Pediatric secondary lens implantation in the absence of capsular support.

PURPOSE: To describe and evaluate alternate techniques for implanting intraocular lenses in children in the absence of adequate capsular support for traditional lens implantation. METHODS: Postoperative results of 18 eyes with posterior chamber intraocular lenses (PCIOLs) sutured to the ciliary sulcus and 10 eyes implanted with anterior chamber intraocular lenses (ACIOLs) were reviewed. Visual outcomes were divided into 2 groups: onset of aphakia during the critical period of visual development (< or = 9 years) and onset after the critical period (> 9 years). Visual outcomes and complications

Acute corneal decompensation after silicone oil removal.

PURPOSE: To assess acute corneal decompensation after silicone oil removal in some aphakic eyes with clear corneas whose anterior chambers were completely filled with silicone oil for a considerable period of time. METHODS: Eight eyes of 8 patients who underwent vitrectomy and intraocular silicone oil injection were studied. All the eyes were aphakic and anterior chambers were completely filled with silicone oil. In all eyes, corneas were clear and no corneal finding indicating keratopathy was detected by slit-lamp microscopy before silicone oil removal. The mean silicone oil removal time was

Secondary posterior chamber intraocular lens implantation in children.

BACKGROUND: Primary intraocular lens (IOL) implantation after cataract aspiration is a widely accepted means of correcting pediatric aphakia. However, little is available in the literature on secondary IOL implantation in children. We present our experience over the past 6 years. METHODS: The charts of 57 aphakic children (61 eyes) who underwent secondary posterior chamber IOL implantation between January 1989 and April 1996 were reviewed. In general, these children were either intolerant of or noncompliant with their contact lenses. An attempt was made to correlate visual outcome with patien

Extracapsular cataract extraction with or without posterior chamber intraocular lenses in eyes with cataract and high myopia.

PURPOSE: To compare the results of insertion of low-power or zero-power posterior chamber intraocular lenses (PC IOLs) after cataract extraction in eyes with high myopia with leaving the eye aphakic. METHODS: The outcome of extracapsular cataract extraction (ECCE) with low-power or zero-power PC IOLs in 184 consecutive highly myopic (axial length = 26.0 mm or longer) eyes was compared with 184 consecutive highly myopic eyes receiving ECCE without PC IOLs. Axial length, preoperative visual acuity, age, sex, and follow-up were similar between both groups. RESULTS: In the pseudophakic eyes, the

Results and complications of secondary intraocular lens implantation.

The results and the postoperative complications of 107 secondary intraocular lens implantations were reviewed over a two-year period. In 84.1% of the cases, postoperative visual acuity was the same or better than preoperative visual acuity with aphakic glasses or contract lenses. The most frequent postoperative complications were secondary glaucoma, six cases (5.6%); cystoid macular edema, five cases (4.6%); and bullous keratopathy, three cases (2.8%).

Extended wear rigid gas permeable lenses used for correction of aphakia.

A study of extended wear rigid gas permeable lenses in aphakic patients was started in 1975. We present an analysis of 186 patients (274 eyes) with from 1 to 194 months (average: 88 months) of extended wear. Patients wore their lenses 24 hours per day, only removing the lenses for periodic cleaning. Approximately one-third of the patients removed their lenses at least once per week; roughly another one-third wore lenses for up to a month between cleanings; and one-third cleaned lenses at intervals up to 6 months. All of the patients in the study achieved vision with their contact lenses compar

The use of contact lenses to correct aphakia in a clinical trial of cataract management.

In a prospective study, 110 eligible patients (111 eyes) were randomly selected to have contact lenses for aphakia (CL). The lens of first choice was a daily-wear hard (DWH) PMMA contact lens. Extended-wear (XW) lenses were fitted to those who failed with DWH lenses. The numbers in the two groups were approximately equal. Eighty per cent of eyes were successfully wearing CLs six months post-operatively and 72% were still wearing them after five years. The mean age of both groups of patients was greater than 70 years but the DWH group was significantly younger (p less than 0.001). However, XW l

Clinical and morphometric results of penetrating keratoplasty with one-piece anterior-chamber or suture-fixated posterior-chamber lenses in the absence of lens capsule.

The clinical records and serial corneal endothelial images of 25 acapsular, pseudophakic eyes with Kelman-style, one-piece, anterior-chamber intraocular lenses and 24 acapsular, pseudophakic eyes with suture-fixated, posterior-chamber intraocular lenses following penetrating keratoplasty were reviewed to determine clinical success and endothelial survival after 1 year. Twenty-two (88%) of 25 grafts in the anterior-chamber intraocular lens group and 23 (96%) of 24 grafts in the sutured posterior-chamber intraocular lens group were clear after 1 year; best corrected visual acuity of 20/40 or bet

The correction of aphakia. XXXVI Edward Jackson Memorial Lecture.

Major advances in the correction of aphakia have been made in the past few years. Extended-wear contact lenses constitute one advance, if not a perfect one. Intraocular lenses are now safer to use, and new research is seeking ways to improve their safety through protection of the corneal endothelium during surgery and stimulation of endothelial healing where damage does occur. Altering the refractive power of the cornea offers another possible approach to the correction of aphakia. Modifications of Barraquer's keratomileusis procedure using preserved, preground corneas may have definite indica

Secondary lens implantation in aphakia: visual results and complications.

We reviewed 70 cases that had undergone secondary lens implantation. The lenses used included 34 Medallion and 36 Choyce Mark VIII lenses. A 20/40 or better vision was present in 74.3% of the cases before implantation, and could be achieved postoperatively in 95.7% of the cases. Corneal changes, i.e. endothelial corneal dystrophy and induced astigmatism, were not noted in cases where Choyce Mark VIII lenses were used. Cystoid macular edema occurred in three cases where surgery followed an intracapsular cataract extraction (5.3%).
