Regional musculoskeletal pain. The elbow, forearm, wrist and hand.

Two main groups of soft-tissue disorders are identified: specific soft tissue syndromes and a non-specific disorder as yet not fully characterized. Specific soft tissue syndromes occur in joint, muscle or nerve and are associated with characteristic symptoms and physical signs. These include epicondylitis at the elbow, tendon disorders at the wrist and nerve entrapments such as carpal tunnel syndrome. The non-specific soft tissue syndrome is primarily described by symptoms of pain but may include muscular symptoms (such as weakness and cramp) or nerve symptoms (such as numbness, pins and needl…

Long-term results of the GSB III elbow arthroplasty.

Between 1978 and 1986, 59 patients received a GSB III elbow prosthesis, six of them in both elbows. Rheumatoid arthritis (RA) was the underlying cause in 51 of the patients and post-traumatic osteoarthritis (PTOA) in eight. Of these, 24 patients (28 prostheses) have since died; two, both operated on bilaterally, had had their implants for more than ten years and had already been assessed for inclusion in the long-term follow-up. Two patients, each with one elbow prosthesis, have been lost to follow-up and three males who are still living (two with PTOA, one with juvenile RA) had their prosthes…

Symmetry of the elbow kinematics during racing wheelchair propulsion.

The purpose of this study was to establish whether bilateral symmetry exists during wheelchair propulsion in the elbow movement pattern of trained wheelchair racers. Seven endurance-trained wheelchair racers volunteered to participate in the study. Each subject was recorded by two-dimensional video analysis while pushing on a single-roller wheelchair ergometer at 14 mph (6.58 ms-1) in their own racing wheelchair. The range of elbow flexion, elbow orientation and selected timing parameters including cycle time and time spent in contact with the handrim (propulsion phase) were obtained from both…

Above-elbow amputation: a case study in restoring function.

Functional goals and treatment are the basis for occupational therapy. Following upper extremity amputation patients can benefit significantly from a program that concentrates on setting and achieving functional goals. Few hand therapists have the opportunity to treat a large number of amputees. This case study of treatment of above-elbow amputation discusses the use of an activities of daily living form as an aid to developing and meeting short-term goals in treatment planning.

Iatrogenic posterior interosseous nerve palsy following an elbow fracture.

Posterior interosseous nerve palsy following elbow surgery is not uncommon. Nevertheless, precise etiology of the palsy is rarely described in the literature. In this paper the possible causes of the posterior interosseous nerve palsy are reviewed and an uncommon iatrogenic lesion after surgical treatment of an elbow fracture is reported.

Results of functioning free muscle transplantation for elbow flexion.

Thirty-eight patients underwent functioning free muscle transplantation for restoration of elbow flexion. Thirty-five patients had sustained brachial plexus injury and 3 had traumatic loss of the biceps muscle. The gracilis muscle was used in 37 patients and the rectus femoris muscle in 4. The transferred muscle was reinnervated by the musculocutaneous (n = 3), intercostal (n = 31), and spinal accessory (n = 4) nerves. Results were assessed by the Medical Research Council grading system and success was defined as a muscle strength of M4. Reinnervation with the musculocutaneous nerve resulted i…

[Arthroscopy of smaller joints: hand, elbow and ankle arthroscopy].

These joints are accessible, though technically demanding, by arthroscope. Above all, the evaluation of the lesions is not simple. Arthroscopy always represents the last diagnostical alternative. The extraction of loose fragments, the debridement, the shaving and the treatment of infections are excellent indications. Postoperative rehabilitation is favoured and shortened with the arthroscopic approach. Elbow arthroscopy did not greatly, develop during the last years. Posttraumatic situations, corpora libera and cartilage lesions represent the main indications. The arthroscopies of the ankle an…

Radial head dislocations in children with below-elbow deficiencies.

Two thirds of the children who have below-elbow deficiencies, congenital or acquired, have concomitant radial head dislocations. The direction of the dislocation depends largely on the length of the residual limb. The dislocation does not require specific surgical treatment and rarely necessitates prosthetic modification.

Radiographic Study of Elbow Injuries in Professional Rodeo Cowboys.

In brief: A radiographic study of 21 bull riders and 18 bareback riders was conducted to determine the bony changes that result over time from elbow injuries in professional cowboys. X-rays revealed 39 bony abnormalities in the bull riders and 37 in the bareback riders. More than 60% of these changes occurred in the medial epicondyle and the posterior aspect of the olecranon process. Surprisingly, 30 of the injuries affected the free arm rather than the gripping arm. The authors recommend that riders tape both elbows to prevent hyperextension and that they wear a pad for protection from an ani…

Utilising an internal joint stabilizer to ensure post-release elbow stability for patients with severe post-traumatic elbow stiffness: A retrospective cohort study.

OBJECTIVE: The treatment of severe post-traumatic elbow stiffness is extremely complex. Complete open release of the elbow joint and reconstruction of stiffness-related injuries are considered crucial; however, these procedures may lead to elbow instability, particularly chronic instability due to underlying conditions. This retrospective study aimed to assess the outcomes of using an internal joint stabilizer (IJS) to ensure post-release stability in these complex cases. HYPOTHESIS: IJS would demonstrate reliability as a treatment technique for severe elbow stiffness associated with intraope…

Triceps-sparing approach results in a better elbow function than a triceps-detaching approach in total elbow arthroplasty: a multicenter prospective cohort study.

BACKGROUND: New surgical approaches have been developed to optimize functional outcomes after total elbow arthroplasty (TEA). Historically, the triceps-detaching approach was predominantly used, involving partial release of the triceps tendon to improve surgical exposure, at the cost of a higher risk of post-operative extensor weakness. More recently, there has been a shift toward triceps-sparing techniques, in which the triceps tendon is preserved at the cost of limited articular visibility, possibly risking component malalignment. There is no consensus on the best surgical approach, and pros…

Approach to pediatric patients with elbow injuries in the emergency department.

Pediatric elbow injuries are a common complaint presenting to emergency departments. The unique anatomy and skeletal immaturity in children can make the elbow particularly difficult to evaluate and manage. Additionally, many healthcare institutions have limited or no access to pediatric orthopedic specialists, leaving management decisions to the clinician at the bedside. This review highlights the nuances of the pediatric elbow examination, addressing both acute and subacute (overuse) injuries. Common fracture types are reviewed, and key principles of x-ray interpretation are demonstrated, inc…

Fatigability of elbow muscles in children and adolescents with spina bifida.

CONTEXT/OBJECTIVE: Due to paralysis in the lower limbs, individuals with spina bifida (SB) rely heavily on upper limb muscles for daily tasks and assisted locomotion. Therefore, reduced upper limb muscle performance may be a key limiting factor. The aim was to investigate differences in elbow muscles strength and fatigability between children and adolescents with SB and their healthy peers. DESIGN: Cross-sectional study. SETTING: Ribeirão Preto Medical School, University of São Paulo, Brazil. PARTICIPANTS: Twenty-three youth with SB (SB, mean age 10.83 years) and 84 age-matched children and…
