Cardiometabolic and Endocrine Correlates of Left Atrial Scar in Patients Undergoing Atrial Fibrillation Ablation.

BACKGROUND: Atrial fibrillation (AF) is increasingly conceptualized as a manifestation of atrial cardiomyopathy characterized by progressive fibrosis and structural remodeling. Systemic inflammation may contribute to AF progression and pathogenesis. Elevated inflammatory biomarkers may represent a pro-inflammatory state, playing a possible role in AF. Left atrial (LA) scar burden serves as a surrogate for atrial fibrosis; however, its association with inflammatory markers is less well established. OBJECTIVES: The objectives of the study are as follows: To evaluate a possible association betwe

Human atrial action potential and Ca2+ model: sinus rhythm and chronic atrial fibrillation.

RATIONALE: Understanding atrial fibrillation (AF) requires integrated understanding of ionic currents and Ca2+ transport in remodeled human atrium, but appropriate models are limited. OBJECTIVE: To study AF, we developed a new human atrial action potential (AP) model, derived from atrial experimental results and our human ventricular myocyte model. METHODS AND RESULTS: Atria versus ventricles have lower I(K1), resulting in more depolarized resting membrane potential (≈7 mV). We used higher I(to,fast) density in atrium, removed I(to,slow), and included an atrial-specific I(Kur). I(NCX) and I(

Invited review: hypertension and atrial fibrillation: epidemiology, pathophysiology, and implications for management.

Hypertension affects around half of the adult population worldwide, being one of the most common cardiovascular disorders. On a population basis, high blood pressure is considered to be the major independent risk factor for atrial fibrillation (AF). The incidence of both diseases has increased significantly in the recent decades and it is expected to continuously surge in the following years. Due to close relation between the both diseases and their frequent coexistence, hypertension and AF become major health priorities. The multidirectional linking between raised blood pressure and AF is bas

Transgenic short-QT syndrome 1 rabbits mimic the human disease phenotype with QT/action potential duration shortening in the atria and ventricles and increased ventricular tachycardia/ventricular fibrillation inducibility.

AIMS: Short-QT syndrome 1 (SQT1) is an inherited channelopathy with accelerated repolarization due to gain-of-function in HERG/IKr. Patients develop atrial fibrillation, ventricular tachycardia (VT), and sudden cardiac death with pronounced inter-individual variability in phenotype. We generated and characterized transgenic SQT1 rabbits and investigated electrical remodelling. METHODS AND RESULTS: Transgenic rabbits were generated by oocyte-microinjection of β-myosin-heavy-chain-promoter-KCNH2/HERG-N588K constructs. Short-QT syndrome 1 and wild type (WT) littermates were subjected to in vivo

Early detection of right atrial dysfunction assessed by MR-feature tracking in patients with arrhythmogenic right ventricular cardiomyopathy.

BACKGROUND: The importance of right atrial (RA) function assessment in arrhythmogenic right ventricular cardiomyopathy (ARVC) is increasingly being recognized. MR-feature tracking (MR-FT) has emerged as a novel approach for quantifying atrial function. Therefore, we aimed to evaluate whether early RA dysfunction, even without RA dilation, can be identified in patients with ARVC using MR-FT. METHODS: Sixty-four patients with ARVC, of whom 47 were non-RA dilated and 17 were RA dilated patients, along with 41 healthy controls were included. RA reservoir, conduit, and booster strain (εs, εe, and

Integrative Observational and Genetic Evidence From the UK Biobank Supports Carotid Intima-Media Thickness as a Window Into Cardiac Remodeling.

BACKGROUND: Carotid intima-media thickness (cIMT) is a noninvasive marker of subclinical atherosclerosis linked to future cardiovascular disease (CVD). However, its relationship with early cardiac remodeling and shared genetic basis with cardiovascular disease remain poorly understood. We investigated the phenotypic and genetic associations between cIMT, aortic and cardiac traits, and cardiovascular disease in the UK Biobank, a large population-based cohort with comprehensive imaging and genetics. METHODS: We analyzed 46 102 participants with carotid ultrasound and cardiac magnetic resonance

Carotid Body Ablation Abrogates Hypertension and Autonomic Alterations Induced by Intermittent Hypoxia in Rats.

Chronic intermittent hypoxia (CIH), the main feature of obstructive sleep apnea, enhances carotid body (CB) chemosensory responses to hypoxia and produces autonomic dysfunction, cardiac arrhythmias, and hypertension. We tested whether autonomic alterations, arrhythmogenesis, and the progression of hypertension induced by CIH depend on the enhanced CB chemosensory drive, by ablation of the CB chemoreceptors. Male Sprague-Dawley rats were exposed to control (Sham) conditions for 7 days and then to CIH (5% O2, 12/h 8 h/d) for a total of 28 days. At 21 days of CIH exposure, rats underwent bilatera

Impaired transport function of the left atrium in patients with lone paroxysmal atrial fibrillation.

BACKGROUND: Although lone paroxysmal atrial fibrillation (LPAF) is clinically defined as an arrhythmia that occurs in the absence of structural heart disease, it has been suggested that the presence of anatomical substrate is related to LPAF. The aim of the present study is to determine whether structural and functional remodeling of the left atrium (LA) occurs in patients with LPAF, and to identify whether frequent episodes of atrial fibrillation (AF) contribute to LA remodeling. METHODS AND RESULTS: Forty-five patients who diagnosed as LPAF and age-, gender-, and body mass index-matched hea

SORBS2 regulates diastolic function through cytoskeletal networks and calcium handling.

Diastolic dysfunction, defined by impaired relaxation and increased ventricular stiffness, is central to heart failure with preserved ejection fraction, yet cardiomyocyte-intrinsic mechanisms remain incompletely understood. Here, we show that SORBS2 is a vital component of murine cardiomyocyte adhesion complexes. Its genetic ablation in mice causes progressive diastolic dysfunction with preserved systolic function, accompanied by atrial enlargement and reduced survival. Postnatal cardiomyocyte-directed re-expression of SORBS2 restores diastolic indices and significantly improves longevity. Mec

Complete Left Bundle Branch Block Resolution After Cardiac Resynchronization Therapy With Serial Phonocardiographic Changes.

BACKGROUND: Cardiac resynchronization therapy (CRT) induces structural and electrical reverse remodeling in patients with heart failure. However, resolution of complete left bundle branch block (CLBBB) following CRT has rarely been described, particularly with serial phonocardiographic assessment. CASE SUMMARY: A 67-year-old man with heart failure associated with atrial fibrillation and CLBBB underwent CRT pacemaker implantation following pulsed-field ablation. Initially, the optimal interventricular (VV) delay required left ventricular pre-excitation, but shifted toward right ventricular pre

Human heart-macrophage assembloids mimic immune-cardiac interactions and enable arrhythmia disease modeling.

Yolk-sac-derived embryonic cardiac tissue-resident macrophages (TRMPs) colonize the heart early in development and are essential for proper heart development, supporting tissue remodeling, angiogenesis, electrical conduction, efferocytosis, and immune regulation. We present here a human heart-macrophage assembloid (hHMA) model by integrating autologous human pluripotent stem cell (hPSC)-derived embryonic monocytes into heart organoids to generate physiologically relevant TRMPs that persist long-term and contribute to cardiogenesis. Using single-cell transcriptomics, live imaging, and proteomic

Spatiotemporal Atlas of Heart Development Reveals Blood-Flow-Dependent Cellular, Structural, Metabolic, and Spatial Remodeling.

Embryonic heart development depends on coordinated interactions between cellular programs, molecular signaling, and biomechanical forces, yet how mechanical cues shape cellular and molecular pathways remains incompletely understood. We perturbed cardiac blood flow by partial left or right atrial ligation (LAL/RAL) in chick embryos, generating chamber-specific hemodynamic gain- or loss-of-function states. Using single-cell and unbiased, high-resolution spatial transcriptomics, we generated a spatiotemporal atlas of flow-dependent tissue development, enabling systematic investigations of bidirec

Matrix Metalloproteinase-Targeted SPECT/CT Imaging for Evaluation of Therapeutic Hydrogels for the Early Modulation of Post-Infarct Myocardial Remodeling.

Following myocardial infarction (MI), maladaptive upregulation of matrix metalloproteinase (MMP) alters extracellular matrix leading to cardiac remodeling. Intramyocardial hydrogel delivery provides a vehicle for local delivery of MMP tissue inhibitors (rTIMP-3) for MMP activity modulation. We evaluated swine 10-14 days following MI randomized to intramyocardial delivery of saline, degradable hyaluronic acid (HA) hydrogel, or rTIMP-3 releasing hydrogel with an MMP-targeted radiotracer (99mTc-RP805), 201Tl, and CT. Tips for management of arrhythmias in endocrine disorders from an European Heart Rhythm Association position paper.

In endocrine diseases, hormonal changes, electrolyte abnormalities, and the deterioration of heart structure can lead to various arrhythmias. In diabetic patients, hypoglycemia, hyperglycemia, and hypokalemia can trigger arrhythmias, and diabetic cardiomyopathy can also cause electrical and structural remodeling to form substrates for arrhythmias. The risk of atrial fibrillation (AF) increases in hyperthyroidism; however, the prevalence of ventricular arrhythmias in hypothyroidism is higher. Besides AF and ventricular tachycardias, bradycardias and atrioventricular blocks can also be seen in p

Conditional dicer gene deletion in the postnatal myocardium provokes spontaneous cardiac remodeling.

BACKGROUND: Dicer, an RNAse III endonuclease critical for processing of pre-microRNAs (miRNAs) into mature 22-nucleotide miRNAs, has proven a useful target to dissect the significance of miRNAs biogenesis in mammalian biology. METHODS AND RESULTS: To circumvent the embryonic lethality associated with germline null mutations for Dicer, we triggered conditional Dicer loss through the use of a tamoxifen-inducible Cre recombinase in the postnatal murine myocardium. Targeted Dicer deletion in 3-week-old mice provoked premature death within 1 week accompanied by mild ventricular remodeling and dram
