Vertebral artery dissection due to indirect neck trauma : an underrecognised entity.

Vertebral artery dissection is an important cause of brain stem stroke, especially in the young. Dissections of carotid and vertebral arteries in neck account for about 20% of strokes in young compared with 2.5% in the elderly. Three patients of vertebral artery dissection related to indirect neck trauma are described. The first patient developed the symptoms while dancing, the second after a trivial fall and the third while he was on a dental chair. None of them had a direct severe neck trauma or concomitant risk factor like hypertension, connective tissue disease or migraine. Clinical sympto

[The limitations of three-dimensional CT angiography (3D-CTA) in the diagnosis of cerebral aneurysms].

OBJECT: The purpose of this study is to show some limitations of 3D-CTA to diagnose cerebral aneurysms. METHODS: Sixteen saccular aneurysms less than 10 mm in diameter were included. Large and complicated aneurysms were excluded. RESULTS: Although information about perforating arteries from the posterior cerebral artery is very important for surgery of basilar bifurcation aneurysms, 3D-CTA could not delineate the perforating arteries. A small posterior communicating artery (Pcom.A.) was not detected, and it was very difficult to differentiate infundibular dilatation of the Pcom. A. from an a

Compression of digital coronary angiograms does not affect visual or quantitative assessment of coronary artery stenosis severity.

Digital coronary angiographic techniques are now widely used in many cardiac catheterization laboratories. However, the full potential of digital imaging technology remains to be achieved because of its enormous storage and exchange requirements. Compression of digital imaging data allows a reduction in the volume of data so that storage and transmission are more efficient and cost-effective. Three angiographers reviewed the original and compressed formats of 96 coronary angiographic sequences in a blinded fashion to assess coronary lesion severity. Compression was achieved using the Joint Pho

Takayasu's arteritis with collateral circulation from the right coronary artery to intracranial vessels--a case report.

A forty-four-year-old woman with Takayasu's arteritis and involvement of the aortic arch and its main branches complained of precordial pain on effort. Exercise electrocardiograms revealed significant ST segment depression in leads II, III, aVF, and V. Coronary arteriograms demonstrated no stenosis. However, the right coronary arteriogram revealed collateral circulation arising from the sinus node artery to the bilateral vertebral arteries and the left internal carotid artery. The collateral circulation was considered to be an important route of blood flow supply to the brain and, at the same

Brachial artery aneurysm and bilateral posterior circulation strokes in a young child with tuberous sclerosis complex.

Although tuberous sclerosis (TS) may affect many organs, vascular manifestations involving medium- and large-size vessels are rare. We present a young child with known TS who presented with bilateral posterior circulation infarcts and subsequently was found to have right-hand ischaemia secondary to a thrombosed brachial artery aneurysm. A wound on his right middle finger failed to heal with conservative management, and digital subtraction angiography and MR angiogram demonstrated a lack of bypass target with microcollateral supply of the forearm only. The right middle digit ischaemia was initi

Three-material decomposition using a dual-layer flat panel detector in the presence of soft tissue motion.

Digital subtraction angiography (DSA) is a widely used technique for the visualization of contrast-enhanced structures. However, temporal subtraction DSA is challenged by misregistration artifacts due to patient motion and incomplete separation of iodine contrast agent from background soft tissue and bone. In this work, we propose an approach that allows three-material decomposition using a dual-layer flat panel detector in the presence of soft tissue motion. We assume the calcium signal (bone) remains stationary in the pre- and post-contrast images but allow soft tissues to move freely (e.g.

Diagnosis of moyamoya disease with transcranial Doppler sonography: correlation study with magnetic resonance angiography.

BACKGROUND AND PURPOSE: Although the diagnosis of moyamoya disease may be confirmed by digital subtraction angiography, recent studies have shown the accuracy of magnetic resonance angiography. Characteristics of transcranial Doppler, a noninvasive and cost-effective method, and specific transcranial Doppler parameters reflecting distinct vascular status in moyamoya disease are explored. METHOD AND PATIENTS: Consecutive patients (> 15 years of age) diagnosed with moyamoya disease by a typical clinical history and digital sub traction angiography or magnetic resonance angiography were included

Anomalous origin of right vertebral artery from right common carotid artery.

Anomalous origin of vertebral arteries is a rare vascular anomaly and mostly discovered as incidental findings during computed tomography angiogram , magnetic resonance angiography or digital subtracted angiogram of the aortic arch and cerebral vessels. Herein, we present an extremely rare case of a 31-year-old female who presented with headache after emotional trauma. A conventional cerebral angiogram showed anomalous origin of the right vertebral artery. This finding was incidentally discovered, and it is of utmost importance for future head and neck endovascular interventions to avoid inadv

Susceptibility-Based Characterization of Cerebral Arteriovenous Malformations.

OBJECTIVES: The aim of this study was to explore blood deoxygenation across cerebral arteriovenous malformations (AVMs) for functional characterization of AVM vasculature. MATERIALS AND METHODS: Fifteen patients with cerebral arteriovenous vascular malformation were prospectively studied by digital subtraction angiography and using a 3 T magnetic resonance imaging system, with which three-dimensional (3D) gradient echo data for the calculation of quantitative susceptibility maps, velocity-encoded 3D gradient echo data for 3D flow assessment, and contrast-enhanced 3D time-of-flight data were a

Clinical status and evolution in moyamoya: which angiographic findings correlate?

Moyamoya is a progressive steno-occlusive cerebrovascular pathology of unknown aetiology that usually involves the terminal portions of the internal carotid arteries and/or the proximal portions of the anterior and middle cerebral arteries bilaterally. The pre-operative Suzuki staging system and post-operative Matsushima grade are nearly universally used markers of natural history and surgical revascularization results, respectively, but their correlation with clinical and radiographic manifestations of moyamoya has not been systematically evaluated in a large cohort. This study evaluated the

Effect of superior retinacular artery damage on osteonecrosis of the femoral head.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) is the result of dysfunctional blood supply, but associations between specific damaged arteries, imaging changes and clinical sign require more understanding. We investigated characteristics of ONFH that pertain to blood supply, imaging appearance, and clinical feature to judge the prognosis of ONFH. METHODS: Clinical data were collected for 92 patients (118 hips) with ONFH, including gender, age, duration of pain (from initial clinical presentation to arthroplasty), cause, stage, and classification. Magmatic resonance imaging and X-rays we

M2/M2 side-to-side rescue anastomosis for accidental M2 trunk occlusion during middle cerebral artery aneurysm clipping: technical note.

OBJECTIVE: A technically feasible and rapid technique for revascularizing the main branches of the middle cerebral artery (MCA) is described. This technique is applied mainly when clipping of an MCA aneurysm is complicated and occlusion of the origin of an MCA main branch results. METHODS: M2/M2 side-to-side anastomosis was applied in two patients in whom unplanned M2 occlusion occurred during the course of complicated MCA aneurysm clipping. The first patient underwent an emergency procedure after temporoparietal intracerebral hemorrhage. Unilateral mydriasis precluded preoperative angiograph

Retrograde endovascular recanalization via the ascending cervical artery for non-conical stump vertebral artery occlusion: a case report.

A 61-year-old man presented with a non-tapered occlusion at the origin of the left vertebral artery, with the right vertebral artery failing to join the left vertebral artery to form the basilar artery, and basilar artery tip occlusion. Early antegrade endovascular recanalization attempts with microwires failed to traverse the occlusion at the left vertebral artery origin. Digital subtraction angiography revealed a well-developed left ascending cervical artery communicating with the V3 segment of the left vertebral artery. We adopted a retrograde endovascular recanalization strategy and, with

Endoscopic third ventriculostomy for hydrocephalus accompanied by dural arteriovenous fistulae: a case report and literature review.

A 54-year-old man presented with gait disturbances, urinary incontinence, and headache for 6 months. Head computed tomography indicated several high-density mass lesions in the quadrigeminal cistern, causing occlusive hydrocephalus. Digital subtraction angiography confirmed tentorial dural arteriovenous fistulae (AVF). Transarterial embolization (TAE) achieved complete angiographic resolution. However, acute occlusive hydrocephalus worsened, necessitating endoscopic third ventriculostomy (ETV). The patient was discharged without new symptoms and no hydrocephalus recurrence at six-month follow-

High-flow post-traumatic priapism: diagnostic and therapeutic workup.

Priapism is a prolonged penile tumescence or erection unrelated to sexual stimulation that lasts more than 6 h. High-flow priapism is a rare condition usually due to a perineal trauma with the formation of an arterio-cavernosal fistula between a cavernosal artery and the lacunar spaces of the penis. High-flow priapism is usually diagnosed by clinical examination and color Doppler ultrasound, and the gold standard therapeutic management is represented by digital subtraction angiography. We report a case of a young patient with high-flow priapism after a motorcycle blunt perineal trauma, examini
