Total laparoscopic radical antegrade modular pancreato-splenectomy with left-posterior superior mesenteric artery first-approach for distal pancreatic cancer: step-by-step technique with a surgical case report (with video).

INTRODUCTION: Left-sided pancreatic cancers are uncommon but seem more aggressive than tumors of pancreatic head. Radical antegrade modular pancreato-splenectomy (RAMPS) was proved to have safe and effective advantages while comparing with standard retrograde pancreato-splenectomy (SRPS) in recent systematic literature reviews and meta-analyses. Laparoscopic SMA first-approach RAMPS was recently proceeded with optimistic perioperative outcomes. CASE PRESENTATION: Our patient is a 67-year-old female with a medical history of diabetes and hypertension, recruited because of upper left quadrant a

An intriguing new diapsid reptile with evidence of mandibulo-dental pathology from the early Permian of Oklahoma revealed by neutron tomography.

The initial stages of diapsid evolution, the clade that includes extant reptiles and the majority of extinct reptilian taxa, is surprisingly poorly known. Notwithstanding the hypothesis that varanopids are diapsids rather than synapsids, there are only four araeoscelidians and one neodiapsid present in the late Carboniferous and early Permian. Here we describe the fragmentary remains of a very unusual new amniote from the famous cave deposits near Richards Spur, Oklahoma, that we recognize as a diapsid reptile, readily distinguishable from all other early amniotes by the unique dentition and l

Development of a postoperative occlusive thrombus at the site of an implanted inferior vena cava filter: A case report.

RATIONALE: Although an inferior vena cave (IVC) filter is placed to prevent fatal pulmonary embolism (PE), several complications associated with an IVC filter have been reported. We describe a case with symptomatic PE, of which the origin was an occlusive IVC thrombus that developed from the placement of an IVC filer after a laparoscopy-assisted total gastrectomy (LATG). PATIENT CONCERNS: A 71-year-old man underwent LATG under general anesthesia alone. He had an IVC filter implanted 13 years ago. An intravenous infusion of unfractionated heparin was substituted for the discontinuation of oral

Anterior tooth root morphology and size in Neanderthals: taxonomic and functional implications.

Comparing modern humans and Neanderthals, we have previously shown that recent modern humans (RMH) and Neanderthals differ in anterior root lengths, and that this difference cannot be explained by group differences in overall mandibular size. Here, we first document the evolutionary changes of root size and shape of the anterior upper and lower dentition in a broad chronological and geographical framework. We then use the size and shape differences between RMH and Neanderthals to classify several isolated teeth from Kebara cave and Steinheim, and to interpret the anterior tooth roots of the Ta

Evaluation of DNA damage in a population of bats (Chiroptera) residing in an abandoned monazite mine.

Ionising radiation has the ability to induce DNA damage. While the effects of high doses of radiation of short duration have been well documented, the biological effects of long-term exposure to low doses are poorly understood. This study evaluated the clastogenic effects of low dose ionising radiation on a population of bats (Chiroptera) residing in an abandoned monazite mine. Bats were sampled from two chambers in the mine, where external radiation levels measured around 20 microSv/h (low dose) and 100 microSv/h (higher dose), respectively. A control group of bats was sampled from a cave wit

An Anatomic Study of the Endoscopic Orbitozygomatic Approach for Extradural Access to the Orbit, Cavernous Sinus, and Infratemporal Fossa.

BACKGROUND AND OBJECTIVES: Access to the orbit, cavernous sinus, Meckel cave (MC), and infratemporal fossa (ITF) remains challenging due to the complex 3-dimensional anatomy and proximity to critical neurovascular structures. An extradural endoscopic orbitozygomatic (OZ) approach has the potential to provide a direct surgical corridor across these compartments while minimizing the limitations of compartment-specific corridors. METHODS: Five formalin-fixed, silicone-injected cadaveric heads (10 sides) were dissected using an extradural OZ approach under operative microscopy supplemented with 0

360° to the Petrous Apex: Comprehensive Surgical Anatomy and Limitations of Open and Endoscopic Endonasal Approaches to the Petrous Apex.

BACKGROUND AND OBJECTIVES: Contemporary skull base surgeons must understand transcranial and endoscopic endonasal approaches to the petrous apex (PA). We provide an anatomic overview and comparison of main approaches to the PA through illustrative anatomic dissections. METHODS: On 10 sides of 5 specimens, transcranial approaches to the PA including the anterior petrosal, transcochlear, and retrosigmoid with suprameatal extension were performed. For endoscopic endonasal approaches, the transclival approach was performed in the midline of 5 specimens and its contralateral transmaxillary extensi

Gastrointestinal Helminths of Three Bat Species in Northern Iran: A Preliminary Molecular and Morphological Assessment.

PURPOSE: Bats harbor a diverse spectrum of gastrointestinal parasites, including trematodes, cestodes, and nematodes. This study aimed to investigate the prevalence and diversity of gastrointestinal helminths in bats from cave and wetland environments in northern Iran, where bat parasitology remains largely unexplored. The research seeks to expand the regional records of bat parasites and establish a parasitological baseline for future ecological and epidemiological studies. METHODS: A parasitological survey was conducted on 13 individuals from three bat species (Miniopterus pallidus, Rhinolo

First report of pathogenic Leptospira in Sri Lankan bats: A potential reservoir risk?

BACKGROUND: The genus Leptospira, classified under the phylum Spirochaetes, includes saprophytic, intermediate, and pathogenic species. Pathogenic Leptospira spp. are the causative agents of leptospirosis, a widespread and often neglected zoonotic disease that causes severe illness in humans, particularly in tropical and subtropical regions. In Sri Lanka, leptospirosis causes annual outbreaks, especially during the monsoon seasons. While rodents are recognized as primary reservoirs, bats have also been identified as potential reservoir hosts. This study aimed to detect and characterize Leptosp

The Key to Percutaneous Balloon Compression Success: Pear or Banana?

OBJECTIVE: The percutaneous balloon compression (PBC) procedure treats trigeminal neuralgia by compressing the Gasserian ganglion in Meckel's cave. On lateral fluoroscopy, a pear-shaped contrast-filled structure mainly consists of regenerative trigeminal rootlets, with a smaller, crescent-shaped, nonrenewable ganglion ('banana') at its base. To effectively minimize the risk of recurrence, it is crucial to apply precise and targeted pressure directly to the ganglion. To demonstrate that the bottom-boosted PBC process may effectively contribute to reducing recurrence. METHODS: From January 2019

Extradural anterior clinoidectomy versus endoscopic transplanum-transcavernous approach to the paraclinoid region: quantitative anatomical exposure analysis.

BACKGROUND: Transcranial anterior clinoidectomy is a conventional microsurgical approach for treatment of paraclinoid aneurysms. The endoscopic endonasal approach (EEA) is an alternative method for clipping intracranial aneurysms. No analysis has been conducted to anatomically compare approaches with respect to treating paraclinoid aneurysms. The surgical anatomical exposures of the paraclinoid region during transcranial extradural anterior clinoidectomy (EAC) and the endoscopic endonasal transplanum-cavernous approach (EETC) are described and quantitatively assessed. METHOD: Seven cadaveric

The Effect of Trigeminal Ganglion Block on Trigeminocardiac Reflex in Elderly Patients with Trigeminal Neuralgia Undergoing Percutaneous Balloon Compression: A Randomized Controlled Study.

BACKGROUND: Trigeminal neuralgia (TN) is a facial neuropathic pain, which is defined as unilateral brief shock-like paroxysmal pain. Percutaneous balloon compression (PBC) was widely used to treat TN under general anesthesia. However, trigeminocardiac reflex (TCR) as a brainstem reflex can induce bradycardia or even cardiac arrest during PBC, which may result in catastrophic consequences in elderly patients. The aim of the present study was to investigate the effect of trigeminal ganglion block on TCR in elderly patients with TN during PBC. MATERIALS AND METHODS: Eighty-two elderly patients u

Pain management for the Nuss procedure: comparison between erector spinae plane block, thoracic epidural, and control.

OBJECTIVE: Pectus excavatum is a congenital deformity characterized by a caved-in chest wall. Repair requires surgery. The less invasive Nuss procedure is very successful, but postoperative pain management is challenging and evolving. New pain management techniques to reduce opioid reliance include the erector spinae plane (ESP) block. We retrospectively examined opioid consumption after Nuss procedure comparing three pain management techniques: ESP block, thoracic epidural (TE), and patient-controlled analgesia (PCA). METHODS: This retrospective cohort study compared pain management outcomes
