Prevalence of extra-appendage thrombosis in non-valvular atrial fibrillation and atrial flutter in patients undergoing cardioversion: a large transoesophageal echo study.

AIMS: The aim of our study was to evaluate the prevalence of left atrial cavity and appendage thrombosis in patients undergoing cardioversion for non-valvular atrial tachyarrhythmias. In persistent atrial tachyarrhythmias, 90% of thromboses are reported to be located inside the left atrial appendage. This prevalence refers to old studies and meta-analysis in a mixed population of valvular and non-valvular atrial fibrillation. Left atrial cavity thrombosis in non-valvular atrial fibrillation has not been investigated recently in large-scale studies. METHODS AND RESULTS: A total of 1,420 consec…

A low-dose, dual-phase cardiovascular CT protocol to assess left atrial appendage anatomy and exclude thrombus prior to left atrial intervention.

Assessment of the left atrial appendage (LAA) for thrombus and anatomy is important prior to atrial fibrillation (AF) ablation and LAA exclusion. The use of cardiovascular CT (CCT) to detect LAA thrombus has been limited by the high incidence of pseudothrombus on single-pass studies. We evaluated the diagnostic accuracy of a two-phase protocol incorporating a limited low-dose delayed contrast-enhanced examination of the LAA, compared with a single-pass study for LAA morphological assessment, and transesophageal echocardiography (TEE) for the exclusion of thrombus. Consecutive patients (n = 122…

Is long-term warfarin therapy necessary in Chinese patients with atrial fibrillation after bioprosthetic mitral valve replacement and left atrial appendage obliteration?

BACKGROUND: Long-term warfarin therapy has been used to decrease thromboembolic events in patients with atrial fibrillation (AF) following bioprosthetic mitral valve replacement (BMVR) and left atrial appendage obliteration (LAAO). A retrospective study was conducted to investigate the efficacy of long-term warfarin or aspirin therapy in patients with AF after BMVR and LAAO. METHODS: A total of 215 patients with persistent AF were given anticoagulation therapy with warfarin for the first 3 months after BMVR and LAAO, continuing warfarin or aspirin therapy according to the surgeon's preference…

An alternative technique for rechanneling of sinus venosus atrial septal defect with partial anomalous pulmonary venous connection using autogenous right atrial appendage.

We report a new technique for closure of sinus venosus atrial septal defect with high partial anomalous pulmonary venous connection. This technique consisted of preservation of the atriocaval junction, advancement of the posterior rim of the atrial septal defect anterosuperiorly and enlargement of the superior caval vein using right atrial appendage. We found this to be a convenient technique and recommend it for correction of this anomaly.

Usefulness of Doppler assessment of pulmonary vein and left atrial appendage flow following pulmonary vein isolation of chronic atrial fibrillation in predicting recovery of left atrial function.

Atrial fibrillation (AF) is a widespread condition that causes significant morbidity and mortality. Recently, pulmonary venous (PV) isolation using radiofrequency ablation has been used successfully to exclude the pulmonary venous ostia, resulting in correction of AF. Further, miniaturized high-frequency ultrasound phased-array transducers currently provide Doppler and 2-dimensional imaging during the ablation procedure. We examined atrial function and its determinants using intracardiac echocardiography before and after PV isolation in 45 patients who had chronic AF (56 +/- 11 years old). PV,

[Percutaneous left atrial appendage closure: Towards enlargement of implantations in centres without on-site cardiac surgery].

Percutaneous left atrial appendage closure is now an established strategy for thromboembolic prevention in atrial fibrillation. However, the organizational requirements governing its performance remain heterogeneous across regulations and healthcare systems, particularly in France. In this context, it is relevant to assess whether, in experienced centers, an organizational model based on specific expertise and a rapid surgical referral pathway could provide a safe alternative to the presence of on-site cardiac surgery. This article is in favor of extension in France of percutaneous left atrial…

Early discontinuation of antithrombotic therapy following left atrial appendage closure: a systematic review and meta-analysis.

OBJECTIVES: Optimal postprocedural antithrombotic management after left atrial appendage closure (LAAC) remains undefined, particularly in patients with high bleeding risk. Early complete cessation (<6 months) of antithrombotic therapy (AT) following LAAC is increasingly adopted, yet supportive evidence is limited. This study sought to evaluate the safety of early (<6 months) complete discontinuation of AT after LAAC. METHODS: A systematic review and meta-analysis of observational studies was conducted. PubMed and Scopus were searched through September 2025. The primary endpoint was stroke ev…

Safety and Efficacy of Left Atrial Appendage Occlusion in Cardiac Amyloidosis: A Single-Center Retrospective Study.

OBJECTIVE: To evaluate procedural success, safety, and clinical outcomes of left atrial appendage closure (LAAC) in patients with cardiac amyloidosis (CA). BACKGROUND: CA is a progressive, infiltrative cardiomyopathy associated with an elevated risk of thromboembolic events, particularly in the setting of atrial fibrillation (AF). These patients are often elderly and at higher risk of bleeding. While LAAC offers an alternative to oral anticoagulation (OAC) for stroke prevention in high-bleeding-risk AF patients, data on its safety and efficacy in CA remain limited. METHODS: We retrospectivel…

Left Atrial Appendage Morphology and Correlation With Stroke: A Narrative and Three-Dimensional Transesophageal Evaluation Review.

The left atrial appendage (LAA) is the predominant site of thrombus formation in patients with non-valvular atrial fibrillation (AF). Accurate evaluation of LAA anatomy is therefore essential for both stroke risk stratification and procedural planning in LAA occlusion (LAAO). Although clinical scores such as CHA2DS2-VA guide anticoagulation strategies, they do not fully capture individual thromboembolic risk. Growing evidence indicates that LAA morphology and geometry provide incremental prognostic information beyond traditional clinical factors. Multiple studies have consistently demonstrated…

Elevated Factor XI is Associated with First and Recurrent Left Atrial Appendage Thrombus of Unknown Origin.

BACKGROUND: Formation of denser and poorly lysable fibrin networks characterizes patients with left atrial appendage thrombus (LAAT) of unknown origin. Elevated factor (F)XI is associated with thromboembolism, including left ventricular thrombus. We investigated whether FXI is increased in LAAT and might predispose to its recurrence and complications. METHODS: In a case-control study, we studied 36 consecutive patients with LAAT of unknown origin following thrombus resolution, versus 36 age-, sex-, and diabetes-matched controls, all without current anticoagulant treatment. Plasma FXI levels w…
