Cervical and Ocular Vestibular Evoked Myogenic Potentials in Fibromyalgia Syndrome Patients.

BACKGROUND: Fibromyalgia syndrome (FMS) is a chronic pain condition that may be associated with dysfunction in the central nervous system. OBJECTIVE: The aim of this study was to assess the vestibulo-spinal reflex (VSR) and vestibulo-ocular reflex (VOR) in FMS using the cervical vestibular evoked myogenic potential (cVEMP) and ocular vestibular evoked myogenic potential (oVEMP) tests, respectively, and to evaluate their relation to disease severity. METHODS: This study included 30 female FMS patients and 30 well-matched healthy controls. They underwent full history taking and assessment of t

Does unilateral utricular dysfunction cause horizontal spontaneous nystagmus?

The presence of spontaneous nystagmus in darkness with a strong horizontal component has been taken to indicate that there is asymmetrical function of the horizontal semicircular canals. If this horizontal spontaneous nystagmus can be suppressed by vision, then it is regarded as due to peripheral horizontal canal dysfunction. However, we report evidence from one patient (61-year-old male), who visited the MSA ENT Clinic, Cassino (FR) Italy, reporting acute, severe vertigo, postural unsteadiness, nausea and vomiting associated with right sudden hearing loss. The patient received instrumental au

Audiovestibular testing in patients with Meniere's disease.

In this article, the present state of the art with respect to audiovestibular testing for Meniere's disease (MD) is reviewed. There is no gold standard for MD diagnosis, and the classic dictum is that even the "best" tests yield positive results in only two-thirds of patients with MD. Still, we advocate the use and further investigation of advanced audiovestibular testing in patients with MD in an attempt to answer the questions that confront any clinician who cares for patients with audiovestibular symptoms.

Role of subjective visual vertical test during eccentric rotation in the recovery phase of vestibular neuritis.

OBJECTIVES: By assessing unilateral utricular function at the acute and subacute stages, we sought to determine the ability of the subjective visual vertical (SVV) during eccentric rotation (dynamic SVV) in detecting latent abnormality of otolith dysfunction in unilateral vestibular neuritis. In addition, we compared the validity of the dynamic SVV with static SVV during the compensated recovery phase. METHODS: The static SVV and dynamic SVV of 19 patients diagnosed with acute unilateral vestibular neuritis and 31 patients diagnosed with the subacute stage of vestibular neuritis were determin

Migraine and isolated recurrent vertigo of unknown cause.

Chronic recurrent attacks of vertigo, not associated with any auditory or neurological symptoms, are a common reason for referral to our neurotology clinic. Even after an extensive neurotological evaluation, some cases remain undiagnosed. We prospectively evaluated 72 consecutive patients who presented to the clinic with isolated recurrent vertigo of unknown cause. All patients underwent diagnostic evaluation to exclude identifiable causes of isolated recurrent vertigo. We compared the prevalence of migraine, according to the International Headache Society (IHS) criteria, in the isolated recur

Discrimination of patients with acoustic neuroma and peripheral vestibular lesions with human posture dynamics.

A group of normal subjects (n = 17) was compared with groups of patients with a diagnosis of vestibular neuritis (n = 18), and acoustic neuromas (n = 35). Fisher linear discriminant analysis was applied to distinguish clusters of parameters characteristic for each disease. Hence it was possible to distinguish the vestibular neuritis patients from the normal group with statistical significance (p < 0.01). Also the patients with an acoustic neuroma could be distinguished from the normal subjects with statistical significance (p < 0.05).

[The Romberg head-shake test within the scope of equilibrium diagnosis].

We review the standard functional tests of static and dynamic equilibrium and discuss the value of these tests. The pendular platform test and the tilting table test were developed for provocation of the balance system, but are not used in clinical investigation because of the complicated apparatus and the difficulties of judgement of the curves recorded. A simple modification for provocation of the balance system, available in the clinic, the consulting room or the doctor's office is recommended, consisting of horizontal head-shaking in the Romberg position during posturographic recording. [Critical remarks on so-called cervicogenic vertigo].

A critical review of neurophysiological literature is given about the part played by the neck proprioceptors in the regulation of vestibular functions. In comparison with vestibular and optokinetic influences the neck proprioceptors are of secondary importance. A critical review of the publications by the supporters of a cervical genesis of vertigo leads to the conclusion that "cervical nystagmus" must be proven before the concept of neck-triggered vertigo can be accepted. In relation to all forms of vertigo, vertigo induced by cervical disorder represents a very low percentage only.

Acute Alcohol Intake Impairs the Velocity Storage Mechanism and Affects Both High-Frequency Vestibular-Ocular Reflex and Postural Control.

BACKGROUND: Acute alcohol intake is known to cause gait instability, dizziness, and lack of psychomotor coordination. Previous studies demonstrated the positive effects of alcohol on the oculomotor system and the low-frequency vestibulo-ocular reflex (VOR). However, the low-frequency VORs is a rather un-physiologic stimulation, and the reported explanations regarding the relations between the alcohol-induced VOR changes and posture control are inconsistent. OBJECTIVE: The present study evaluates how acute alcohol intake affects more physiologic mid- to high-frequency VORs, postural control, a

Live imaging and functional changes of the inner ear in an animal model of Meniere's disease.

The symptoms of Meniere's disease (MD) are generally considered to be related to endolymphatic hydrops (EH). There are many recent reports supporting the possibility that vasopressin (VP) is closely linked to the formation of EH in Meniere's disease. Based on this, we developed a clinically relevant animal model of Meniere's disease in which a VP type 2 receptor agonist was administered after electrocauterization of the endolymphatic sac. We report live imaging of the internal structure, and functional changes of the inner ear after electrocauterization of the endolymphatic sac and administrat

Central paroxysmal positional nystagmus: Characteristics and possible mechanisms.

OBJECTIVE: The diagnosis of central paroxysmal positional nystagmus (CPPN) is challenging, and the mechanisms require further elucidation. This study aimed to determine the characteristics and mechanisms of CPPN. METHODS: Seventeen patients with CPPN were subjected to analyses of their clinical findings, MRI lesions, and oculographic data on spontaneous and positional nystagmus. RESULTS: The direction of CPPN was mostly aligned with that of the head motion during the positioning, and 3 types of CPPN were identified: downbeat nystagmus on straight-head hanging, upbeat nystagmus on uprighting,

Benign paroxysmal positional vertigo in an airline pilot.

Benign paroxysmal positional vertigo (BPPV) is one of the most common types of peripheral vertigo, characterized by violent whirling vertigo after a positional change. Although the condition is termed "benign," the clinical presentation can be incapacitating for pilots in certain maneuvers. We present a case of an airline transport pilot with the complaint of vertigo for 5 d. The vertigo was aggravated by head movements when looking up or rolling over, lasting for a few seconds. The patient was diagnosed with BPPV, and he was treated with physical therapy with the use of Epley maneuver. The ai

Postural and locomotor control in normal and vestibularly deficient mice.

We investigated how vestibular information is used to maintain posture and control movement by studying vestibularly deficient mice (IsK-/- mutant). In these mutants, microscopy showed degeneration of the cristae of the semicircular canals and of the maculae of the utriculi and sacculi, while behavioural and vestibulo-ocular reflex testing showed that vestibular function was completely absent. However, the histology of Scarpa's ganglia and the vestibular nerves was normal in mutant mice, indicating the presence of intact central pathways. Using X-ray and high-speed cineradiography, we compared

Vestibular evoked myogenic potential (VEMP) in patients with acoustic neuromas.

OBJECTIVE: To study the utility of VEMP (vestibular-evoked myogenic potential) in the diagnosis of acoustic neuromas. METHODS: Eighteen patients with unilateral acoustic neuromas were subjected to this study. Myogenic potential responding to loud click stimuli was recorded at ipsilateral sternocleidomastoid muscle. A normal range of VEMP was obtained from 20 controls. VEMP responses were compared with both, clinical symptoms and results of caloric tests. RESULT: Thirteen out of 18 patients showed decreased responses of VEMP at the affected side. VEMP responses seemed to have little relation

Vestibular reactions to spaceflight: human factors issues.

Vestibular function, along with other sensory systems influencing spatial orientation, can have a profound influence on the ability of astronauts to function effectively. Beyond the well-known problems of space motion sickness, vestibular effects can influence astronaut well-being and performance during all phases of a space mission. This paper discusses some of the major vestibular reactions affecting human factors encountered in all space missions, and covers them chronologically in the following sequence: launch, early on-orbit, late on-orbit, EVA, artificial gravity, re-entry, and post-lan

[Isolated vertigo in vertebrobasilar ischemia--does it exist?].

An isolated vertigo may occur in vertebrobasilar ischemia, especially as the first symptom of cerebellar ischemia or basilar artery occlusive disease. Based on neuroanatomical findings, these short-lasting vertigo attacks result from transient insufficiency of the anterior inferior cerebellar artery (AICA), which supplies the inner ear and leads to ischemia of the upper parts of the vestibular labyrinth. Persisting vertigo with or without lateropulsion may occur in cerebellar infarctions with involvement of the medial inferior hemispheres due to occlusion of the medial branch of the posterior

Diagnostic testing for endolymphatic hydrops.

The development of a sensitive and specific diagnostic test for endolymphatic hydrops has eluded investigators for over 30 years. The absence of such a test has hampered basic and clinical research into Meniere's disease and related entities. Presently used tests are limited in their applicability because of their low sensitivity and specificity and a poor understanding of the underlying physiologic principles. Despite this, it is generally agreed that some of these studies have merit in selected situations. This article reviews the present state of diagnostic testing for endolymphatic hydrops

The use of the logistic model in space motion sickness prediction.

The one-equation and the two-equation logistic models were used to predict tested subjects' susceptibility to motion sickness in KC-135 parabolic flights using data from other ground-based motion sickness tests. A data set containing data from 6 provocative tests, 2 vestibular function tests, and 1 motion sickness experience questionnaire from 162 subjects was used in this study. The prediction results from the logistic models were compared with those from the previously-used Bayes linear discriminant analysis procedures. The results based on this data set show that the logistic models correct…
