physical activity from published genome-wide association studies (GWAS). Statistics of SUI come from the GWAS database. MR estimation was performed usin…

The views of women and their physicians on decision-making for stress urinary incontinence.

INTRODUCTION: Treatment decisions for stress urinary incontinence (SUI) are preference sensitive, because the disease is non-lethal and there are multiple reasonable treatment options. However, little is known about patients' and physicians' preferred decision-making styles for SUI. To aid physicians in their counselling and decision-making in consultations for SUI, we studied patients' and physicians' preferred and perceived decision-making in medical specialist consultations for SUI. METHODS: This mixed-methods study combined the validated control preference scale (CPS) and the CPS percepti…
