Q fever: current concepts.

Persons with Q fever usually present with severe retrobulbar headache, a fever to 104 degrees F or higher with shaking chills, general malaise, myalgia, chest pain, and sometimes pneumonia and hepatitis. Cattle, sheep, goats, and ticks are the primary reservoirs of the etiologic agent, Coxiella burnetii. Humans are usually infected by inhaling infectious aerosols. Because C. burnetii can survive for long periods in the environment, it poses a continuing health hazard once it is disseminated. Q fever usually occurs sporadically, but large outbreaks are frequently observed throughout the world, …

Q fever in France.

In France Q fever is a notifiable disease, but information could only be obtained from fragmental inquiries. Routine serologic examination of 75 901 sera during the years 1982-85 showed 430 Q fever positive sera (mean rate 0.57%). Our laboratory could confirm 135 cases of clinical Q fever. In the region of Dijon, 4.4% of the country population was seropositive, but among breeders 25% were positive. 36 persons (8%) among 245 employees of the departmental veterinary services were seropositive, some with very high titers, but without obvious clinical symptoms. It seems that infection by Coxiella …

[Typhoid fever in childhood].

Typhoid fever is an uncommon disease in the Netherlands. Acquisition occurs mainly during holidays in an endemic area. The history of three children, admitted in the summer of 1985, will be discussed, in one of them the disease had a complicated course. A short review of the literature is given, especially regarding pathogenesis, diagnosis and treatment. Cultures of blood, faeces and bone-marrow are essential for diagnosis. Bone marrow culture remains the most effective method for recovery of the causative agent, especially in children who previously were treated with antibiotics. Chlorampheni…

Trimethoprim in enteric fever.

Seventy-one patients suffering from typhoid or paratyphoid fever were treated with trimethoprim. Sixty-three were cured. Only three patients continued to excrete Salmonella typhi in stools at the time of discharge from hospital. Trimethoprim alone is a suitable agent for the treatment of enteric fever.

Typhoid fever complicating pregnancy.

Typhoid fever in pregnancy may result in infection of the fetus, with consequent abortion. Currently, it is a rare complication of pregnancy in the industrially developed countries. This report documents a case of maternal typhoid fever with septic abortion, in which trans-placental infection of the fetus caused its demise.

Subclinical chronic Q fever.

Seven patients are described in whom chronic Q fever was detected by serology (Coxiella burneti phase I antibody titre greater than 1:200) during routine screening at admission for cardiac catheterisation. None had clinical evidence of endocarditis, hepatitis or other foci of infection. Three of the patients were kept under observation without antibiotic treatment for periods of six, 18 and 20 months. In two patients of this group, cardiac tissue was obtained at operation and in one patient seroconversion following guinea-pig inoculation indicated the presence of Coxiella burneti infection. Fo…

[Boutonneuse fever in children].

Due to an increase in the number of cases of spotted fever, a prospective study was undertaken in 31 children affected by the disease, including clinical, laboratory and microbiological data from may to october of 1983. The most frequent symptoms found were: fever 100%, "tache noire" 87%, and a maculopapular rash 81%. The Weil-Felix reaction was positive in 65% of them and indirect immunofluorescence was positive in 81%, confirming the sensitivity and specificity of this serologic procedure. The 27 children who received specific treatment, 26 with tetracycline and one with chloramphenicol, dif…
