Skin appendage involvement in anal intraepithelial neoplasia.

BACKGROUND: High-grade anal intraepithelial neoplasia (AIN III) may be premalignant. Surgical excision of large areas of anal epithelium carries significant morbidity. Ablation treatments may carry less morbidity; however, the depth of ablation is uncertain and failure to ablate dysplasia in hair shafts and other skin appendages may lead to early recurrence. METHODS: This study assesses morphometric aspects of skin appendages in perianal skin and anal canal mucosa in tissues from 30 patients with AIN III. Both normal and dysplastic epithelium was assessed in each patient. The depth to which A

Podophyllin versus scissor excision in the treatment of perianal condylomata acuminata: a prospective study.

Thirty-seven patients with previously untreated perianal condylomata acuminata were randomly treated by application of 25 per cent podophyllin or scissor excision. The result of treatment was assessed by an independent observer. At 6 weeks, scissor excision completely cleared the warts from 16 of 18 patients (89 per cent) compared with 15 of 19 patients (79 per cent) treated with podophyllin. Following complete clearance by surgery, two patients had recurrent warts at 18 weeks and a further single patient at 42 weeks, giving a cumulative recurrence rate of 19 per cent. In contrast, five of 15

Pancreatic metastasis from anorectal malignant melanoma.

This report describes a rare case of anorectal malignant melanoma with metastasis to the pancreas, liver, and small bowel. The patient presented with lower gastrointestinal bleeding and an anal mass. Computed tomography (CT) showed a 4.7 × 6.2 cm mass in the lower rectum. The patient underwent surgery and radiotherapy, achieving a two-year recurrence-free period. However, subsequent CT surveillance revealed a 1.6 cm hypo-dense mass at the head of the pancreas and small liver lesions. An endoscopic ultrasound (EUS)-guided fine-needle biopsy of the pancreatic mass was performed, showing a 2.7 ×

Screening of human papilloma virus-related anogenital cancers in immunocompromised women: a systematic review of clinical practice guidelines.

OBJECTIVE: Human papillomavirus-related anogenital cancers disproportionately affect immunocompromised patients due to poor clearance rates and accelerated oncogenicity. This systematic review compares anogenital cancer screening Clinical Practice Guidelines among different subpopulations of immunocompromised women. METHODS: We conducted a systematic review of guidelines addressing cervical, anal, vaginal, and vulvar cancer screening among immunocompromised women published between 2004 and 2025, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Perianal giant condyloma acuminatum (Buschke-Löwenstein tumor). Case report and review of the literature.

UNLABELLED: Giant condyloma acuminatum, also known as Buschke-Löwenstein tumor (BLT) is a very rare sexually transmitted disease that affects the ano-genital region. BLT is a slow growing cauliflower-like tumor, but unlike simple condyloma, it is locally aggressive and destructive, malignant transformation occuring in 40-60% of cases. We present a case of perianal carcinomatous BLT and discuss some controversies about the nature and treatment of this disease. Our aim is to emphasize some basic points in the management of giant perianal condyloma acuminatum. CONCLUSIONS: The patient with BLT m

Acquired immunodeficiency syndrome-associated cancers in Sub-Saharan Africa.

Sub-Saharan Africa is considered home to more than 60% of all human immunodeficiency virus (HIV) infected cases, with an estimated adult prevalence of 8.0%. It is stated that this region has contributed more than 90% of childhood deaths related to HIV infection and about 93% of childhood acquired immunodeficiency syndrome (AIDS)-related deaths. Although no country in Africa is spared of the infection, the bulk is seen in East and South Africa, with the highest recorded rates of 20% to 50% in Zimbabwe. On the other hand, West Africa is less affected, while countries in Central Africa have relat

Epidemiology of Cancer in Kidney Transplant Recipients.

Kidney transplantation is the ideal treatment modality for patients with end-stage kidney disease, with excellent outcomes post-transplant compared with dialysis. However, kidney transplant recipients are at increased risk of infections and cancer because of the need for immunosuppression. Kidney transplant recipients have approximately two to three times greater risk of developing cancer than the general population, and cancer is a major contributor to morbidity and mortality. Most of the increased risk is driven by viral-mediated cancers such as post-transplant lymphoproliferative disorder,

Increased risk and pattern of secondary malignancies in patients with invasive extramammary Paget disease.

BACKGROUND: Extramammary Paget disease (EMPD) is often associated with underlying or distant synchronous malignancies. The prognosis for affected patients is generally favourable; however, the risk of secondary malignancies is unknown. OBJECTIVES: The goal of the study was to analyse the incidence, prognosis and pattern of secondary malignancies for patients with invasive EMPD using data from the Surveillance, Epidemiology and End Results (SEER) Program. METHODS: We searched the SEER Program database for patients diagnosed with invasive EMPD between 1973 and 2008. Demographic data, outcome a

Adenocarcinoma of the rectosigmoid colon seeding into pre-existing anal fistula.

This paper reports a rare case of rectosigmoid colon carcinoma metastasizing to anal fistula. The patient is a 57-year old man with a 7-year history of a fistula in ano. Colonoscopy revealed a rectosigmoid carcinoma. He underwent high-anterior resection. Two years and 3 months later, a subcutaneous tumor was found at the external opening of fistula. Trans perineal tumor excision plus fistulectomy was performed. Pathology revealed that the colon cancer and the perianal tumor were both moderately differentiated adenocarcinoma. Many reports support the concept of tumor cell implantation in mucosa

[Prognostic factors and management of rare cancers].

The management of rare cancers is controversial. The role of adjuvant therapies, such as chemotherapy and radiation therapy remains unclear. The limited number of patients included in the studies cannot generally allow any consistent statistical analysis to determine either prognostic factors or the impact of surgery and adjuvant therapies on local control and long-term survival. The Rare Cancer Network is a multi-institutional cooperative group created in 1993 to initiate large retrospective studies on rare cancers with the aim of improving our knowledge in terms of management, outcome, and p

[Treatment of condyloma acuminatum with 0.5% 5-fluorouracil-solution, A double-blind clinical trial].

30 males and 29 females with condyloma acuminatum took part in a double-blind randomized trial with 0,5% 5-fluorouracil (5-FU) salicylic acid containing varnish for warts versus the varnish without 5-FU. The females were treated twice weekly at the clinic, the males were treated once daily at home by themselves. The 5-FU solution was found significantly superior to placebo (P less than 0,01). 60% of the 5-FU treated patients were healed with a median healing time of two weeks in the males and three weeks in the females. In the placebo group 27,6% healed with a median healing time of 2,5 weeks

Prevalence of High-Grade Anal Dysplasia and Anal Cancer in Veterans Living With HIV and CD4/CD8 Ratio as a Marker For Increased Risk: A Regional Retrospective Cohort Study.

BACKGROUND: The Department of Veterans Affairs cares for the largest population of patients with HIV of any healthcare system in the United States. Screening for anal dysplasia/cancer is recommended for all veterans with HIV. Exams are invasive, burdensome, and resource intensive. We currently lack markers of disease to tailor screening. OBJECTIVE: The purpose of this study was to establish the prevalence of advanced anal disease (high-grade dysplasia and anal cancer) and to determine whether CD4/CD8 ratio correlates with risk. DESIGN: This was a retrospective regional cohort study of vetera

[Melanoma of the anal margin].

BACKGROUND: Primary anal mucosal melanoma is rare and is associated with a poor prognosis. The observation of a case of anal melanoma at a localized stage in a woman led us to analyze recent data from the literature on therapeutic alternatives. PATIENTS AND METHODS: A 49-year-old woman presented with a pigmented swelling of the anal margin that had begun three months earlier. Complete local excision of the tumour was performed with the conservation of the anal sphincters. Histological examination revealed SSM mucosal melanoma. Abdominoperineal resection was finally performed because of tumour

Sentinel lymph node biopsy progress in surgical treatment of cancer.

BACKGROUND: Forty-three years after the first description of the sentinel lymph node technique in malignant tumours of the parotid by Gould, sentinel lymph node biopsy (SNLB) has become a precious tool in the treatment of solid tumours. METHODS: In the following review we give a synopsis of the fundamentals of the sentinel lymph node concept and then proceed to an overview of recent advances of SNLB in gastrointestinal cancers. RESULTS: In some tumour entities, SNLB has been shown to reflect reliably the lymph node status of the tumour-draining lymph node basin. In melanoma and breast cancer

Human papillomavirus-related malignancies in HIV infection: anal and oropharyngeal cancers.

Human papillomavirus (HPV)-related cancers, including anal cancer and oropharyngeal cancer, occur more frequently in individuals living with HIV infection than in the general population. Strategies for prevention among individuals with HIV infection include HPV vaccination, anal cancer screening programs, and early initiation of antiretroviral therapy (ART). HPV vaccination is not yet optimally used; a stronger and more persistent effort is needed to increase vaccination rates. Although anal cancer screening is not recommended by all authorities, there is a least some evidence that screening a
