Pseudosimultaneous fast and slow pathway conduction: a common electrophysiologic finding in patients with dual atrioventricular nodal pathways.

Two ventricular responses following termination of rapid atrial pacing were noted in 24 of 87 patients with dual atrioventricular (AV) nodal pathways and supraventricular tachycardia. In all 24 patients, the AH intervals of the first and second ventricular responses were comparable with those of the fast and slow pathways, respectively. Careful analysis of the whole pacing sequence revealed that, in 21 patients, this phenomenon resulted from sustained slow pathway conduction with long AH intervals. In these patients, as the AH interval of each paced beat was progressively lengthened during pac

Standard diagnostic programmed electrical stimulation protocols in patients with paroxysmal recurrent tachycardias.

Programmed stimulation, as a diagnostic and therapeutic modality in arrhythmia evaluation, suffers from lack of standardization. If the sensitivity, specificity, and predictive value of this procedure are to be considered seriously, then an evaluation of various stimulation protocols is warranted. Study variables are numerous and influence the outcome of the studies.

Delayed termination of re-entrant atrioventricular nodal tachycardia.

Termination of atrioventricular nodal (AVN) re-entrant tachycardia by one or two induced premature beats generally occurs within one tachycardia cycle from the last premature beat. Two cases are described in which programmed stimulation during sustained re-entrant AVN tachycardia caused delayed termination in the second or third tachycardia cycle following the extrastimuli. The site of block was the antegrade pathway in one case and the retrograde pathway in the other. The most likely mechanism was induced second degree block in one limb of the tachycardia circuit. Delayed termination provided

[Ventricular response in atrial fibrillation : normal conduction, consequences of conduction disorders and preexcitation syndromes. Therapeutic implications].

Atrial fibrillation is the most common arrhythmia in man. It is characterized by: 1) a rapid and irregular atrial electrical activity, 2) an irregular ventricular response which follows complex laws in the absence of advanced disorder of atrio-ventricular conduction. The study of these laws of complex transformation which govern the ventricular response can be approached by histograms of the RR interval: the shortest interval between two QRS complexes corresponds approximately to the functional refractory period of the atrio-ventricular conduction pathways--the longest interval corresponds to

Nodoventricular accessory atrioventricular connection associated with dual atrioventricular pathways: a case report and review of the literature.

A case is presented of a patient free from tachyarrhythmias or prior evidence of pre-excitation syndrome, in whom electrophysiologic study revealed evidence of dual AV nodal pathways associated with a nodoventricular bypass tract. Programmed atrial extrastimuli testing revealed that conduction through the fast AV nodal pathway was associated with a normal HV interval and normal QRS morphology. Conduction through the slow AV nodal pathway was always associated with a short HV interval and ventricular pre-excitation. No tachyarrhythmias could be induced with atrial extrastimuli or rapid atrial p

[Auriculo-Hisian conduction of patients with bundle branch block. Clinical significance].

The clinical and electrophysiological data in 52 consecutive patients with bundle branch block and followed-up for an average period of 20.8 … (321 +/- 111 ms) than in Group A (246 +/- 59 ms) (p < 0.05). The effective refractory period of the atrioventricular node was also significantly longer in Group B (492 +/- 190 ms) than in Group A (333 +/- 125 ms (p < 0.05). On the other hand, there was no significant difference in the HV interval or in the number of patients managed by permanent pacing.

[Intranodal reciprocal dual rhythm].

In a patient with spells of paroxysmal supraventricular tachycardia of two types, the stimulation tests demonstrated a dual intranodal conduction responsible sometimes for "usual" reciprocal rhythms (slow anterograde pathway and rapid retrograde pathway), sometimes for "reversed" reciprocal rhythms (rapid anterograde pathway and slow retrograde pathway). The latter contrasted with the lack of discontinuity of the retrograde nodal function curve (V1-V2 versus A1-A2).

Unusual mechanism in the initiation of the paroxysmal supraventricular tachycardia in a patient with WPW syndrome.

A heretofore unreported unusual mechanism in the initiation of the paroxysmal supraventricular tachycardia (PSVT) in a patient with WPW (Wolff-Parkinson-White) syndrome was observed using His bundle recordings. The patient initially had some degree of AV conduction disturbance at the level of the AV node. A premature atrial impulse initially activated the ventricle exclusively through the accessory pathway and the same impulse re-excited the ventricle via the AV nodal-His axis after finishing the pure pre-excitation with marked prolongation of the AH and the HV intervals. After finishing this

Paroxysmal supraventricular tachycardia with unusual induction. Concealed reentry or automaticity?

In a patient with documented paroxysmal supraventricular tachycardia, earlier atrial extrastimuli consistently induced His-ventricle (H-V) block and "atrioventricular junctional" beats, which were always followed by an echo or paroxysmal supraventricular tachycardia. In "atrioventricular junctional" beats, a V wave was preceded by an H deflection with the same intervals as that of the sinus beat. As for the underlying mechanism of paroxysmal supraventricular tachycardia, two possibilities were considered: (1) concealed atrioventricular nodal reentry, and (2) "triggered automaticity."

An experimental study on canine A-V junctional pacemaker--evaluation of the automaticity by overdrive suppression and autonomic blockade by practolol and atropine.

1. The automaticity of the A-V junction was evaluated in 15 awake dogs with experimentally induced A-V junctional rhythm. 2. The spontaneous heart rate of these dogs ranged from 54 to 112 beats a minute, showing about 1.5-2.5 times slower than that of control dogs with sinus rhythm. The duration of asystole after overdrive in these dogs prolonged significantly in accordance with an increase in the frequency of stimulation for overdrive, and its mean +/- SD attained 4.7 +/- 1.1 seconds after overdrive at a rate of 2.5 times the spontaneous heart rate. 3. By administration of atropine (0.04 mg/k

The conduction system in corrected transposition with situs inversus.

The location and course of the conduction system were investigated by serial section in two hearts with corrected transposition of the great arteries in situs inversus. In the first case, characterized by dextrocardia, the interventricular septum was intact, while in the second case with levocardia, a high ventricular septal defect was associated with pulmonary atresia. In both hearts, a regular posterior connecting AV node was present, left-sided in one case and right-sided in the other. The bundle branches were distributed in the morphologically appropriate ventricles. Extensive hemorrhage a

Congenital Complete Heart Block Requiring Temporary Pacemaker Placement at Birth: A Case Report.

Congenital heart block (CHB) is a rare congenital cardiac disease where disruption of the atrioventricular (AV) node results in cardiac conduction abnormalities. The majority of CHB cases are associated with neonatal lupus antibodies that are associated with autoimmune destruction and fibrosis of the AV node, most commonly resulting in a congenital complete heart block (CCHB). We report the case of a 23-year-old mother who was found to have a fetus with significant bradycardia at a 21-week anatomy ultrasound. Subsequent fetal echocardiograms demonstrated a third-degree heart block, and laborat

Lyme carditis manifesting with sinoatrial exit block: a case report.

BACKGROUND: Lyme disease is the most common vector-borne disease in North America and Europe. Infection with the spirochete Borrelia burgdorferi complex can involve cardiac tissue causing Lyme carditis (LC). Due to the infection of conductive tissue, LC typically presents with varying degrees of atrioventricular conduction block. Here, we provide the first evidence that conductive tissue of the sinus node can be involved in LC resulting in higher degree sinoatrial (SA) block with concomitant syncope. CASE SUMMARY: We report the case of an otherwise healthy 31-year-old female presenting with L

Polymerase chain reaction analysis revealed that in the sinus node, the atrioventricular node, and the bundle branches, 35 to 40% of total mtDNA molecules harbored the common deletion. In contrast, in the contractile myocardium, 10 to 20% of total mtDNA was deleted (P = .05). These results demonstrate that in Kearns-Sayre syndrome, the condu …
