Challenges and controversies in fetal diagnosis and treatment: hypoplastic left heart syndrome.

Today, almost 70% of babies with hypoplastic left heart syndrome (HLHS) will survive into adulthood, although significant long-term morbidity and mortality still exists. Prenatal diagnosis of HLHS is increasingly common, allowing improved counseling, and the potential for fetal intervention if indicated. Exciting progress continues to be made in the area of fetal diagnosis and intervention, specifically catheter intervention for intact atrial septum or severe aortic stenosis. Pediatric cardiologists should be keenly aware of the flaws of staged palliation for the treatment of HLHS, and need to…

Infective endocarditis: therapeutic options and indications for surgery.

Infective endocarditis is a serious, life-threatening condition with mortality of 30% at one year. Established treatment is a combination of anti-microbial therapy and close interface between multiple specialist teams of cardiologists, microbiologists and cardiac surgeons to ensure availability of early surgery to those patients who require it. There are evidence-based established indications for surgery and a shifting body of evidence advocating earlier surgical intervention. The development of complications is often the driving cause of referral for surgical intervention. Here we discuss the…

Cardiac infections: focus on molecular diagnosis.

The role of different types of infections in heart diseases is more important than commonly thought, with new and re-emerging infections (i.e., Mycobacterium tuberculosis). This review addresses the pathology of infective pericarditis, myocarditis, and endocarditis, mainly focusing on the significance of molecular techniques in the detection of infective agents. Molecular investigations represent important ancillary diagnostic tools and combined with other conventional approaches provide a more precise final diagnosis. A close collaboration and communication among cardiologists, cardiac surgeo…

Successful angioplasty of an occluded aberrant coronary artery: a rare cause of acute myocardial infarction.

A 48-year-old woman was admitted for an acute infero-lateral myocardial infarction (AMI). The coronary angiography showed an occluded aberrant left circumflex artery taking off from the right sinus of Valsalva of the aortic root. This rare coronary anomaly represents a challenge for interventional cardiologists, especially in the setting of AMI, since it may be difficult to identify an aberrant occluded vessel, to predict its origin and course and finally to choose the material which offers an adequate support during the revascularization procedure. In this particular case, the percutaneous tr…

Usefulness of clinical assessment of the carotid pulse in the diagnosis of aortic stenosis.

In a prospective controlled study, we compared the blinded assessment of the carotid pulse (independent of other clinical signs) by 1 of 3 cardiologists with applanation tonometry on the radial and carotid pulse (time to dominant peak and maximum rate of rise [-dP/dt]). We studied 23 patients with significant isolated aortic stenosis (AS) and 23 control patients. The odds ratio of finding a slowly rising pulse, representing AS, compared with a finding of a "normal"/"indeterminate" pulse was 10.4. Radial dP/dt was significantly lower in the group with AS, whereas carotid dP/dt was not significa…

Cardiovascular problems and diving.

In the US the Diving Alert Network has a list of physicians for medical consultation. I am one of the cardiologists on the list and I get about a call a day about some cardiac question that raises issues of safety or fitness for diving. I will discuss the commoner categories such as coronary artery disease, congenital heart disease, valvular disorders, arrhythmias, conduction disorders, pacemakers, some of the common cardiac drugs used in the treatment of hypertension and some other relatively benign medical conditions that raise questions about the use of the drugs rather than the condition i…

A review of liver transplantation for gastroenterologists.

The 1980s will be remembered as the era of transplantation. During this period both liver and heart transplantation have their places as established therapeutic procedures for selected patients with end stage organ failure. As increasing numbers of patients are transplanted, others practicing physicians, especially gastroenterologists and cardiologists, will need to become familiar with all aspects in the care of patients that they have referred for transplantation. Only through close cooperation between transplant surgeons and this new breed of "transplant internists" will all suitable patien…

[Proposal for a diagnostic and therapeutic care pathway of the Piedmont Region on lipid-lowering and antithrombotic treatment in patients with peripheral arterial disease].

The aim of this paper is to present the diagnostic and therapeutic care pathway on peripheral arterial disease, recently developed in the Piedmont Region, Italy. It proposes a combined approach between the cardiologist and vascular surgeon for optimizing the treatment of patients with peripheral artery disease, which includes the most recently approved antithrombotic and lipid-lowering drugs. The goal is to promote a greater awareness on peripheral vascular disease, in order to implement its treatment patterns and consequently to perform an effective secondary cardiovascular prevention.

Amiodarone-induced thyrotoxicosis.

We see an increasing number of patients with amiodarone-induced thyrotoxicosis. This condition can be treated pharmacologically, but treatment over several months may give rise to adverse reactions. In most cases we recommend that amiodarone therapy be continued despite newly detected thyrotoxicosis. Particularly in cases of heart failure, one should not wait too long before considering thyroidectomy. Treatment of amiodarone-induced thyrotoxicosis must be delivered with close collaboration between endocrinologist and cardiologist.

Cocaine induced myocardial infarction.

The case of a 29 year old man who presented with chest pains after the use of cocaine is reported. The diagnosis of myocardial infarction was made on the electrocardiogram changes and lactate dehydrogenase profile. The diagnosis may be overlooked if there is no direct questioning about the use of drugs such as cocaine. Diazepam has an important role in the management of cardiac complications after cocaine use and should be used early in management. The use of thrombolysis should be a joint decision between the emergency physician and cardiologist.

[Diagnosis and treatment of fetal and neonatal tachycardia].

Severe intrauterine cardiac failure due to supraventricular tachycardia in a 29 week fetus was diagnosed using 2 dimensional and Doppler echocardiography. The prenatal tachycardia was successfully treated, the postnatal finding suggested Wolff-Parkinson-White syndrome. The fetal echocardiography by providing information on cardiac structure and haemodynamics may provide diagnostic and therapeutic useful informations. For the optimal pre- and perinatal management of this life-threatening condition absolutely essential a brief collaboration between the pediatric cardiologist and obstetricians.

[Aneurysm of the membranous heart septum].

Cardiac membranous septal aneurysm is a rare congenital abnormality which occurs in 0.3% of patients as suspected congenital heart disease and in 19.1% as suspected ventricular septal defect. The main method for its diagnosis is biplane cineangiocardiography along with ventricular contrast injection. The patients with membranous septal aneurysm without hemodynamic abnormalities, complications, and concomitance with other congenital heart diseases require outpatient cardiologist's observation. Hemodynamic abnormalities, complications, and concurrent heart diseases are indications for surgery.

Combined mitral and aortic stenosis in parturient: Anesthesia management for labor and delivery.

Maternal heart disease complicates 0.2-3% of pregnancies. The optimal management of the pregnant patient with cardiac disease depends on the co-operative efforts of the obstetrician, the cardiologist and the anesthesiologist involved in peripartum care. A comprehensive understanding of physiology of pregnancy and pathophysiology of underlying cardiac disease is of primary importance in provision of obstetric analgesia or anesthesia for these high-risk groups of patients. We report a successful and uncomplicated use of epidural anesthesia for labor and delivery in patient with combined aortic a

A Review of Wolff-Parkinson-White for the General Practitioner.

Wolff-Parkinson-White (WPW) syndrome was first described nearly a century ago and has been found to have a relatively high prevalence within the general population. The risk of sudden cardiac death due to a high-risk accessory pathway in pediatrics is substantially higher than in adults necessitating further risk stratification to identify and treat these high-risk patients. It is of upmost importance for the general practitioner to be able to identify patients with WPW, which is often diagnosed incidentally, and refer to a cardiologist for further evaluation.

[A little bit about sphingolipidoses in cardiology: a clinical case of Fabry disease].

The article presents a clinical case of Fabry disease in a woman, characterized by multisystemic lesions, late onset and predominant clinical picture of heart failure. The features of this pathology are described in detail with an emphasis on instrumental studies of the cardiovascular system, and the progressive course of Fabry disease is analyzed. This clinical observation illustrates the importance of family screening and detailed anamnesis, shows the process of supervision of a patient with an orphan disease by a cardiologist of a regional hospital in real clinical practice, thereby increas

Red Flags Suggesting Cardiac Transthyretin Amyloidosis (ATTR) in Clinical Practice.

Cardiac wtATTR is caused by extracellular deposition of misfolded proteins in the heart. It mostly affects elderly men and is still clearly underdiagnosed. Recognizing red flags suggesting wtATTR is key for a timely diagnosis, enabling the patient to profit from effective therapies. If general practitioners suspect cardiac amyloidosis, it is crucial to rapidly exclude AL-amyloidosis by immunoelectrophoresis, immunofixation as well as light-chain assay, because AL-amyloidosis needs urgent hematologic therapy. After that, the patient should be referred to the cardiologist for further assessment.

Treatment of calcified coronary artery lesions.

Heavily calcified coronary plaques represent a complex lesion subset and a challenge to the interventional cardiologist, as they are often resistant to simple plaque modification with conventional balloon angioplasty. Inadequate plaque modification can lead to stent underdeployment, which itself predisposes to in-stent restenosis and stent thrombosis. Over the years, a number of mechanical devices ranging from modified angioplasty balloons to atherectomy devices have become available in order to tackle such lesions. Here we review these devices concentrating on the evidence behind their use.

Diagnosis and management of hypertrophic cardiomyopathy.

The clinical spectrum of hypertrophic cardiomyopathy (HCM) is complex and includes a variety of phenotypes, which leads to different types of manifestations. Although most of the patients are asymptomatic, a significant proportion of them will develop symptoms or risk of arrhythmias and sudden cardiac death (SCD). Therefore, the objectives of HCM diagnosis and management are to relieve the patients' symptoms (chest pain, heart failure, syncope, palpitations, etc.), prevent disease progression and major cardiovascular complications and SCD. The heterogeneity of HCM patterns, their symptoms and
