Between February and October 1987, a febrile illness killed 14 persons and seriously affected at least 14 others in Shumpillan, a remote Peruvian mountain village of 353 people. The illness was characterized by fever, headache, chills, and pallor. The fatality rate of untreated cases was 88%. The patients, 71% of whom were male, were 1-75 years of age. Fatal illnesses progressed from lethargy to coma to death in 3-60 days. Patients treated empirically with chloramphenicol survived. Bartonella bacilliformis was isolated from the whole blood of 3 patients. A serologic study revealed a high preva

The first occurrence of Botulism described in Perú took place in Huancayo city, in November 1988. Twelve persons became ill, two of them died. All of them ate "salchipapas" a very popular meal in the country done with fried potatoes, hot dogs, eggs and dressed with mayonnaise) in a restaurant near their working place. Six of the twelve patients were immediately taken to Lima and admitted in Edgardo Rebagliati Martins National Hospital from the Social Security Peruvian Institute, and treated by the International Medicine Department. We studied five of the six patients; they all had similar symp

The retrospective analysis of dysentery morbidity in Blagoveshchensk for the period of 1960-1987 was made. The regularities linking general natural and biological factors triggering the epidemic process with dysentery morbidity among the population are emphasized. The study revealed that under the conditions of Blagoveshchensk dairy products were of major epidemic importance among factors contributing to the transmission of dysentery. Such a factor as flies also had a definite influence on the epidemic process of dysentery. Another risk factor was drinking water which influenced the epidemic p

Audiological and other long-term neurological sequelae were determined in 157 cases and their controls matched for age, sex and village 6 to 12 months after an epidemic of group A meningococcal meningitis in rural West Africa. 19 cases (12.1%) and 3 controls (1.9%) had moderate or severe neurological sequelae of any type (P less than 0.001); 6 cases (3.9%) and no controls had severe or profound sensorineural hearing loss (P = 0.03). There was no difference in conductive hearing loss between cases and controls. Other cranial nerve sequelae (except visual defects) and generalized neurological an

In 1982, an epidemic of acute trichinosis occurred in Bitburg with 402 cases. The acute stage was observed and documented in 193 patients. The first signs of the disease appeared on average 17 days after eating trichina-containing meat. Muscle pain was reported by 85% of patients, lid and/or facial edema by 83%, gastrointestinal complaints by 62%, fever by 60%, flu-like symptoms by 56%, headache by 26%, eye complaints by 19%. Eosinophils were present in the blood of 92%. Trichinella antibodies were demonstrated in 98% of patients examined for them. Interstitial myositis was found in 90% of mus

Epidemics of measles had been seen biennially over the past two decades at Princess Mary Hospital, Auckland, until 1980. After a four year gap, there has been a further epidemic. The medical records of 201 measles cases seen between July 1984 and August 1985 were reviewed. Maori and Pacific Island children were over-represented (86%), as were children aged less than 2 years (53%). Two children died. The most common complications were otitis media (98 cases), pneumonia (70 cases) and croup (28 cases, for which five children were ventilated). The epidemic was chiefly due to low immunisation rate…

As a consequence of the rubella epidemic in Austria in 1979, prophylaxis against this disease was intensified. In spite of this measure, however, there was an increase in cases of rubella in 1984/85. The peak incidence in both years occurred in May. In 1984, most cases were found in Vienna, Lower and Upper Austria, and Salzburg, whereas in 1985, Styria and Vienna were most affected. The clinical diagnosis of rubella was confirmed serologically in 685 patients. Of these, 151 were pregnant women and 104 were women of childbearing age who gave no information as to pregnancy. Due to the virtually

22 cases of syphilis were diagnosed in the last part of 1982 in the Department of Dermatology and Venereology. Out of them 21 were male cases and 16 were homo- or bisexuals. Most cases were in the secondary stage and Herxheimer reaction only developed in these. Serological titers to both lipoidal and treponemal antigens were extremely high. All bisexual males were positive in herpes and CMV complement fixation test. Two of the homosexuals appeared with iridocyclitis and motile treponemes were isolated from the anterior chamber in one case.

Analysis of routine hospital statistics confirms anecdotal reports of an epidemic of surgery for glue ear. The surgical rate rose by 74% in Oxford Region between 1975 and 1981. The peak ages for surgery are 5-7 years, and the rates are higher in boys than in girls (1.4:1.00). The relative risk is high in social class I (1.16) and low in social class V (0.61). Although the prevalence of glue ear is hard to ascertain, the current surgical epidemic probably reflects an increase in apparent prevalence rather than a real increase in morbidity.

We investigated an outbreak of illness in electronics plant workers. Questionnaire data revealed that 98 employees experienced symptoms, including light- headedness , headache, sleepiness, and numbness/tingling of the face or extremities. Attack rates by work station followed no apparent pattern. Extensive medical and environmental evaluation, including air sampling for numerous solvents and gases, provided no physical or chemical explanation for the epidemic. Compared with well employees, ill employees were more commonly female, complained more frequently of bothersome odors, and believed tha

Five patients with hand-, foot- and mouth disease, caused by infection with Coxsackievirus type A16, are described in an epidemic outbreak of this disease. Hand-, foot- and mouth disease, is characterized by a most distinctive enanthem-exanthemcomplex. In 65% of the cases a vesiculopapular exanthem is noted. Most cases are described in epidemic outbreaks. Although the vast majority of occurrences of the disease have been due to Coxsackie A16 viral infections, occasionally Coxsackie A5, A10 and enterovirus 71, seldom Coxsackievirus B1 or B3, have been etiologically incriminated. South Africa has the highest motorcycle accident and associated casualty rate in the world. In dealing with the problem South Africa lags far behind other countries where rider training programmes have had astonishing success. Accident statistics, problems relating to motorcycling and protective clothing are discussed. Recommendations are made which if implemented should reduce the accident and casualty rate by at least 50%; this indicates the size of the problem in this neglected field of automotive preventive medicine.

A series of 100 roller skating injuries is presented. Roller skating injuries have been occurring at a higher rate than the previously reported skateboarding epidemic of 1977. The severity of injury has been lower, 32% fractures and dislocations occurring whilst roller skating, compared to 60% whilst skateboarding. In particular a striking reduction is seen in ankle fractures. Fifty questionnaires detailing method of injury were analysed.
