Variability of fasting plasma glucose increased risks of diabetic polyneuropathy in T2DM.

OBJECTIVE: To examine whether variations in fasting plasma glucose (FPG), as measured by the coefficient of variation (CV), is a predictor of diabetic polyneuropathy (DPN) risk, considering glycated hemoglobin (HbA1c) and other traditional risk factors. METHODS: Type 2 diabetic patients enrolled in the National Diabetes Care Management Program were ≥30 years of age and free of DPN (n = 36,152). They were enrolled in 2002-2004 and were monitored until 2011. The related factors were analyzed using Cox proportional hazards regression models. RESULTS: During an average 7.23 years of follow-up, a

Epidemiological insights on malnutrition: some resurrected, others restructured, a few retired.

In its modern sense, epidemiology has progressed from a classical concern of causality as expressed within biological limits to recognition that a variety of social factors have proportionate significance in the origin and behavior of human nutritional disorders. Basically an analytical process, nutritional epidemiology has grown to include, among objectives for prevention and control, a synthesis of plans for intervention; the monitoring of action programs introduced; and again a parallel analytic mission to evaluate such accomplishments as programs bring forth. Nutritional epidemiology, so e

Partner support, communication and family planning choices among Nigerian women: a qualitative study.

INTRODUCTION: Partner involvement plays a critical role in shaping women's contraceptive choices, yet evidence on its influence remains limited in Nigeria, where contraceptive prevalence is low. This study explored how partner support and communication affect women's family planning decisions. METHODS: A descriptive phenomenological qualitative study was conducted over 6 weeks at the family planning clinic of a tertiary healthcare facility in southwest Nigeria, using in-depth interviews, with data analysed through inductive thematic analysis following Braun and Clarke's six-step framework. Leveraging Community-Based System Dynamics to Understand Long Covid Disparities in African American Communities: A Model for Health Equity Research.

BACKGROUND: Long Covid disproportionately affects African American communities, exacerbating pre-existing health disparities and systemic barriers to care. Conventional public health interventions often fail to address the complex systemic issues at play due to a lack of grounding in community-certified knowledge about the broader societal context that produces the disparities and in which the interventions must operate. New methods are needed to elicit community perspectives on living with long Covid. METHODS: This study employed Community-Based System Dynamics (CBSD) workshops, conducted in

Community-Driven Health Education: Role of Health Literacy Seminars in Empowering Underserved Populations and Medical Students.

Health literacy has increasingly been recognized as a crucial determinant of health outcomes, specifically among underserved populations facing immense barriers to accessing and understanding their healthcare plans. This research paper advocates for community-based health literacy seminars to address this disparity. These seminars can help underserved populations connect with individuals involved in healthcare within their communities while also providing experiential learning opportunities for medical students, fostering the growth of communication skills and cultural competence. The study ai

Maternal survival in developing countries: what has been done, what can be achieved in the next decade.

Every year, approximately 600000 women die of pregnancy-related causes - 98% of these deaths occur in developing countries. For every woman who dies, at least 30 suffer injuries and often, permanent disability. The challenge today is to re-orient programmes on priority interventions, and to mobilize sufficient resources for their implementation. More resources have been put into antenatal care than into delivery care and the management of complications of births and unsafe abortions. This article describes the effective strategies for reducing maternal death, their policy requirements and prog

Clinical value of patient-specific three-dimensional printing of congenital heart disease: Quantitative and qualitative assessments.

OBJECTIVE: Current diagnostic assessment tools remain suboptimal in demonstrating complex morphology of congenital heart disease (CHD). This limitation has posed several challenges in preoperative planning, communication in medical practice, and medical education. This study aims to investigate the dimensional accuracy and the clinical value of 3D printed model of CHD in the above three areas. METHODS: Using cardiac computed tomography angiography (CCTA) data, a patient-specific 3D model of a 20-month-old boy with double outlet right ventricle was printed in Tango Plus material. A pediatrician's view. Skin manifestations of bioterrorism.

The physician must be in contact with the local public health infrastructure as soon as a potential biological agent is perceived as possible. Most states are now setting up contingency plans and means to address these issues in a systematic way. This involves using local health departments, police departments, fire departments, National Guard units, and federal agencies such as the CDC and the FBI. The key component, however, is actually identifying a biological agent in the community and then moving quickly to isolate those who may be at risk of spreading the infection.

An evaluation of the knowledge and practices of trained traditional birth attendants in Bodinga, Sokoto State, Nigeria.

To improve Maternal and Child Health services especially in the rural areas, a programme to train traditional birth attendants (Ungo Zoma) was established by the Sokoto State government of Nigeria in 1975. The impact of the training programme on the knowledge and practices of traditional birth attendants (TBAs) in a rural community in the state was studied. Seventy-four TBAs, consisting of 43 trained and 31 untrained attendants, were interviewed. Statistically significant differences were observed in the proportion of both groups of TBAs able to recognize high risk pregnancies and deliveries f

Advance care planning with individuals experiencing homelessness: Literature review and recommendations for public health practice.

Vulnerable populations in the United States experience disparities in access to advance care planning and may have significant unmet health care needs at the end of life, including unrelieved suffering. People who are homeless have increased morbidity and mortality risks, yet lack opportunities to communicate end-of-life preferences. This paper includes a narrative literature review of advance care planning interventions and qualitative investigations into end-of-life concerns among people experiencing homelessness. Trials of clinician-guided interventions with homeless individuals demonstrate

Organization of funding for community-based HIV testing in Côte d'Ivoire: potentially counterproductive search for efficiency?

Since the early 2010s, the President's Emergency Plan for AIDS Relief (Pepfar) and the Global Fund to Fight AIDS, Tuberculosis and Malaria have stiffened their result-based management strategies. Disbursement conditions are increasingly demanding for funded NGOs (achievement of quantified objectives, frequent reporting, and monitoring of activities), and their remuneration depends on the results achieved. This article aims to analyze the effects of this strengthening of policies based on New Public Management on local implementation of activities, based on the example of community-based HIV te

Mapping and Cataloguing Microbial and Biochemical Determinants of Health: Towards a 'Database of Salutogenic Potential'.

Microbial and biochemical research has historically focused on pathogenic agents due to their clear association with disease. This is a perspective that has saved countless lives but encourages a skewed, threat-centered view of microbes and biogenic compounds. Emerging evidence shows that exposure to diverse environmental microbiomes and natural biochemical products is also salutogenic-promoting health and resilience. Here we introduce the 'Database of Salutogenic Potential', a prototype relational repository cataloguing environmental microbes and biochemical compounds linked to health benefit

Leveraging Stakeholder Engagement and Virtual Environments to Develop a Strategy for Implementation of Adolescent Depression Services Integrated Within Primary Care Clinics of Mozambique.

Psychiatric disorders are the number one cause of disability in adolescents worldwide. Yet, in low- and middle-income countries (LMIC), where 90% of adolescents reside, mental health services are extremely limited, and the majority do not have access to treatment. Integration of mental health services within primary care of LMICs has been proposed as an efficient and sustainable way to close the adolescent mental health treatment gap. However, there is limited research on how to effectively implement integrated mental health care in LMIC. In the present study, we employed Implementation Mappin

Reproductive Health Experiences of Females Diagnosed with Young-Onset Colorectal Cancer: A Multi-Method Cross-Sectional Survey.

OBJECTIVE: Given the increasing risk of young-onset colorectal cancer (yCRC) among adults under 50 years, it is important to understand impacts on reproductive health. Our objective was to assess experiences with reproductive health after yCRC diagnosis among females. METHODS: We conducted a cross-sectional study among females, 18 years or older, who have been diagnosed yCRC and are able to communicate in English. Data were gathered using an online survey involving both quantitative (e.g., multiple choice) and qualitative (e.g., open-ended text) questions on pregnancy history, influence of yC

Association between community walkability and hypertension: Evidence from the Wuhan Chronic Disease Cohort Study.

While community walkability is recognized as a key environmental factor for health status, evidence linking it specifically to hypertension is rather limited. To fill the knowledge gap, we concluded a cross-sectional study among 6421 eligible participants from the Wuhan Chronic Disease Cohort. A well-developed algorithm was performed to evaluate community walkability across Wuhan, quantified as Walk Score. We then calculated each participant residential Walk Score using the geographic information system. The logistic and linear regression models were conducted to determine the relationship bet

Prediction of community prevalence of human onchocerciasis in the Amazonian onchocerciasis focus: Bayesian approach.

OBJECTIVE: To develop a Bayesian hierarchical model for human onchocerciasis with which to explore the factors that influence prevalence of microfilariae in the Amazonian focus of onchocerciasis and predict the probability of any community being at least mesoendemic (>20% prevalence of microfilariae), and thus in need of priority ivermectin treatment. METHODS: Models were developed with data from 732 individuals aged > or =15 years who lived in 29 Yanomami communities along four rivers of the south Venezuelan Orinoco basin.
