Reverse dorsal digital and metacarpal flaps: a review of 27 cases.

Reverse dorsal digital and metacarpal flaps use the dorsal skin of the digital or metacarpal areas, and they are based on the arterial branches anastomosing the volar and dorsal arterial networks of the fingers. These flaps are transposed as reverse island flaps. Dissection of the flap is easy, fast, and preserves the collateral nerve and artery to the fingertip. A series of 27 flaps is reviewed, with more than 6 months of follow-up. Skin defects in all patients were located over or beyond the proximal interphalangeal joint as far as the fingertip and were combined with bone, joint, or tendon

The role of joint receptors in human kinaesthesia when intramuscular receptors cannot contribute.

1. Kinaesthetic acuity was tested at the distal interphalangeal joint of the middle finger when the hand was postured so that the joint was effectively disengaged from its muscular attachments. Subjects were required to detect the direction of 5 deg movements applied at different angular velocities from a mid-position under control conditions, after intra-articular injection of a plasma expander and after intra-articular injection of a local anaesthetic. 2. Kinaesthetic performance was enhanced after the injection of a plasma expander and deteriorated after injection of local anaesthetic. This

Pachydermodactyly in an 11-Year-Old Female: A Multimodal Ultrasonography Perspective.

BACKGROUND: Pachydermodactyly (PDD) is a rare benign fibromatosis that mimics inflammatory arthritis, posing diagnostic challenges in pediatric populations. CASE PRESENTATION: We report an 11-year-old female with unilateral proximal interphalangeal (PIP) joint swelling. IMAGING FINDINGS: Gray-scale ultrasound revealed symmetric dermal thickening (2.02 mm) with hypoechoic texture, while Doppler confirmed absent vascularity. Shear wave elastography demonstrated elevated tissue stiffness (shear wave velocity: 4.8 m/s). CONCLUSION: This case underscores the utility of non-invasive multimodal ul

Dynamic circle traction for severely comminuted intra-articular finger fractures.

Treatment of severely comminuted unstable intra-articular finger fractures has three goals: reconstruction of the articular surfaces, prevention of redislocation of those surfaces, and proper healing of the capsuloligamentous apparatus. Dynamic circle traction (DCT), as described by Schenck (1986), is based on these principles and is the first method of treatment combining traction with intermittent or continuous passive motion. In order to improve on this method we applied continuous, instead of intermittent, passive motion in a newly developed DCT-device (continuous dynamic circle-traction =

Zone III Small Finger Both Flexor Tendon Rupture after a Single Corticosteroid Injection: A Unique Case Report.

INTRODUCTION: Finger flexor tendon injuries commonly occur as a result of direct trauma. On rare occasions, spontaneous ruptures can occur. Many pathologic processes have been described in the literature contributing to spontaneous finger flexor tendon ruptures, with most leading to progressive weakening of the tendon until it ultimately fails. We present a unique case of a Zone III spontaneous combined flexor digitorum superficialis (FDS) and flexor digitorum profundus (FDP) rupture of the small finger in a 66-year-old female, 3 weeks after a corticosteroid injection for trigger finger. To th

Design and Development of a Topology-Optimized Three-Dimensional Printed Soft Gripper.

In the past decade, a rich repertoire of soft robots, designed from biomimetic and intuitive approaches, has been developed to overcome challenges faced by their rigid-bodied counterparts. However, these design approaches are greatly limited by the designers' experience and inspiration. In this article, the structural design problem is mathematically modeled under the framework of topology optimization, and solved by a new implementation tool that combines Abaqus/CAE and Matlab coding. Herein, a pneumatic soft gripper with two identical fingers was developed as a practical application. To fulf

Swan-neck deformity: mechanism and surgical treatment.

There are several forms of swan-neck deformity which are amenable to surgical treatment with good results, if based on an adequate study of the mechanism. Selection of the most suitable surgical procedure must be based on the pathological type.

Dynamic Tenodesis Technique for Ulnar Drift With Extensor Tendon Subluxation due to Rheumatoid Arthritis.

PURPOSE: We investigated the outcomes of an original functional reconstruction procedure for ulnar drift (UD) with extensor tendon subluxation due to rheumatoid arthritis. METHODS: From 2018 to 2022, reconstruction was performed in 10 patients (31 fingers), with UD due to rheumatoid arthritis and a minimum of 2-year follow-up. The mean age was 57 years, and the mean follow-up period was 4.5 years. Ulnar drift was assessed using the Fearnley classification and included 17 stage 1 cases and 14 stage 2 fingers. A half-slip tendon was created just above the metacarpal head. An anchor was driven i

[Orthopedic therapy in rheumatic diseases].

The treatment of rheumatic disease requires an interdisciplinary approach in which the orthopedic surgeon is also involved. His contribution includes both conservative and operative procedures. Early synovectomy applied to joints and tendons is considered a preventive measure. Arthroscopic synovectomy belongs in the hands of an experienced operator. Joints that are not directly involved in weight-bearing can be treated by means of arthroplastic procedures, provided the cooperation of the patient is ensured. Arthrodesis is used preferentially to treat articular destruction of the ankle joint, t

Proximal interphalangeal joint prosthetic arthroplasty.

Surgical treatment of the painful proximal interphalangeal (PIP) joint has evolved considerably over the past century. If arthrodesis has long been the gold standard and remains so for some, prosthesis would meet patient demand: mobility and indolence. Faced with a demanding patient, the surgeon must decide the indication, the type of prosthesis, the approach, the post-operative follow-up, etc. The history of the development of PIP prostheses demonstrates the complexity of the treatment of the destroyed PIP: appearance of prostheses, evolution and sometimes disappearance from the market for co

Posttraumatic ulnar subluxation of the extensor tendons: a reconstructive technique.

Ulnar subluxation of the extensor digitorum communis over the metacarpal head can compromise the function of the extensor mechanism to the affected digit. Extensor displacement can be posttraumatic, developmental, congenital, or caused by arthritis of the metacarpophalangeal joint. A tendon centralizing procedure was performed on five digits in three patients who had posttraumatic ulnar subluxation of the extensors at the metacarpophalangeal joint after conservative management failed. No recurrences of subluxation have been noted in this series, and all patients have regained full range of mot

Swelling, Stiffness, and Dysfunction Following Proximal Interphalangeal Joint Sprains.

PURPOSE: Proximal interphalangeal (PIP) joint sprains are common injuries that often result in prolonged swelling, stiffness, and dysfunction; however, the duration of these sequelae is unknown. The purpose of this study was to determine the duration of time that patients experience finger swelling, stiffness, and dysfunction following a PIP joint sprain. METHODS: This was a prospective, longitudinal, survey-based study. To identify patients with PIP joint sprains, the electronic medical record was queried monthly using International Classification of Disease, Tenth Revision, codes for PIP jo

Silicone implant arthroplasty in patients with idiopathic osteoarthritis of the metacarpophalangeal joint.

PURPOSE: The outcome of silicone metacarpophalangeal (MCP) joint implant arthroplasty in the osteoarthritic patient population has not been well established. Typically patients with idiopathic osteoarthritis have no history of underlying systemic disease and may respond well to treatment with MCP joint implant arthroplasty. This study examined the efficacy of silicone MCP joint implant arthroplasty for patients with idiopathic osteoarthritis for whom nonsurgical treatment had failed. METHODS: Of 14 patients (15 arthroplasties) who had silicone MCP joint implant arthroplasty for idiopathic ost
