EFNS/ENS Guidelines for the treatment of ocular myasthenia.

BACKGROUND AND PURPOSE: The symptoms of acquired autoimmune ocular myasthenia are restricted to the extrinsic eye muscles, causing double vision and drooping eyelids. These guidelines are designed to provide advice about best clinical practice based on the current state of clinical and scientific knowledge and the consensus of an expert panel. SEARCH STRATEGY: Evidence for these guidelines was collected by searches in the MEDLINE and Cochrane databases. The task force working group reviewed evidence from original articles and systematic reviews. The evidence was classified (I, II, III, IV) an

Treatment developments and the unfolding of the quality of life discussion in childhood medulloblastoma: a review.

PURPOSE: To describe how the quality of life (QOL) discussion in childhood medulloblastoma (MB) relates to treatment developments, survival and sequelae from 1920 to 2014. METHODS: Articles containing "childhood medulloblastoma" and "quality of life" were identified in PubMed. Those containing phrases pertaining to psychological, emotional, behavioral or social adjustment in the title, abstract or keywords were selected. Inclusion of relevant older publications was assured by cross-checking references. RESULTS: 1920-1930s: suction, electro-surgery, kilovolt (KV) irradiation. Survival = month

Predictors of seizure freedom after surgical management of tuberous sclerosis complex: a systematic review and meta-analysis.

PURPOSE: To evaluate the rate of seizure freedom in surgical management of tuberous sclerosis complex (TSC) and to identify factors associated with postoperative seizure outcome. METHODS: A comprehensive literature search identified articles published since 1990 that described seizure outcomes in patients with TSC who underwent resective surgery. Two reviewers assessed article eligibility and independently extracted the data. Eleven potential prognostic variables were identified and dichotomized for analyses. Random or fixed effects meta-analyses were used depending on the presence or absence

Movement disorders in women: a review.

The field of women's health developed based on the recognition that there are important sex-based differences regarding several aspects of medical illnesses. We performed a literature review to obtain information about differences between women and men for neurological movement disorders. We identified important differences in prevalence, genetics, clinical expression, course, and treatment responses. In addition, we found that female life events, including menstruation, pregnancy, breast feeding, menopause, and medications prescribed to women (such as oral contraceptives and hormone-replaceme

Wavelet adaptation for automatic voice disorders sorting.

Early diagnosis of voice disorders and abnormalities by means of digital speech processing is a subject of interest for many researchers. Various methods are introduced in the literature, some of which are able to extensively discriminate pathological voices from normal ones. Voice disorders sorting, on the other hand, has received less attention due to the complexity of the problem. Although, previous publications show satisfactory results in classifying one type of disordered voice from normal cases, or two different types of abnormalities from each other, no comprehensive approach for autom

Dementia in the movies: the clinical picture.

OBJECTIVES: Visual media influence the general public's perceptions and attitudes regarding people with mental conditions. This qualitative study investigates the depiction accuracy of dementia's clinical features in motion pictures. METHOD: Using the search terms 'dementia', 'Alzheimer's disease' and 'senility' movies with release dates between January 2000 and March 2012 were sought on the Internet Movie Database. Based on four selection criteria 23 movies were included. Independently, three researchers watched all movies, scored symptoms, capacities, and behaviors. Scores were discussed an

A new approach for measuring the work and quality of histopathology reporting.

AIMS: Cancer datasets drive report quality, but require more work to inform compliant reports. The aim of this study was to correlate the number of words with measures of quality, to examine the impact of the drive for improved quality on the workload of histopathology reporting over time. METHODS AND RESULTS: We examined the first 10 reports of colon, breast, renal, lung and ovarian carcinoma, melanoma resection, nodal lymphoma appendicitis and seborrhoeic keratosis (SK) issued in 1991, 2001 and 2011. Correlations were analysed using Pearson's partial correlation coefficients. Word count inc

Donor vigilance: what are we doing about it?

Donor vigilance is the systematic monitoring of adverse reactions and incidents in blood donor care with a view to improving quality and safety for blood donors. Standard international definitions are available for surveillance purposes. In recent years advances have been made in determining risk factors for vasovagal and other adverse reactions to blood donation as well as in evaluating preventive measures. Blood establishments should record all adverse reactions in blood donors. Besides its use for individual donor care, this information can be reviewed within and between organisations to gu

The effect of antihypertensive treatment on headache and blood pressure variability in randomized controlled trials: a systematic review.

Antihypertensive drugs reduce headache but it is unclear whether there are differences between drug classes. Calcium channel blockers (CCBs) decrease variability in systolic blood pressure (SBPV) and stroke risk more than other classes, possibly due to decreased vascular tone. If so, there might be a correlation between drug-class effects on variability in SBP and on headache. We determined antihypertensive class effects on SBPV and headache during follow-up in a systematic review of randomized controlled trials. We determined pooled estimates of treatment effect on group variability in BP (va

Analysis of hazardous material releases due to natural hazards in the United States.

Natural hazards were the cause of approximately 16,600 hazardous material (hazmat) releases reported to the National Response Center (NRC) between 1990 and 2008-three per cent of all reported hazmat releases. Rain-induced releases were most numerous (26 per cent of the total), followed by those associated with hurricanes (20 per cent), many of which resulted from major episodes in 2005 and 2008. Winds, storms or other weather-related phenomena were responsible for another 25 per cent of hazmat releases. Large releases were most frequently due to major natural disasters. For instance, hurricane

[Fibromyalgia and sleep: a review].

INTRODUCTION: Although medical literature has devoted little attention to sleep compared to other symptoms of fibro-myalgia, the American College of Rheumatology has recently proposed that sleep should be a central aspect of the clinical assessment of fibromyalgia. Despite this, no comprehensive reviews have encompassed the physiopathological, psycho-social and therapeutic aspects of the relationship between sleep and fibromyalgia. DEVELOPMENT: A bibliographic search of the terms 'fibromyalgia' and 'sleep' was made in the platforms Scopus and OVID (Medline, PsychINFO and Ovid Nursing Database

Effects of β-blocker selectivity on blood pressure variability and stroke: a systematic review.

OBJECTIVES: β-Blockers increase variability in systolic blood pressure (SBP), which probably explains their lesser effectiveness in preventing stroke vs myocardial infarction compared with other agents. This increase in variability in blood pressure (BP) may be particularly marked on non-cardioselective agents, potentially calling into question the widespread first-line use of propranolol in migraine with aura, elderly patients with essential tremor or anxiety, and other groups at risk of stroke. METHODS: We determined β-blocker subclass effects on variability in BP and stroke risk in a syste

Advancements in web-database applications for rabies surveillance.

BACKGROUND: Protection of public health from rabies is informed by the analysis of surveillance data from human and animal populations. In Canada, public health, agricultural and wildlife agencies at the provincial and federal level are responsible for rabies disease control, and this has led to multiple agency-specific data repositories. Aggregation of agency-specific data into one database application would enable more comprehensive data analyses and effective communication among participating agencies. In Québec, RageDB was developed to house surveillance data for the raccoon rabies variant

Utility of dilation and curettage in the diagnosis of pregnancy of unknown location.

OBJECTIVE: We sought to determine utility of uterine evacuation for diagnosis of nonviable pregnancy of unknown location (PUL). STUDY DESIGN: We conducted a cohort study to assess the prevalence of ectopic pregnancy (EP), overall, and stratified by presenting signs and symptoms in women with a nonviable PUL. RESULTS: Of the 173 women, 66 (38%) had miscarriage (spontaneous abortion [SAB]) and 107 (62%) had EP. When initial human chorionic gonadotropin (hCG) was <2000 mIU/mL, the odds of an EP were greater (odds ratio, 4.32; 95% confidence interval, 2.04-9.12). Demographic factors, obstetric h

The simulation of hallucinations to reduce the stigma of schizophrenia: a systematic review.

BACKGROUND/OBJECTIVES: Many people with schizophrenia face stigmatisation. Several methods have been produced to simulate the auditory and visual hallucinations experienced by people with schizophrenia in order to increase empathy and understanding about the condition. However, there has been no review of such methods. This systematic review aims to determine whether and how simulated hallucinations are effective in reducing stigma, and if simulated hallucinations are safe and acceptable. METHODS: Medline, Embase, PsycInfo, the Cochrane Library, CINAHL, and Worldcat Dissertations and Theses w

The association between operative repair of bladder injury and improved survival: results from the National Trauma Data Bank.

PURPOSE: The bladder is the most commonly injured genitourinary organ from blunt pelvic trauma. In this study we describe traumatic bladder injuries in the United States, their management and association with mortality. MATERIALS AND METHODS: We queried the 2002 to 2006 National Trauma Data Bank for all subjects with bladder injury. Demographics, mechanism of injury, coexisting injuries, type of bladder injury, and operative interventions for bladder and other abdominal trauma are described. Multivariate logistic regression analysis was used to examine the relationship between bladder injury

Spino-pelvic sagittal balance of spondylolisthesis: a review and classification.

INTRODUCTION: In L5-S1 spondylolisthesis, it has been clearly demonstrated over the past decade that sacro-pelvic morphology is abnormal and that it can be associated to an abnormal sacro-pelvic orientation as well as to a disturbed global sagittal balance of the spine. The purpose of this article is to review the work done within the Spinal Deformity Study Group (SDSG) over the past decade, which has led to a classification incorporating this recent knowledge. MATERIAL AND METHODS: The evidence presented has been derived from the analysis of the SDSG database, a multi-center radiological dat
