Occipitocervical Fixation: A Single Surgeon's Experience With 120 Patients.

BACKGROUND: Occipitocervical junction instability can lead to serious neurological injury or death. Open surgical fixation is often necessary to provide definitive stabilization. However, long-term results are limited to small case series. OBJECTIVE: To review the causes of occipitocervical instability, discuss the indications for surgical intervention, and evaluate long-term surgical outcomes after occipitocervical fixation. METHODS: The charts of all patients undergoing posterior surgical fixation of the occipitocervical junction by the senior author were retrospectively reviewed. A total

Clinical presentation and management of proatlas segmentation defect presenting with palatal myoclonus: case report.

Clinical presentation of craniovertebral junction disorders may range from acute catastrophic neurological deficits to insidious signs and symptoms that may mask the underlying etiology. Prompt recognition and treatment is essential to avert long-term neurological morbidity. Proatlas segmentation disorders are a rare group of developmental disorders involving the craniocervical junction. Abnormal bony segmentation leads to malformed bony structures that can in turn lead to neurological deficits through bony compression of the cervicomedullary junction. This report details a proatlas segmentati

Partial midfacial duplication.

A case of craniofacial duplication is presented. Details on this rare form are described, and its treatment is discussed with brief review of the pertinent literature. We excised the duplicate maxilla and also discovered bilateral macrostomia. A year later, a mass appeared again. By CT scan, we found that there was a mass in the skull base extruding to the superior wall of cavitas pharyngis. The mass below the sphenoid bone and the ethmoid bone connected with the skull base. CT scan also showed malformation of the first cervical vertebra and odontoid process had bifurcated. Once again, we exci

A case of Collet-Sicard syndrome associated with traumatic atlas fractures and congenital basilar invagination.

An 18 year old man with congenital basilar invagination developed multiple lower cranial nerve (CN) palsies including CN IX to XII after a traffic accident. Computed tomography of his skull base revealed a two part atlas Jefferson fracture. Normally, lower cranial nerves (CN IX-XII) pass through a space between the styloid process and the atlas transverse process. Atlas burst fractures rarely cause neurological deficits because of a greater transverse and sagittal diameter of the spinal canal at the atlas, and a tendency of the lateral masses to slide away from the cord after injury. However,

[Local anomalies and secondary degenerative-dystrophic damage in the craniovertebral region].

Clinical and x-ray studies of patients with focal anomalies of the osteoarticular apparatus of the craniovertebral area demonstrated that half of the cases showed early complications in the form of secondary degenerative-dystrophic lesions. This process is aggravated proportionally to the degree of pronounced interrelation disturbances and overstrain in the craniovertebral area. The secondary degenerative-dystrophic lesions indicate disordered compensation and significantly aggravate the clinical state and working capacity of the patients.

Living in a food desert in Louisiana and its effects on gynecologic cancer outcomes.

OBJECTIVE: Food insecurity is becoming recognized as an important measure of public health. Louisiana has a poorer health index and a higher food insecurity rate than the national average. This study aims to investigate how living in a food desert affects the stage at diagnosis and 5-year overall survival in patients with gynecologic cancers. METHODS: Data on genital cancers diagnosed between 2015 and 2019 among Louisiana women aged 20 years and older were from the Louisiana Tumor Registry. The food desert data was based on the USDA Food Access Research Atlas (FARA). The covariates included i

Dystopic Os Odontoideum Causing Cervical Myelopathy: A Rare Case Report and Review of Literature.

Os odontoideum (OO) was first described by Giacomini in 1886 as separation of the odontoid process from the body of the axis. Instability can consequently occurs at this level due to the failure of the transverse atlantal ligament (TAL) and this atlantoaxial instability can be a cause of progressive neurological deficits. It is considered a rare anomaly of the odontoid process. It is a disease with controversial etiology, debatable incidence, and only a partly known natural history owing to the paucity of the literature on this topic. There are insufficient demographic data about the occurrenc

Molecular markers for cervical cancer prognosis: a prognostic model using stemness-related lncRNAs and functional analysis of EMX2OS.

BACKGROUND: Cancer stemness-related long non-coding RNAs (lncRNAs) play a crucial role in tumor initiation and progression. This study aimed to identify stemness-related lncRNAs in cervical cancer (CESC) and evaluate the prognostic significance, clinical relevance, and biological functions. MATERIALS & METHODS: Transcriptome data from the Cancer Genome Atlas were used to construct a co-expression network of cancer stemness-related genes and CESC-specific lncRNAs, focusing on cervical squamous cell carcinoma and endocervical adenocarcinoma. Identified stemness-related lncRNAs were used to devel

Nocardia rubra cell wall skeleton-induced MARCO expression: implications for improved phagocytosis and cytokine secretion in tumor-associated macrophages.

INTRODUCTION: Nocardia rubra cell wall skeleton (Nr-CWS) demonstrates a significant therapeutic effect against human papillomavirus (HPV) infection, but its precise immunomodulatory mechanisms warrant further investigation. This study investigates how Nr-CWS influences tumor-associated macrophages (TAMs) immune functions through macrophage receptor with collagenous structure (MARCO)-mediated mechanisms. METHODS: Cervical tissues of three cervical intraepithelial neoplasia (CIN) patients receiving Nr-CWS monotherapy and HPV infection turning negative were collected before and after treatment,

Multi-Omics Analysis of CDKN2A (p16INK4a) in Cervical Carcinoma in the Context of Human Papillomavirus and in Endometrial Carcinoma.

BACKGROUND: CDKN2A (p16^INK4a^) is integral to the regulation of the RB-E2F cell-cycle checkpoint and is widely acknowledged as a surrogate marker for high-risk human papillomavirus (HPV)-related cervical neoplasia. Nevertheless, its diagnostic and prognostic significance in uterine corpus endometrial carcinoma (UCEC), a predominantly HPV-independent malignancy, remains inadequately characterized. This study utilized an integrated multi-omics approach to examine CDKN2A dysregulation in cervical squamous cell carcinoma (CESC) and UCEC. METHODS: Pan-cancer and tumor-normal differential expressi

A case report of microvascular decompression for cervical myelopathy and radiculopathy caused by tortuous and abnormal bilateral vertebral artery.

BACKGROUND: Tortuous/abnormal vertebral arteries (VAs) sometimes cause neurovascular compression syndromes (NVCs), such as trigeminal neuralgia, hemifacial spasm, and, rarely, myelopathy/radiculopathy. Abnormalities/tortuosity of the VA at the level of the atlas and axis are of particular note; these may be characterized by a persistent first intersegmental artery (PFIA) and C2 segmental type of VA. Herein, we report a 72-year-old male who presented with cervical myelopathy/radiculopathy due to bilateral tortuosity of the PFIA resulting in spinal cord compression at the craniocervical junction

Clinical and quantitative analysis of patients with crowned dens syndrome.

BACKGROUND: Crowned dens syndrome (CDS) is a radioclinical entity defined by calcium deposition on the transverse ligament of atlas (TLA). In this study, the novel semi-quantitative diagnostic criteria for CDS to evaluate the degree of calcification on TLA by cervical CT are proposed. METHOD: From January 2010 to September 2014, 35 patients who were diagnosed with CDS by cervical CT were adopted as subjects in this study. Based on novel criteria, calcium deposition on TLA was classified into "Stage" and "Grade", to make a score, which was evaluated semi-quantitatively. The correlation between

Clinical and Radiographic Outcomes of C1 Laminectomy Without Fusion in Patients With Cervical Myelopathy That Is Associated With a Retro-odontoid Pseudotumor.

PURPOSE: A retro-odontoid pseudotumor that is not associated with rheumatoid arthritis or hemodialysis is clinically rare. The majority of surgeons select transoral resection as the surgical treatment, often followed by posterior fusion or posterior decompression and fusion. In contrast, some authors have reported success with simple decompression without posterior stabilization in cases where atlanto-axial instability (AAI) is either absent or minor. In this study, we have evaluated the clinical and radiographic outcomes of C1 laminectomy without fusion as the surgical treatment for patients
