Combined entrapment of the median and anterior interosseous nerves in a pediatric both-bone forearm fracture.

Unrecognized entrapment of the median nerve and its anterior interosseous branch in the midportion of the forearm occurred in a 12-year-old girl following reduction of a closed both-bone fracture. Following delayed diagnosis, treatment required secondary neurolysis and microscopic neurorrhaphy. Meticulous prereduction and postreduction neurovascular examination is recommended to prevent this unique complication.

Management of subtrochanteric fractures of the femur in children.

We report the results of treatment in 99 children with subtrochanteric fractures of the femur. Late reviews of 60 of the 65 children treated with traction and then a hip spica after stabilization of the fracture by callus, and of 20 of the 22 treated with early closed reduction and a hip spica yielded 80% satisfactory outcomes. However, 27% of the fractures treated with early closed reduction required remanipulation. The satisfactory outcomes in the traction and delayed spica group would be increased to nearly 100% by using only skin traction, as persistent lengthening of the femur and pin-sit

Ipsilateral floating hip and knee, or floating lower limb - A rare case report.

INTRODUCTION: Ipsilateral floating hip and knee injuries at the same time are very rare injuries, occurring due to high velocity trauma. With a small number of such case reports, the treatment guidelines regarding surgical fixation are yet to be formulated. CASE REPORT: We report an interesting case of hip dislocation, comminuted acetabular fracture, intertrochanteric and shaft of femur fracture and both bones leg fracture ipsilaterally. Open reduction of hip, antegrade femoral nailing with recon nail and closed tibial nailing was done in a single setting. Acetabular fracture was treated non-

Hybrid fixation with ESIN for both bone forearm fractures in adults: A case report and literature review.

OBJECTIVE: Both bone forearm fractures are common in children and rare in adults. The main surgical treatment is open reduction and internal fixation (ORIF) with plate, while the hybrid fixation of elastic-stable intramedullary nailing (ESIN) and the plate has been rarely reported before. CASE REPORT: We report a case of a 29-year-old male patient with both bone forearm fractures. Temporarily closed reduction and plaster external fixation were performed in the emergency room, and the patient was admitted to the orthopedic trauma ward for surgery. The patient underwent open reduction and plate

Use of a Minimally Invasive Traction Repositor versus Conventional Manual Traction for the Treatment of Tibial Fractures: A Comparative Study from a Tertiary Hospital in China.

BACKGROUND: Closed reduction and intramedullary nail fixation of tibial fractures may not utilize a fracture table or reduction aids like a femoral distractor, and only manual traction will help aid the reduction process. This study aimed to describe and further investigate the effectiveness of an originally designed minimally invasive traction repositor (MITR) for the treatment of tibial fractures. METHODS: From January 2018 to April 2021, a total of 119 eligible patients with tibial shaft fractures were included and retrospectively assigned to two groups according to different reduction met

Radiographic fracture features predicting failure of internal fixation of displaced femoral neck fractures.

INTRODUCTION: Fixation-related complications of displaced femoral neck fractures treated by internal fixation are accompanied by high mortality and morbidity. The aim of this study is to investigate the pre- and postoperative radiographic fracture characteristics in relation to patient age and the occurrence of reoperation caused by fixation failure. METHODS: The preoperative radiographs of all patients presenting with a proximal femur fracture between January 2004 and December 2012 were retrospectively assessed for fracture type and dislocation (AP and lateral view). Patients with a displace

Percutaneous osteosynthesis of Galeazzi fracture-dislocation.

BACKGROUND: A Galeazzi fracture is defined as a fracture of the radius associated with dislocation of the distal radio-ulnar joint (DRUJ). The conventional surgical technique of nailing does not give enough stability and open reduction, internal fixation with the plate is associated with numerous complications. The stacked nailing for the management of these injuries provides adequate stability, maintains the relationship of the DRUJ and promotes uneventful union by closed technique. The purpose of this study is to evaluate the results of simple, user-friendly, low cost elastic stacked nailing

Supracondylar elbow fractures with impaction of the medial condyle in children.

We reviewed the cases of 13 children with supracondylar elbow fractures with impaction of the medial wall. The displacement was underestimated in two patients and treated as a minimally displaced fracture by simple immobilization without reduction, resulting in a cubitus varus deformity. In one patient, the correct diagnosis was made, but treatment by reduction was refused by the parents. A loss of carrying angle of 10 degrees resulted from this. Two patients referred for treatment of cubitus varus were treated elsewhere for the initial injury: one by reduction and plaster cast immobilization

[Nasal fractures in adults].

The risk of complications warrants treatment of most dislocated nasal fractures. Other injuries including other facial fractures and septal haematoma must be treated if present at the initial presentation. The usual treatment for a simple nasal fracture is closed reduction in local anaesthesia after five to seven days. Complicated cases require open reduction in general anaesthesia. Later revision of the deviated nose may become necessary in patients suffering from complications such as persistent nasal stenosis and/or deformity.

Interphalangeal dislocation of the fourth toe with avulsion-fracture in a child: report of a case.

Phalangeal dislocations of toes are extremely rare in childhood and usually can be treated by closed reduction. We present a proximal interphalangeal dislocation of the fourth toe with an irreducible avulsion fracture of the middle phalanx requiring open reduction. To our knowledge concomitant avulsion fractures in this condition have not been reported thus far in pediatric patients. The pathological mechanism of this injury is discussed, and the significance of the plantar plate for joint stability is emphasized.

Both Bone Forearm Fracture with Complex DRUJ Dislocation: What Are the Odds?

Background  Distal radioulnar joint (DRUJ) dislocation can occur as an isolated injury or in association with fracture radius (Galeazzi fracture-dislocation), Essex-Lopresti lesion or, rarely, with fracture of both radius and ulna (termed "Galeazzi type fracture"). DRUJ dislocations can be simple or complex. While simple DRUJ dislocation can be reduced by closed methods once the associated fractures are fixed anatomically, complex dislocation does not reduce by closed means. A complex DRUJ dislocation occurring in a both bone forearm fracture is an extremely unusual pattern of injury. Case Des

'Temporomandibular joint hematoma nerve block'-a new technique in management of mandibular condylar fractures.

PURPOSE: Management of mandibular condylar fracture has invited a great deal of controversy in maxillofacial trauma care. In the orthopaedic literature, surgeons have exhaustively described the use of a 'hematoma block' technique during closed reduction (CR) of the ankle or other long bone fractures. Post-traumatic ankylosis of the temporomandibular joint (TMJ) is due to development and progression of the intra-articular hematoma. We improvised their technique for use during CR of mandibular condylar fractures. The desirable effects which can be achieved with our proposed 'novel TMJ hematoma n

Operative treatment of displaced Salter-Harris III distal tibial fractures.

Displaced, isolated Salter-Harris III fractures of the distal tibia are infrequent. Typically, the mechanism of injury is an external rotation stress to the adolescent ankle. The distal tibial growth plate fuses from medial to lateral during adolescence. If the anterior tibiofibular ligament remains intact, lateral distal epiphysis of the tibia. Closed reduction of displaced fractures is usually unsuccessful. In 5 displaced fractures, periosteal flaps were frequently found to have blocked closed reduction. Follow-up evaluation after open reduction and internal fixation revealed anatomic healin

Surgical management of delayed irreducible Gartland III supracondylar fractures in children: open reduction and internal fixation versus external fixation.

BACKGROUND: Percutaneous pinning has been accepted as the "gold standard" for displaced supracondylar humeral fracture (SHF) in children. However, to secure an anatomic reduction and to reduce the rotated fragment, open reduction is often necessary when there is inadequate stabilization or a satisfactory reduction has failed to be achieved. The study aimed to compare the efficacy of the open reduction and bioabsorbable poly-d,l-lactic acid (PDLLA) pin fixation method with the closed reduction and lateral external fixation method in irreducible delayed Gartland type III SHF in children.
