Bone mineral density measurement in puerperal women as a predictor of persistent osteopenia.

The clinical value of bone mineral density (BMD) measurement in puerperal women is uncertain. Our aim was to examine the effectiveness of BMD measurement in puerperal women for identification of persistent osteopenia and osteoporosis. We addressed cross-sectional and longitudinal changes in BMD, assessed using dual-energy X-ray absorptiometry (DXA), in a postpartum female cohort from a single center in Japan. We measured BMD of the lumbar spine (L2-L4) with DXA in 2,436 puerperal women within 7 days of delivery (study 1). For 210 of the women, the BMD was measured again after 5-10 years (study

Bone mineral metabolism in children with juvenile idiopathic arthritis--preliminary report.

INTRODUCTION: There are very few reports assessing bone mineral mass and its metabolism in the course of juvenile idiopathic arthritis (JIA). STUDY OBJECTIVE: To assess the levels of selected serum markers of bone formation (OCN) and resorption (CTx) in JIA children. MATERIAL AND METHODS: The study involved 52 children with JIA diagnosed according to the EULAR criteria of 1997, aged 6-18 years. All patients underwent densitometric measurements using dual-energy X-ray absorptiometry (DXA) to assess TBBMD (g/cm2), Spine BMD (g/cm2), Z-score for SBMD, TBBMC (g), and LBM (g). The following param

A structural approach to the assessment of fracture risk in children and adolescents with chronic kidney disease.

Children with chronic kidney disease (CKD) have multiple risk factors for impaired accretion of trabecular and cortical bone. CKD during childhood poses an immediate fracture risk and compromises adult bone mass, resulting in significantly greater skeletal fragility throughout life. High-turnover disease initially results in thickened trabeculae, with greater bone volume. As disease progresses, resorption cavities dissect trabeculae, connectivity degrades, and bone volume decreases. Increased bone turnover also results in increased cortical porosity and decreased cortical thickness. Dual-energ

Thawing of frozen calcaneus bone specimens has no effect on the bone mineral density using dual energy x-ray absorptiometry: a study in rabbits and humans.

The purpose of this paper is to study whether the deep freezing of bone and later thawing affects the bone mineral density (BMD) measurement. We used 56 calcanei from 28 adult female New Zealand white rabbits and 102 human calcanei from 51 donors post-mortem (27 men and 24 women, age 30-89). Dual energy x-ray absorptiometry evaluated BMD of the frozen specimen and of the thawed specimen. A main analysis compared BMD of each specimen in the frozen and thawed states. The mean BMD of 224 areas of frozen rabbits' calcanei was 0.31 +/- 0.08 g cm(-2) (95% confidence interval (CI) from 0.30 to 0.32 g

Quantitative ultrasound calcaneus measurements: normative data for the Greek population.

Quantitative ultrasound (QUS) is a peripheral bone densitometry technique that is rapidly gaining in popularity, and is widely used worldwide for the assessment of skeletal status. This, however, generally occurs in the absence of adequate clinical guidelines. As accurate interpretation of the results and correct classification in individual fracture risk assessment are of great value, the present study was carried out to establish a reference database for calcaneal QUS measurements across age group and gender in Greece. A total of 1205 subjects (821 females and 384 males) from three age group

Bone mineral density is related to blood pressure in men.

The aim of this study was to determine the relationships between bone mineral density (BMD) and blood pressure in 214 men, age 20-76. BMD measurements were done by dual X-ray absorptiometry using a Lunar DPXMD densitometer at the lumbar spine (L2-L4) and different femoral regions. Systolic (SBP) and diastolic (DBP) blood pressure were measured using an MPC-350 sphygmomanometer. Physicians gathered demographic data and participants' dietary intake of calcium were determined by using food frequency questionnaires. After adjusting for age, body mass index, dietary calcium, and exercise history, m

Interobserver reproducibility of criteria for vertebral body exclusion.

UNLABELLED: We studied reproducibility of the ISCD vertebral exclusion criteria among four interpreters. Surprisingly, agreement among interpreters was only moderate, because of differences in threshold for diagnosing focal structural defects and choice of which vertebra among a pair discordant for T-score, area, or BMC to exclude. Our results suggest that reproducibility may be improved by specifically addressing the sources of interobserver disagreement. INTRODUCTION: Although DXA is widely used to measure vertebral BMD, its interpretation is subject to multiple confounders including osteoa

Diagnosis of osteoporosis in men, premenopausal women, and children.

The World Health Organization (WHO) classification of bone mineral density is based on population studies in postmenopausal women. However, the increased use of bone densitometry has raised questions regarding the applicability of this classification to men, premenopausal women, and children. These questions were addressed at the International Society for Clinical Densitometry 2003 Position Development Conference. T-scores can be used and a diagnosis of osteoporosis made for T-scores of -2.5 or less (male reference database) in men age 65 yr and older and in men from 50 to 64 yr of age if othe

[Screening for postmenopausal osteoporosis. Women with distal radius fractures should be evaluated for bone density].

In a case-control study at two country hospitals in Sweden, 110 consecutive postmenopausal women (age 50-75) with distal radius fracture were examined with the DXA-technique, on the injured radius, lumbar spine, and the right hip within three weeks after the fracture occurrence. Data was compared with 55 age-matched controls from The Swedish Population Register. The incidence of osteoporosis according to WHO's definition (T-score < -2.5) at any measurement site was higher in the fracture group, 44 per cent compared with 27 per cent in the control group. The fracture group had 12 per cent lower

Osteoporosis in anorexia nervosa: prevention and treatment.

BACKGROUND: Osteoporosis is a very prevalent complication of anorexia nervosa. In contrast to the many other medical complications of anorexia, osteoporosis and its sequelae of fractures, kyphosis, and pain may persist regardless of the overall treatment outcome. DISCUSSION: Traditional well-proven therapies for postmenopausal osteoporosis are not as effective against osteoporosis in anorexia nervosa. Therefore, clinicians who treat these patients must become increasingly vigilant about osteoporosis in regards to preventive, diagnostic, and treatment strategies.

Bone mineral density loss in patients with urolithiasis: a follow-up study.

BACKGROUND: Recurrent calcium urolithiasis is often associated with disorders of calcium metabolism. The purpose of this investigation was to assess bone mineral content (BMC) and bone mineral density (BMD) over a period of 1 year in patients with urolithiasis and to determine the factors that could have influenced the changes in bone density during that period. METHODS: The patient group comprised 34 men aged 41.2 plus minus 7.9 years with recurrent urolithiasis. A wide spectrum of biochemical measurements was performed. Bone mineral density (g/cm(2)), bone mineral content (BMC), and bone ar

Quantitative ultrasound in postmenopausal osteoporosis.

Ultrasound has been proposed as a low-cost, radiation-free method for osteoporosis assessment in postmenopausal women. Large prospective studies have shown that ultrasound parameters can be used for fracture risk estimate in this population, providing that adequate quality control is performed. The places of both ultrasound and the current gold standard method for bone assessment, dual energy x-ray absorptiometry, are still to be determined. Further studies are needed on the diagnosis of osteoporosis using ultrasound, because current diagnostic thresholds, designed by the World Health Organiza

High bone mass and hypocalcaemic myopathy in a patient with idiopathic hypoparathyroidism.

The clinical manifestations of hypoparathyroidism are mainly characterised by increased neuromuscular irritability as a consequence of hypocalcaemia. Occasionally, elevation of the muscle enzymes may mimic polymyositis. Reduced parathyroid hormone production, but also vitamin D treatment and calcium supplementation, may contribute to the increased bone mass found in patients with postsurgical hypoparathyroidism. We report the case of a 36-year-old woman with untreated idiopathic hypoparathyroidism and a high bone mass despite severe muscle impairment due to hypocalcaemic myopathy.

Calcitriol for bone loss in patients with primary biliary cirrhosis.

Because osteoporosis is a common complication of primary biliary cirrhosis, we evaluated the effects of calcitriol (1alpha, 25-dihydroxyvitamin D3) on bone mineral density in 34 women with primary biliary cirrhosis (stage I disease in 16 patients, stage II in 9, stage III in 4, and stage IV in 5). Patients were assigned randomly to receive calcitriol (0.5mg twice a day) or not. Bone mineral density of the lumbar vertebrae was measured by dual-energy X-ray absorptiometry at least twice during a period of 12-43 months. The mean annual change in bone mineral density was 0.1% in the treatment grou

[X-ray densitometry and biochemical parameters of bone tissue metabolism in patients with vibration disease].

To reveal systemic and local osteoporosis, the authors studied biochemical markers of bone metabolism in vibration disease patients. The vibration disease patients appeared to have the most frequent and marked osteoporosis in peripheral bones--hands (in 90% of cases) and forearms (in 66.7%). Prevalence of systemic osteoporosis and osteopenia reached 11.7 and 48.3% in the select respectively. According to biochemical markers, bone reconstruction state was characterized by moderately intensified bone resorption and diminished bone formation.
