Isolated abducens nerve palsy complicating cytomegalovirus infection.

This report describes an immunocompetent 5-year-old female with isolated abducens nerve palsy complicating a cytomegalovirus infection and documented with polymerase chain reaction performed on cerebrospinal fluid; treatment with ganciclovir was associated with rapid clinical improvement. It may be the first report of cytomegalovirus detected in the central nervous system as a cause of isolated abducens nerve palsy.

Human granulocytic anaplasmosis in Poland: clinical déjà vu or a new chapter in tick-borne diseases?

OBJECTIVES: Tick-borne diseases represent an increasing public health concern worldwide, including human granulocytic anaplasmosis (HGA). HGA is gaining relevance in travel medicine due to its expanding geographic distribution. Its true incidence remains unknown, largely due to underreporting and diagnostic challenges. HGA manifests itself by nonspecific symptoms and may be misdiagnosed as another tick-borne disease. This study aimed to direct focus on clinical manifestations of this disease and to examine the role of co-infection with other tick-borne pathogens on its clinical manifestations

[Relationship between human parvovirus B19 infection and spontaneous abortion].

OBJECTIVE: To determine whether there is a relationship between parvovirus B19 infection and spontaneous abortion. METHODS: 105 embryo tissues from spontaneous abortions of unknown cause during 1994-1995 were studied using nested polymerase chain reaction (PCR) assay for parvovirus B19 DNA. As controls 40 fetal tissues were also examined. Among which, 20 were collected from induced abortions, and the others were stillbirths. The positive specimens underwent further study for TORCH infections. RESULTS: 26 (24.5%) were B19 DNA positive in the case group and 2 (5.0%) in the control group. The d

The limits of PCR-based diagnostics in parasitology.

Polymerase chain reaction (PCR) plays a central role in parasite diagnostics but is increasingly interpreted as a general test for parasitic infection. Because parasites lack shared molecular features amenable to universal assays, PCR is inherently primer bounded and hypothesis confirming. This article clarifies the interpretive limits of PCR and argues for aligning diagnostic claims with parasite biology.

Spotted-fever group Rickettsia in Dermacentor variabilis, Maryland.

Three-hundred ninety-two adult Dermacentor variabilis were collected from six Maryland counties during the spring, summer, and fall of 2002. Infection prevalence for spotted fever group Rickettsia was 3.8%, as determined by polymerase chain reaction. Single strand conformational polymorphism (SSCP) analysis followed by sequencing indicated that all infections represented a single rickettsial taxon, Rickettsia montanensis.

What role does Helicobacter pylori eradication play in gastric MALT and gastric MALT lymphoma?

The concept of mucosa-associated lymphoid tissue (MALT) has been introduced to differentiate biological functions from behavior of nonnodal vs. nodal lymphoid tissues. Lymphomas arising from MALT also behave differently than typical nodal lymphomas. In contrast to other tissues, MALT in the stomach is almost exclusively a result of Helicobacter pylori infection. Thus, MALT is part of the host defense against the pathogen H. pylori. Consequently, lymphomas arising from gastric MALT may be a clonal evolution starting from the infection. In low-grade gastric MALT lymphoma, cure of the infection m

Dynamics of Pneumocystis carinii air shedding during experimental pneumocystosis.

To better understand the diffusion of Pneumocystis in the environment, airborne shedding of Pneumocystis carinii in the surrounding air of experimentally infected rats was quantified by means of a real-time polymerase chain reaction assay, in parallel with the kinetics of P. carinii loads in their lungs. P. carinii DNA was detected in the air 1 week after infection and increased until 4-5 weeks after infection before stabilizing. A significant correlation was shown between lung burdens and the corresponding airborne levels, suggesting the possibility of estimating the fungal lung involvement t

Inflammatory brachial artery aneurysm with amyloidosis due to nontuberculous mycobacterial infection: a case report.

The patient was a 72-year-old man with a history of kidney transplantation and long-term administration of methylprednisolone and immunosuppressants who underwent resection and reconstruction of a brachial artery aneurysm. A pathological examination confirmed mycobacteria, indicating an inflammatory aneurysm with amyloid kappa light chain deposition caused by intracellular nontuberculous mycobacterium. To our knowledge, this is the first demonstration of an inflammatory brachial artery aneurysm with amyloidosis caused by nontuberculous mycobacterial infection. When a nontuberculous mycobacteri

Applications of polymerase chain reaction-based methods for the diagnosis of plague (Review).

Plague is an acute bacterial infection caused by Yersinia pestis. The three major clinical forms of plague are bubonic, pneumonic and septicemic, which have high case-mortality rates. Therefore, rapid and reliable diagnostic tools are crucial. Currently, bacteriological means and traditional serological assays are used for detecting infection with Y. pestis. However, such methods have their limitations. Polymerase chain reaction (PCR) is one of the most useful tools for rapid diagnosis of plague. The present review introduced the main PCR techniques and their applications for detecting and con

A Stepwise Household Transmission of Vaccine-Strain Varicella-Zoster Virus Resulting in Neonatal Varicella.

We describe a rare case of neonatal varicella from sequential household transmission of varicella-zoster virus. A vaccinated sibling developed herpes zoster before delivery, followed by maternal varicella and neonatal infection at 20 days of life. The infant showed generalized vesiculopustular eruptions with mild respiratory symptoms. Polymerase chain reaction confirmed infection presumed to be caused by the vaccine strain because the sibling had no history of natural varicella and there were no other exposure sources. Intravenous acyclovir was administered with good response. The patient reco

Diagnosis of congenital toxoplasmosis in a renal transplant recipient mother.

We report the case of a first trimester toxoplasmosis infection in a renal transplant recipient. Real-time polymerase chain reaction in amniotic fluid at 18 weeks was negative for Toxoplasma gondii but at 26 weeks major fetal hydrocephalus was discovered leading to medical termination of pregnancy. Pathological examination confirmed lesions consistent with congenital toxoplasmosis. The herein case report, as well as data from the French reference centre for congenital Toxoplamosis (1835 cases in the past eight years), suggests that the strategy of management of pregnancy's first trimester Toxo

Adenovirus-induced obstructive uropathy with acute renal failure in an immunodeficient child.

Viral infections represent severe complications in immunodeficient patients, associated with significant morbidity and mortality. We report a case of a bone marrow-transplanted adolescent with hemorrhagic cystitis 4 weeks after transplant, associated with renal failure because of obstructive pyelonephritis. Diagnostic workup finally revealed adenovirus infection. A double-J stent was inserted with spontaneous favorable evolution thereafter. Adenovirus infection in an immunocompromised patient can cause hemorrhagic cystitis without specific symptoms and might evolve toward obstructive pyeloneph

West nile virus RNA not detected in urine of 40 people tested 6 years after acute West Nile virus disease.

West Nile virus (WNV) causes an acute infection that is usually cleared by an effective immune response after several days of viremia. However, a recent study detected WNV RNA in the urine of 5 of 25 persons (20%) tested several years after their initial acute WNV disease. We evaluated an established cohort of 40 persons >6 years after initial infection with WNV. Urine collected from all participants tested negative for WNV RNA by reverse-transcription polymerase chain reaction and transcription-mediated amplification. Prospective studies are needed to determine if and for how long WNV persist

Detection of human cytomegalovirus genome in uterus tissue.

To analyze persistent infection by human cytomegalovirus (HCMV) in vivo, specimens obtained from various sources and autopsied organs were examined for the presence of HCMV DNA, mRNA transcripts and antigens by polymerase chain reaction, in situ hybridization and immunostaining. The HCMV genome was detected in lung, liver, kidney, and blood vessels at an average positive rate of 15%. The highest PCR-positive rate was observed with cervical smears. Subsequent examination of uterus tissues from patients with myoma revealed HCMV transcripts and antigens in glandular epithelial cells, leukocytes,

Monkeypox Presenting as an Isolated Finger Infection: A Case Report.

CASE: A 50-year-old man with HIV presented with an isolated dorsal index finger abscess. He underwent incision and debridement, but no pus was encountered. Surgical pathology demonstrated pox virus features. He then developed body pustules confirmed as monkeypox through polymerase chain reaction. The surgical wound deteriorated, prompting a second debridement of necrotic skin. Vesicles consistent with viral infection then appeared. The finger healed with local wound care within 3 months, achieving excellent function. CONCLUSION: This case illustrates an uncommon presentation of monkeypox, res

Adenovirus Interstitial Nephritis: An Unusual Cause for Early Graft Dysfunction.

We describe a rare case of adenovirus interstitial nephritis in a 37-year-old man, 4 weeks following deceased donor renal transplantation. He presented with gross hematuria and acute graft dysfunction. A renal biopsy revealed necrotizing tubulointerstitial nephritis with intranuclear viral inclusions in the tubular epithelial cells. Immunohistochemistry and polymerase chain reaction confirmed adenovirus infection. Reduction in immunosuppression alone resulted in rapid improvement of graft function. Awareness of the clinical and characteristic biopsy findings may help establish the correct diag

Manifestations of Herpes Virus Infections in the Nervous System.

The 3 neurotropic human herpes viruses, herpes simplex virus (HSV) type 1 and 2, and varicella-zoster virus (VZV) are capable of establishment of latent viral infection in trigeminal and dorsal root ganglia. HSV-1, and more rarely HSV-2, carries the potential to cause meningoencephalitis, with devastating clinical consequences. Immediate diagnosis, based on clinical presentation, MRI imaging, and molecular diagnosis by polymerase chain reaction, and initiation of therapy are mandatory to reduce mortality and neurologic permanent sequelae. VZV is associated with postprimary infection and reacti
