Non-fear emotions in changes in posttraumatic stress disorder symptoms during treatment.

BACKGROUND AND OBJECTIVES: Posttraumatic stress disorder (PTSD) is not only associated with fear but also with other emotions. The present study aimed to examine if changes in shame, guilt, anger, and disgust predicted changes in PTSD symptoms during treatment, while also testing if PTSD symptoms, in turn, predicted changes in these emotions. METHODS: Participants (N = 155) with childhood-related PTSD received a maximum of 12 sessions of eye movement desensitization and reprocessing or imagery rescripting. The data was analyzed using Granger causality models across 12 treatment sessions and 6…

The Correlation Between Fear Avoidance Beliefs and Physical Activity in Unilateral Vestibulopathies.

BACKGROUND AND PURPOSE: In individuals with unilateral vestibulopathy (UVP), physical activity (PA) is recommended to stimulate central vestibular compensation. However, the presence of fear avoidance beliefs might negatively influence PA. The objectives of this study were to investigate the relationship between fear avoidance beliefs and PA and to compare PA levels between individuals with UVP in an acute/subacute vs chronic phase. METHODS: In this cross-sectional study, PA was measured using a triaxial accelerometer. Fear avoidance beliefs were quantified using the Vestibular Activities Avo…

Dynamic causal modeling reveals increased cerebellar- periaqueductal gray communication during fear extinction.

INTRODUCTION: The extinction of fear memories is an important component in regulating defensive behaviors, contributing toward adaptive processes essential for survival. The cerebellar medial nucleus (MCN) has bidirectional connections with the ventrolateral periaqueductal gray (vlPAG) and is implicated in the regulation of multiple aspects of fear, such as conditioned fear learning and the expression of defensive motor outputs. However, it is unclear how communication between the MCN and vlPAG changes during conditioned fear extinction. METHODS: We use dynamic causal models (DCMs) to infer e…

Contemporary Approaches Toward Neuromodulation of Fear Extinction and Its Underlying Neural Circuits.

Neuroscience and neuroimaging research have now identified brain nodes that are involved in the acquisition, storage, and expression of conditioned fear and its extinction. These brain regions include the ventromedial prefrontal cortex (vmPFC), dorsal anterior cingulate cortex (dACC), amygdala, insular cortex, and hippocampus. Psychiatric neuroimaging research shows that functional dysregulation of these brain regions might contribute to the etiology and symptomatology of various psychopathologies, including anxiety disorders and post traumatic stress disorder (PTSD) (Barad et al. Biol Psychia…

Exploration driven by a medial preoptic circuit facilitates fear extinction in mice.

Repetitive exposure to fear-associated targets is a typical treatment for patients with panic or post-traumatic stress disorder (PTSD). The success of exposure therapy depends on the active exploration of a fear-eliciting target despite an innate drive to avoid it. Here, we found that a circuit running from CaMKIIα-positive neurons of the medial preoptic area to the ventral periaqueductal gray (MPA-vPAG) facilitates the exploration of a fear-conditioned zone and subsequent fear extinction in mice. Activation or inhibition of this circuit did not induce preference/avoidance of a specific zone. …

Childhood trauma and fear of childbirth: findings from a birth cohort study.

The aim of this study is to investigate if experiencing childhood trauma (emotional abuse, emotional neglect, physical abuse, physical neglect, or sexual abuse) or a greater total burden of childhood trauma increase the risk of fear of childbirth (FOC). This study included 2556 women living in Southwest Finland. Women were recruited during routine ultrasound visits at gestational week (gwk) 12. Experiencing childhood trauma was assessed in retrospect with the Trauma and Distress Scale (TADS) questionnaire completed at gwk 14. Information on the diagnosis of FOC (ICD-10 diagnosis O99.80) was ob…

COVID-19 Fear Impact on Israeli and Maltese Female "Help" Profession Students.

BACKGROUND: The aim of this cross-sectional study was to examine the impact of COVID-19 fear on the well-being of Israeli and Maltese female "help" profession (e.g., social work and psychology) undergraduate students. This cross-national comparison includes factors of depression, anxiety, anger, loneliness, nervousness, substance use, eating behavior, burnout, and resilience. The study hypothesis is that country status, even with different social-cultural characteristics including religiosity, is not a significant factor associated with COVID-19 fear impact on select behavioral characteristics…

Generalization of beneficial exposure effects to untreated stimuli from another fear category.

Previous research has shown that fear associated with one stimulus often spreads to other stimuli with similar perceptual features as well as across different stimulus categories. Exposure is considered as the most effective intervention to attenuate exaggerated fear. The extent to which exposure treatment effects can generalize to fears not targeted during treatment remains elusive. Previous studies on possible generalization of beneficial effects of exposure used stimuli sharing the same stimulus category and/or stimuli having high perceptual similarity. The current study examined whether ex…

Adolescence alcohol exposure impairs fear extinction and alters medial prefrontal cortex plasticity.

After experiencing a traumatic event people often turn to alcohol to cope with symptoms. In those with post-traumatic stress disorder (PTSD) and a co-occurring alcohol use disorder (AUD), PTSD symptoms can worsen, suggesting that alcohol changes how traumatic memory is expressed. The objective of this series of experiments is to identify how alcohol drinking (EtOH), following cued fear conditioning and extinction, impacts fear expression in mice. Molecular (activity-regulated cytoskeleton-associated protein, Arc/arg3.1) and structural (dendrite and spine morphometry) markers of neuronal plasti…

Heightened generalized conditioned fear and avoidance in women and underlying psychological processes.

Heightened generalization of conditioned fear and avoidance to safe stimuli resembling threat is a key feature of pathological anxiety and might contribute to the increased prevalence of anxiety-related disorders among women. Though animal studies have documented over-generalized fear in female versus male rodents, analogous work in humans is sparse, and no studies to date have examined gender differences in generalized avoidance. We addressed this gap by testing 170 self-identified women (n = 85) and men (n = 85) using a video game-based task assessing generalized Pavlovian fear (perceived th…

Facing Your Fear in Immersive Virtual Reality: Avoidance Behavior in Specific Phobia.

Specific phobias are the most common anxiety disorder and are characterized by avoidance behavior. Avoidance behavior impacts daily function … hobic and 16 non-phobic participants. As the non-phobics were quite heterogeneous in terms of their Fear of Spiders Questionnaire (FSQ) scores, we subdivided them into six "fearfuls" that had elevated FSQ scores, and 10 "non-fearfuls" that had no fear of spiders. The phobics successfully managed to complete the procedure and showed consistent avoidance …

Modeling subjective fear using skin conductance: a preliminary study in virtual reality.

Reliably measuring fear perception could help evaluate the effectiveness of treatments for pathological conditions such as specific phobias or post-traumatic stress syndrome (e.g., exposure therapy). In this study, we developed a novel vir-tual reality (VR) scenario to induce fear and evaluate the related physiological response by the analysis of skin conductance (SC) signal. Eighteen subjects voluntarily experienced the fear VR scenario while their SC was recorded. After the experiment, each participant was asked to score the perceived subjective fear using a Likert scale from 1 to 10. We use…

Childhood obesity: overcoming the fear of having healthier weight conversations with families.

Introduction Clinicians may find raising the issue of weight with patients or carers and having healthier lifestyle conversations uncomfortable, out of fear or experience of causing offence. A two-cycle audit was completed in a specialist paediatric dental service to ascertain whether healthier weight conversations were being had with patients and their carers.Materials and methods The inclusion criteria for the audit were paediatric patients who were having a general anaesthetic assessment for dental extractions as a result of dental caries. A gold standard was set that all patients having a …

Conditional effects of state fear of negative evaluation on restrictive eating behaviors.

Fear of being evaluated negatively by others is a risk factor for the development of disordered eating. Specifically, trait level fear of negative evaluation (FNE) predicts disordered eating severity above other social anxiety traits. However, it remains unclear how state levels of this fear may impact eating behavior. The current study aimed to examine this risk factor in the context of state-level eating behavior. We hypothesized that: (a) elevated levels of state FNE would enhance the relationship between restraint and restriction and (b) we sought to explore the predictive nature of high t…

Fear of COVID-19 Scale (FCV-19S) across countries: Measurement invariance issues.

AIM: The threats of novel coronavirus disease 2019 (COVID-19) have caused fears worldwide. The Fear of COVID-19 Scale (FCV-19S) was recently developed to assess the fear of COVID-19. Although many studies found that the FCV-19S is psychometrically sound, it is unclear whether the FCV-19S is invariant across countries. The present study aimed to examine the measurement invariance of the FCV-19S across eleven countries. DESIGN: Cross-sectional study. METHODS: Using data collected from prior research on Bangladesh (N = 8,550), United Kingdom (N = 344), Brazil (N = 1,843), Taiwan (N = 539), Ital…

Sex-dependent effects of endocannabinoid modulation of conditioned fear extinction in rats.

BACKGROUND AND PURPOSE: Women are twice as likely as men to develop post-traumatic stress disorder (PTSD) making the search for biological mechanisms underlying these gender disparities especially crucial. One of the hallmark symptoms of PTSD is an alteration in the ability to extinguish fear responses to trauma-associated cues. In male rodents, the endocannabinoid system can modulate fear extinction and has been suggested as a therapeutic target for PTSD. However, whether and how the endocannabinoid system may modulate fear expression and extinction in females remains unknown. EXPERIMENTAL A…
