Case report: holmium laser resection and lasertripsy for intravesical ureterocele with calculus.

The holmium laser has established an important role in urology, particularly in the treatment of benign prostatic hyperplasia (BPH) and urolithiasis. Its light (wavelength 2140 nm) has a high coefficient of absorption in water, such that 95% of the laser energy is absorbed in 0.5 mm of water. Thus, the majority of the laser energy is contained superficially, imparting the holmium laser with excellent cutting and tissue ablation properties. It provides sharp incision with simultaneous hemostasis. Perhaps the most significant benefit of the holmium laser is its versatility when delivered through…

Bilateral xanthine nephrolithiasis in a dog.

Xanthinuria is an uncommon metabolic disorder clinically manifested as urolithiasis. There are two forms of the disease, congenital and iatrogenic. The former was diagnosed in a four-year-old dachshund bitch. The patient was presented with signs of terminal chronic renal failure. Urine was bacteriologically sterile with massive amorphous crystalluria. Bilateral nephrolithiasis was diagnosed at necropsy. The kidneys were shrunken with marked atrophy of the renal cortex and medulla. Infrared spectrometry revealed that the stones comprised 100 per cent xanthine. Later, two of the patient's siblin…

Urinary stone disease: a practical guide to metabolic evaluation.

Urolithiasis is the third most common affliction of the urinary tract, after infections and diseases of the prostate. Approximately 10% of the North American population will have at least one urinary calculus by age 70. A thorough history and physical examination will usually confirm the diagnosis of urinary stone disease. A simplified laboratory evaluation may be performed for patients with a single stone episode. A more extensive evaluation is required for patients with recurrent, metabolically active stones and patients at high risk (ie, cystine stone formers and those who develop bladder d…

[A long ureteral polyp with ureteropelvic junction obstruction: report of a case].

A case of long ureteral polyp with ureteropelvic junction obstruction in a 29-year-old man is reported. He had complained of occasional left flank pain since several years earlier. Excretory urography showed left giant hydronephrosis and no urolithiasis. Retrograde pyelography revealed a mass lesion of the left upper ureter. At exploration, the left ureteropelvic junction was found to be obstructed by an aberrant lower pole renal vessel. At just distal of the junction, a finger-like ureteral polyp, 5 cm in maximal length, was present and accompanied with several little polyps. Partial ureterec…

Renal Diffuse Large B-cell Lymphoma Mimicking Acute Pyelonephritis on 18 F-FDG PET/CT.

A 58-year-old man with a history of lithotripsy for urolithiasis presented with persistent fever and pollakisuria. As empirical anti-infective therapy was ineffective, 18 F-FDG PET/CT was performed to detect potential malignancy, revealing bilateral renal enlargement with multiple nodular cortex uptakes and an enlarged left axillary lymph node, suggestive of pyelonephritis. However, biopsies of the axillary lymph node and right kidney confirmed high-grade diffuse large B-cell lymphoma (DLBCL). Following R-DA-EPOCH chemotherapy, the patient achieved sustained remission. This case highlights tha…

Significance of Salmonella typhi bacteriuria.

Bacteriuria due to Salmonella typhi usually occurs following recent typhoid fever or in chronic carrier states. Data from 18 patients with S. typhi bacteriuria, seen during 5 years, were analyzed. Fourteen patients had localized urinary tract infection due to S. typhi. Four others had bacteriuria, probably associated with typhoid fever. Localized abnormalities of the urinary tract and kidneys and also systemic diseases were found to predispose patients to S. typhi bacteriuria. Local abnormalities encountered included urolithiasis (n = 3), prostatic hypertrophy (n = 1), and tuberculosis (n = 1)…

Spontaneous Ureteric Rupture Secondary to Ureteric Calculus: A Case Report.

Spontaneous ureteric rupture is a rare clinical entity defined by the extravasation of urine from the ureter in the absence of external trauma or instrumentation. It is most frequently associated with obstructive urolithiasis. We report the case of a 37-year-old male presenting with acute, severe abdominal pain secondary to spontaneous proximal ureteric rupture from an obstructing calculus. Despite the absence of advanced imaging like CT, a definitive diagnosis was achieved using Intravenous Urogram (IVU). The patient was managed conservatively and achieved full recovery without surgical inter…

[Decompression of upper urinary tract in various pathological conditions].

Obstruction of the upper urinary tract (UUT) is a heterogeneous clinical case, which, if necessary or in case of complications, is relieved by percutaneous nephrostomy or ureteral JJ-stent placement, which have its "pros and cons" in relation to complications, costs, difficulties in performing various pathological conditions, quality of life of patients and further treatment. The European Association of Urology Guidelines on Urolithiasis 2024 are based on the 2010s data review., fourteen years later, the optimal method of UUT decompression in various pathological conditions remains under debat…

Macrohaematuria in Anticoagulated Patients Abstract. Macrohaematuria should always be clarified as a symptom, regardless of whether anticoagulation exists. The most common causes are haemorrhagic cystitis and in men prostate hyperplasia. The most important other differential diagnoses are urothelial carcinoma and urolithiasis. Primary diagnostics include a rapid urine test, urine culture and sonography of the urinary tract. The complete examination is performed by cystoscopy and computer tomography. Coagulation should always be checked. Light bleeding can be clarified urologically on an outpat…

The diagnostic role of urinary N-acetyl-beta-D-glucosaminidase (NAG) activity in the detection of renal tubular impairment.

The kidney function can be assessed by a number of methods. The urinary excretion of enzymes, in particular N-acetyl-beta-D-glucosaminidase (NAG), is considered a relatively simple, cheap, fast and non-invasive method in the detection and follow-up of renal tubular function under various conditions. The determination of urinary NAG provides a very sensitive and reliable indicator of renal damage, such as injury or dysfunction due to diabetes mellitus, nephrotic syndrome, inflammation, vesicoureteral reflux, urinary tract infection, hypercalciuria, urolithiasis, nephrocalcinosis, perinatal asph…

[Urological and nephrological emergency--value of diagnostic imaging].

Many diseases of the kidneys and the urinary tract may develop to urinary emergencies. A prompt evaluation of these critical situations is of utmost importance concerning therapeutic consequences, and implicates a considerable challenge for the radiologist. The use of the appropriate imaging method allows rapid diagnosis, and thus enables optimal therapeutic management. In this article we describe the most important urinary emergencies: urolithiasis, hydronephrosis, acute renal infection, acute obstruction, testicular torsion, complications after renal transplantation, and urinary trauma. [A case of Prader-Willi syndrome accompanied with a renal stone].

Little information is available regarding the correlation between Prader-Willi syndrome and urolithiasis. We report a patient with Prader-Willi syndrome with a renal uric acid (UA) stone. A 23-year-old male patient was admitted to our department with gross hematuria and left flank pain. The blood and urine examination demonstrated hyperuricemia with the presence of UA crystals in the urine. Excretory urography revealed a radiolucent stone (17 x 27 mm) in the left renal pelvis suggesting a UA stone. The stone was removed successfully using extracorporeal shockwave lithotripsy (ESWL) combined wi…

ESR Essentials: renal imaging in children-practice recommendations by the European Society of Paediatric Radiology.

Renal pathology is common in childhood. Urolithiasis is relatively uncommon in children and is primarily diagnosed with US, although computed tomography (CT) may occasionally be necessary. Solid renal lesions identified on US should be further evaluated in highly specialised paediatric centres. Mild t …

[Urosepsis: pathophysiology, diagnosis, and management-an update].

Urinary tract infections vary widely in their clinical spectrum, ranging from uncomplicated cystitis to septic shock. Urosepsis accounts for 9-31% of all cases of septicemia and is often associated with nosocomial infections. A major risk factor for urosepsis is the presence of obstructive uropathy, caused by conditions such as urolithiasis, tumors, or strictures. The severity and course of urosepsis depend on both the virulence of the pathogen and the patient's specific immune response. Prompt therapy, including antimicrobial treatment and eradication of the infection source, along with suppo…

Vesicocutaneous fistula at site of closed suprapubic tube tract as the first presenting sign of giant bladder stone.

Bladder stones represent approximately 5% of all cases of urolithiasis and are typically identified and managed long before causing irreversible renal injury. We present a case of a man in his 40s with a prior history of a gunshot wound to the abdomen who presented with leakage from a previously healed suprapubic tube tract and was found to have a giant bladder stone with a resulting renal injury. He subsequently underwent a combined open cystolithotomy and vesicocutaneous fistulotomy during his hospitalisation, which helped to improve his renal function. In addition to there being few reporte…

Uroperitoneum attributable to ruptured urachus in a yearling bull.

Ruptured urachus was found to result in uroperitoneum in a yearling Beefmaster bull. The uroperitoneum was initially believed to be attributable to ruptured bladder secondary to urolithiasis; however, catheter decompression of the bladder through an ischiatic urethrotomy did not resolve the uroperitoneum. The persistent urachus was diagnosed and removed through caudal right flank laparotomy with the bull standing. The urachus was attached to the umbilicus, communicated with the lumen of the bladder, and had a mucosal lining. Ruptured urachus is an unusual cause of uroperitoneum, but can cause …
