Recurrence of Dupuytren's contracture: A consensus-based definition.

PURPOSE: One of the major determinants of Dupyutren disease (DD) treatment efficacy is recurrence of the contracture. Unfortunately, lack of agreement in the literature on what constitutes recurrence makes it nearly impossible to compare the multiple treatments alternatives available today. The aim of this study is to bring an unbiased pool of experts to agree upon what would be considered a recurrence of DD after treatment; and from that consensus establish a much-needed definition for DD recurrence. METHODS: To reach an expert consensus on the definition of recurrence we used the Delphi met

[Early recognition and management of the disease].

Glaucoma is a progressive disease of the optic nerve potentially leading to a destruction of the nerve and therefore to blindness. It is a multifactorial disease and patients of all age groups can be affected. Different risk factors influence the course of the disease. The quality of life of the patients is already reduced when the patients show moderate changes of their visual field. The patients experience more falls and may be forced to stop driving. With an early and adequate treatment the progression of the disease can be slowed down. The early recognition of the disease is important and

The role of biopsy for small renal masses.

The incidence of small renal masses (SRMs) has been increasing due to the more liberal use of abdominal imaging. This increased detection has driven the attention of clinicians to the characterization of these lesions and toward a better understanding of their natural history. To this end, renal tumour biopsies (RTBs) have a crucial role as they provide vital pathological information. The improved quality and accuracy of RTBs provide urologists with a very truthful tool to support and guide treatment decisions. The future of RTB will combine pathological, molecular and genetic information that

Guidelines (short version) of the French Otorhinolaryngology - Head and Neck Surgery Society (SFORL) on patient pathway organization in ENT: The therapeutic decision-making process.

OBJECTIVES: The authors present the guidelines of the French Otorhinolaryngology - Head and Neck Surgery Society (SFORL) for patient pathway organization in head and neck cancer, and in particular for multidisciplinary team meetings. The present article concerns the therapeutic decision-making process. METHODS: A multidisciplinary work group was entrusted with a review of the scientific literature on the above topic. Guidelines were drawn up, based on the articles retrieved and the group members' individual experience. They were then read over by an editorial group independent of the work gro

Management of pancreatic trauma: A pancreatic surgeon's point of view.

BACKGROUND: Pancreatic trauma occurs in 0.2% of patients with blunt trauma and 1-12% of patients with penetrating trauma. Traumatic pancreatic injuries are characterised by high morbidity and mortality, which further increase with delayed diagnoses. The diagnosis of pancreatic trauma is challenging. Signs and symptoms can be non-specific or even absent. METHODS: A critical review of studies reporting the management and outcomes of pancreatic trauma was performed. RESULTS: The management of pancreatic trauma depends on the haemodynamic stability of the patient, the degree and location of pare

The utility of large language models in oncological multidisciplinary team meetings: A systematic review.

Large language models (LLMs) have emerged in recent years as innovative artificial intelligence systems with early potential in clinical decision-making. This is the first systematic review to evaluate LLMs' oncological decision-making and compare their treatment recommendations to "gold standard" oncological multi-disciplinary team (MDT) decision-making. PubMed, EMBASE and Medline databases were last searched on 20th January in line with PRISMA guidelines. All relevant peer-reviewed publications comparing LLM and MDT treatment recommendations in patients with cancer were included. Studies usi…

A multi-modal approach for decision making in bladder cancer.

Bladder cancer remains a major global health challenge, characterized by diagnostic uncertainty, substantial treatment costs and high recurrence rates. Current diagnostic and treatment modalities, including cystoscopy, transurethral resection of bladder tumour and standard histopathology, have limitations, including the inability to detect flat lesions, frequent understaging and interobserver variability, highlighting a crucial need for improved approaches. Advances in artificial intelligence (AI), blue-light cystoscopy, narrow-band imaging, cytology and urinary markers show promise in enhanci

The Role of Artificial Intelligence in Chronic Liver Diseases and Liver Transplantation.

In hepatology, pattern recognition in laboratory data and clinical characteristics is the hallmark of clinical care. Artificial intelligence (AI) tools, like machine or deep learning and large language models, provide interesting mechanisms for facilitating care advancement. The complexity and diversity of data, as well as genetic, environmental, and lifestyle factors, all contribute to individualized recommendations intuitively made by clinicians for patients with liver disease. AI tools provide the opportunity to train on high-volume data and simulate the clinician's subconscious thought pro

Evidence-informed recommendations on managing breast screening atypia: perspectives from an expert panel consensus meeting reviewing results from the Sloane atypia project.

Evidence-based clinical guidelines are essential to maximize patient benefit and to reduce clinical uncertainty and inconsistency in clinical practice. Gaps in the evidence base can be addressed by data acquired in routine practice. At present, there is no international consensus on management of women diagnosed with atypical lesions in breast screening programmes. Here, we describe how routine NHS breast screening data collected by the Sloane atypia project was used to inform a management pathway that maximizes early detection of cancer and minimizes over-investigation of lesions with uncerta

The performance of artificial intelligence large language model-linked chatbots in surgical decision-making for gastroesophageal reflux disease.

BACKGROUND: Large language model (LLM)-linked chatbots may be an efficient source of clinical recommendations for healthcare providers and patients. This study evaluated the performance of LLM-linked chatbots in providing recommendations for the surgical management of gastroesophageal reflux disease (GERD). METHODS: Nine patient cases were created based on key questions addressed by the Society of American Gastrointestinal and Endoscopic Surgeons (SAGES) guidelines for the surgical treatment of GERD. ChatGPT-3.5, ChatGPT-4, Copilot, Google Bard, and Perplexity AI were queried on November 16th

Beading plot: a novel graphics for ranking interventions in network evidence.

BACKGROUND: Network meta-analysis is developed to compare all available treatments; therefore it enriches evidence for clinical decision-making, offering insights into treatment effectiveness and safety when faced with multiple options. However, the complexity and numerous treatment comparisons in network meta-analysis can challenge healthcare providers and patients. The purpose of this study aimed to introduce a graphic design to present complex rankings of multiple interventions comprehensively. METHODS: Our team members developed a "beading plot" to summary probability of achieving the bes…

Thyroid cytology: The reality before and after the introduction of ultrasound classification systems for thyroid nodules.

BACKGROUND: Several ultrasound-based systems for classification of thyroid nodules are available. They allow for a better triage of the nodules that require cytological assessment, and lead to standardized recommendations. Our aim was to compare patients and nodules referred to fine-needle aspiration (FNA) before and after the introduction of these systems. METHODS: A retrospective study comparing two cohorts of patients referred for FNA was performed (386 patients and 463 nodules in 2015; 220 patients and 263 nodules in 2021). RESULTS: The sex distribution (89.1% vs 85.9% females, p=0.243),
