During the elective course of human dissection at the University of Toyama in 2007, we encountered a rare case of double aortic arch accompanied by sub- and pre-aortic left brachiocephalic veins (LBV), and anomalous origin and course of the left vertebral artery in a Japanese elderly female. The double aortic arch formed a complete vascular ring that encircled the trachea and the esophagus. The sub-aortic LBV traversed below the aortic arches between the ascending aorta and the trachea. In addition, there was a small pre-aortic LBV passing anterior to the origins of the aortic arches. The left

Transverse process fractures of the cervical vertebrae have been considered rare and insignificant. In a retrospective study of 216 patients with cervical fractures evaluated by plain films and computed tomography, we found that transverse process fractures were common. Transverse process fractures were present in 24% of patients with cervical fractures and accounted for 13.2% of all cervical fractures. Cervical radiculopathy and brachial plexus palsy were present in 10% of patients with transverse process fractures. In 78% of transverse process fractures, CT scanning showed that the fracture

To compare the diagnostic values of high-resolution magnetic resonance (HR-MRI) with computed tomographic angiography (CTA) in young adults with ischemic stroke due to cervical artery dissections. Totally 42 symptomatic patients were recruited in this study. All the 42 patients underwent both HR-MRI and CTA, including 28 patients with dissections confirmed by Digital Subtraction Angiography (DSA) and 4 patients with vertebral artery dissections diagnosed by follow-up. CTA and HR-MRI images were separately and blindly analyzed by two radiologists. The sensitivity, specificity, positive and nega…

Posterior fossa ischemia is not a very frequent situation. It is responsible for about 25% of all ischemic strokes, and the vast majority of the cases are related to atherosclerotic stenosis of the vertebral and/or basilar arteries. Acute ischemia can also occur in the setting of vertebral artery dissection, traumatic or spontaneous. Recently, blunt trauma has been increasingly recognized as a cause for craniocervical artery injury. The management options for both traumatic and atherosclerotic lesions of the posterior fossa are still under debate. We present a case of a delayed onset of hemody…

An increasing number of cases with fenestration that is frequently associated with aneurysms have been reported, largely in the vertebral and basilar arteries and less commonly in the middle and anterior cerebral arteries by angiography as well as by autopsy. However, the anomaly of the internal carotid artery associated with bilateral arterial dissections is exceedingly rare. This represents the first reported case of fenestration of the internal carotid artery associated with arterial dissections. The embryological basis and clinical significance of this anomalous condition are discussed and

We present a case of a foramen magnum meningioma in a 42-year-old female who presented with headaches for 2 years, associated with decreased sensation and overall feeling of "heaviness" of the right arm. The tumor posed significant mass effect on the brainstem, and given the location of the tumor in the anterolateral region of the foramen magnum, a right far lateral approach was chosen. The approach incorporates the interfascial dissection technique to safely expose and preserve the vertebral artery in the suboccipital triangle. After drilling the posteromedial portion of the occipital condyle

Acute vertebrobasilar dissection may cause subarachnoid hemorrhage by rupturing through the adventia or cerebral infarct by progressive occlusion of the true lumen. Recent reports on the endovascular management of this condition have focused on treatment of pseudoaneurysms. We report two cases where angioplasty or stent placement was successfully used to improve compromised blood flow secondary to vertebrobasilar dissection.

We report on a patient with traumatic dissection limited to the basilar artery causing a locked-in syndrome. Magnetic resonance imaging (MRI) showed infarction of the basis pontis and a high signal intensity ring surrounding the central lumen of the basilar artery. Vertebral angiography revealed a double lumen within the basilar artery, consistent with a dissection. MRI and angiography are complementary to each other, and we consider both to be necessary for accurate diagnosis. Because MRI is less invasive than angiography, MR examinations should be performed before angiography for screening.

INTRODUCTION: Unlike spontaneous cervical artery dissection (CeAD), the incidence of CeAD after a major trauma is not well characterized. This population-based observational study investigates the epidemiology and natural history of traumatic cervical artery dissection (CeADs) using data from the Rochester Epidemiology Project (REP) in Olmsted County, USA.
METHODS: The REP system database has a high likelihood of complete case ascertainment and comparable demographics (age, sex) to the US population. After the initial screening of the database with ICD-based codes for carotid and vertebral di

BACKGROUND: The purpose of our meta-analysis is to evaluate the endovascular therapy (EVT) in patients with cervical artery dissection (CAD)-related acute ischemic stroke (AIS) by comparing its efficacy and safety with the ones of intravenous thrombolysis (IVT).
METHODS: A systematic search on EVT to CAD-related ischemic stroke is performed. The meta-analysis models are applied to calculate either the risk ratio (RR) with 95% confidence interval (CI) or pooled proportions with 95% CI of favorable functional outcome (mRS = 0-2), excellent functional outcome (mRS = 0-1), symptomatic intracrania

OBJECTIVE: To present the results of the nonrandomized arm of the Cervical Artery Dissection in Stroke Study (CADISS-NR) trial, comparing an … for superiority of anticoagulation or antiplatelet therapy in prevention of stoke after carotid and vertebral artery dissection; however, all data are from nonrandomized studies and randomized studies are required. The nonrandomized CADISS data show a lower rate of recurrent stroke than reported in some previous studies.

INTRODUCTION: Vertebrobasilar artery dissection (VBD) results from blood entering a tear in the intima of the vertebral artery (VA), basilar artery (BA), or their branches. T1-Sampling Perfection with Application-optimized Contrast using different flip angle Evolution (T1-SPACE) is a valuable tool for detecting intramural hematoma (IMH). Despite its potential, clinical adoption of susceptibility-weighted imaging (SWI) remains limited. This study aimed to clarify the usefulness of SWI and T1-SPACE for detecting IMH in VBD.
METHODS: Consecutive patients admitted with symptomatic VBD within 28 d

OBJECTIVES: Neuroprotection is of paramount interest in cardiac surgery. Right axillary artery cannulation is well established in aortic surgery because it significantly improves survival and outcome, but malperfusion of the right brain after direct cannulation has been reported. Anatomically, 4 vessel segments are potentially amenable for cannulation of the subclavian and axillary arteries. Clinical studies vary widely in dissection sites and cannulation techniques. We investigated critical flow dynamics in the right brain caused by arterial inflow after direct cannulation and specified cannu

UNLABELLED: Dissection of cervical arteries constitutes a medical emergency. Although relatively rarely, activities classified as sports and recreation may be a cause of arterial dissection independently of neck or head trauma. The purpose of the present paper was to present a series of cases of cerebrum-cervical arterial dissection in individuals during or soon after the practice of these sports activities.
METHODS: Retrospective data on patients with arterial dissection related to sports and recreation.
RESULTS: Forty-one cases were identified. The most frequently affected vessel was the v

Ultrasound is the most widespread diagnostic procedure in obstructive disease of the arteries supplying the brain. The combined non-invasive information on morphology and function makes duplex ultrasound the procedure of choice in screening and follow-up of carotid artery disease. This review deals with all relevant aspects of color duplex ultrasound of the carotids and the vertebral arteries. After a short introduction into the clinical background, the paper focuses on aspects of examination technique. In the main part of the review the relevant ultrasound findings in carotid artery disease a
