Among the patients of the Accident Hospital of the Co-operative Trade Association Tübingen, there were 11 cases of acute cholecystitis between 1985 and 1991. Acute cholecystitis occurred after polytrauma (n = 5), multiple fractures (n = 2), head injury (n = 1), fracture of femoral neck (n = 1) or elective hip surgery (arthrodesis, total hip replacement) (n = 2). The mean age was 57 (16-89) years, acute cholecystitis was confirmed 27 (6-54) days after trauma of surgery. 7 cases presented as acute acalculous cholecystitis, whereas in 4 cases of acute cholecystitis cholecystolithiasis was present…

Severe Leptospira autumnalis infection was associated with acute acalculous cholecystitis and pancreatitis in a 66-year-old man. He was successfully treated with antimicrobial agents and supportive therapy, including hemodialysis. We review these uncommon manifestations and the effectiveness of antimicrobial therapy in advanced leptospirosis.

The authors report on their personal experience with percutaneous cholecystostomy (PC) in 36 cases of acute cholecystitis (AC). PC was successful in 34 cases, improving the clinical condition of the patients within the first hour following treatment and normalizing the laboratory data within 24-48 hours. Twenty-seven patients with calculous AC underwent cholecystectomy within 4-15 days. In 6 cases (3 calculous and 3 acalculous ACs) PC was the only treatment performed; in one case (calculous AC) PC was combined with lithotomy, the stone fragments being extracted via the cholecystostomy. Accordi…

Acute cholecystitis is a common cause of emergency room admissions in elderly patients, and may have an atypical course with serious complications and high mortality. The authors present 131 elderly patients (aged 70 and older) who were treated for acute cholecystitis. The most common complaint was right upper abdominal pain (73%), followed by fever (55%), vomiting (48%), palpable mass (22%) and jaundice (13%). Twelve per cent of the patients were in septic shock on admission. Most patients (74%) had severe concomitant disease, increasing their operative risk significantly. Patients were prepa…

The conservative treatment in the cases of primary attack of acute calculous cholecystitis is not effective, this is one of the results of the analysis made on the basis of the experience of thirty years and treatment of 3520 patients with acute cholecystitis. Thus, 90% of such acute attacks turn into chronic, and frequently into relapse forms, besides this 75% of patients get complicated forms of illness already in the first year of treatment. Active surgical and medical tactics give the possibility to reduce the postoperative lethality by 6 times and the number of postoperative complications…

From analysis of the results of ultrasonic examination in 1,428 patients with acute cholecystitis the authors determined the semeiotics of various forms and complications of the disease. The most common ultrasonic sign of acute cholecystitis is a triad of symptoms: enlarged gallbladder, thickened walls, and fixed hyperchostructures with an acoustic shadow in the projection of the neck of the gallbladder. Ultrasonic examination allows the presence of destructive cholecystitis to be detected with high precision on basis of the sign of a double gallbladder contour. Complications like pericystic i…

Acute cholecystitis is well established as one of the high-risk factors bactibilla and wound infection. However, many patients with acute cholecystitis do not have bactibillia. Therefore, we analyzed 20 clinical and laboratory parameters in 49 patients with acute cholecystitis to determine which factor(s) predicted bactibilla. Twenty-one (42.9%) of 49 patients with pathologically proved acute cholecystitis had positive bile and/or gallbladder wall cultures. Univariate analysis suggested that a preoperative temperature greater than 37.3 degrees C, a total serum bilirubin level greater than 8.6 …

A case of xantomatogranulomatous cholecystitis found in a review of 615 surgical gallbladders is reported, an incidence of 0.16%. Symptoms were those of acute cholecystitis. There was xantogranulomatous infiltration of the neighboring tissues. Both macroscopic and imaging features were those of a carcinomatous lesion. The definitive diagnosis was made by peroperative frozen sections.

The analysis of lethal outcomes in 108 (3.66%) of the 2974 patients operated on for acute cholecystitis was carried out. In 40.4% of the patients, mainly the elderly and senile, a death was caused by acute hepatorenal failure.

A 48-year-old man developed progressively more severe epigastric pain, pain on pressure in the right upper abdomen and fever up to 38.6 degrees C so that acute cholecystitis was suspected. Ultrasound did not demonstrate a gall-bladder but a sickle shaped, dense echo with a distal adjoining echo-free zone. Computed tomography revealed air in the gall-bladder lumen as well as intramural and pericholecystic air pockets, findings pathognomonic for emphysematous cholecystitis. In addition pneumoperitoneum was diagnosed. Subsequent cholecystectomy intraoperatively revealed a gangrenous, nonperforati…

An increasingly important subset of patients with biliary tract disease are those with acute cholecystitis. The indications for both routine or selective intraoperative cholangiography (IOC) during elective cholecystectomy may not be appropriate for this group. The utility of IOC in the presence of acute cholecystitis was examined. The medical records of 223 patients with histologically confirmed acute cholecystitis were reviewed. Clinical and laboratory criteria included age, sex, white blood cell count (WBC), serum bilirubin and alkaline phosphatase levels. In 52 (23%) patients, IOC was not …

Experience in operative treatment of 228 patients for acute acalculous cholecystitis is analysed. This disease differs from acute calculous cholecystitis essentially in the main clinical and morphological signs. Four types of acute acalculous cholecystitis were distinguished according to origin: vascular, enzymatic, enzymatic-vascular, and obturative. The clinical forms are shown: primary vascular, emphysematous, posttraumatic, gallbladder torsion, enzymatic, obturative. It is pointed out that acute acalculous cholecystitis is characterized by rapid destruction of the cystic wall and frequent …

Six patients with acute cholecystitis which had developed after various mechanical injuries were under observation. Ultrasonic examination, cholescintigraphy, and laparoscopy were conducted in establishing the diagnosis. Two patients were subjected to emergency operation. Laparoscopic drainage of the gallbladder was undertaken in 4 patients to relieve acute cholecystitis. Operation was carried out after acute inflammation had abated and treatment of the trauma was concluded. There were no fatal outcomes.

Functional inhibition of the drug liver metabolism seems to start at early subclinical stages of cholecystitis. Antipyrine++ level in the initial saliva sample drops significantly and continues to change with growing structural impairment of the biliary system in progression of chronic noncalculous cholecystitis into calculous one.

Operations were performed on 221 patients for tentative diagnosis of acute cholecystitis at the Surgical Department of Goslar District Hospital, between January 1982 and August 1986. Evaluation was possible of 179 of these cases. This group was compared with 96 patients who had undergone surgery some time after admission to hospital, between January 1980 and December 1981, with 90 patients of these patients being evaluated. Mortality, postoperative complications, length of surgical sessions, and length of hospitalisation of immediate treated patients were lower than those recorded from patient…
