Two successfully treated cases of benign colovesical fistula are presented. The condition should be suspected whenever patients with colorectal disease develop urinary symptoms. Flexible colonoscopy and a lateral abdominal X-ray should be the initial investigations; but barium enema, cystoscopy and, rarely, CT scan may be necessary to confirm the presence of a colovesical fistula. Patients should be managed conservatively during the acute phase, and then be treated definitively by a single-stage bowel resection. Simple closure or resection and closure is used for the bladder-defect.

Review of vesicointestinal fistulae cases treated in the Urology Unit at our hospital over the last few years. Most frequently seen cause was colorectal cancer (66%) and most frequent location was vesicosigmoideal (50%). Clinical signs and symptoms were varied, mainly urinary infection in 100% cases, pneumaturia in 66% and fecaluria in 50%. Opaque enema and cystography were great diagnostic aids as complementary methods. Also U.I.V., cystoscopy, rectoscopy and C.A.T. were used. Treatment was surgical in all cases where the patient's general health status allowed it.

Nineteen patients with iatrogenic urogenital fistulae were treated between 1985 and 1989. All resulted from obstetric and gynaecological surgery. Major symptoms were leakage of urine through the vagina, menouria and perineal dermatitis. The fistulae formed after treatment for benign conditions in 18 patients. Fistulae complicating cancer surgery were uncommon. All patients with uretero-vaginal fistulae underwent preliminary percutaneous nephrostomy. Twelve patients underwent reconstruction using either the Boari-Ockerblad flap (5), uretero-neocystostomy (4) or other techniques (3), all with ex…

On the basis of 32 surgical cases of recurrent urorectal fistulas, review of relevant literature, the authors define factors determining choice of treatment and preoperative preparation policy. Such individual approach may secure success in many cases. A new surgical procedure of surgical treatment of urethrorectal fistulas involving abdominoanal resection of the rectum with implantation of the sigmoid into the anus has been tried and found satisfactory.

Aortoenteric fistulas were first reported in 1822. Primary aortoenteric fistulas are uncommon (less than 200 cases reported). Secondary aortoenteric fistulas are a well-recognized complication of prosthetic grafts (incidence from 0.4 to 2.4%). Atherosclerosis, gallstones, foreign bodies, carcinomas, and diverticular disease are the most common etiologies. Diagnosis is difficult with most studies being nondiagnostic. A high incidence of suspicion is required to successfully diagnosis preoperatively. Surgical repair is required for survival of the patients and should consist of the following: 1)…

World Health Organization (WHO) estimates that in the developing countries 5 million women each year suffer severe maternal morbidity and high on this list is obstetric fistulae (vesico-vaginal and/or recto-vaginal). This article tries to explain why these fistulae still occur, comments on measures of prevention and how treatment might best be provided for those women who suffer from this terrible affliction.

Mammillary fistulae develop between the lactiferous ducts of the breast and areolar skin. Over a 14-year period 41 fistulae were treated in 36 patients (34 women, 2 men). In 24 women at least one subareolar abscess (mean 2.5) had been incised and drained previously. Fourteen fistulae developed after discharge of an inflammatory mass and three after surgical biopsy. Twelve women underwent 13 fistulectomies, two of which required reoperation. The remaining 24 patients with complicated fistulae (n = 28) had previously undergone multiple surgical procedures; 12 patients had 13 fistulae treated by …

Carotid cavernous fistulas treated by endovascular means provide nurses with an excellent opportunity to use their skills in assessment. With knowledge of the local anatomy, that assessment must include evaluation of cranial nerves II through VI, to establish a neurological baseline and monitor changes. The patient problems attributed to pathology are multiple, and require active nursing intervention to optimize recovery and return to independent function.

The current incidence of purulent inflammatory fistulas is 31.1%. An analysis of 36 patients with these fistulas has identified stages of their formation. Anaerobic flora and a primary cause of pelvic and adnexal abscess formation were major determinants of fistula formation.

Three cases of the bladder-uterine fistulas treated within the last three years (1982-1984) have been presented. All patients underwent cesarean sections (two patients once, and the third - thrice). The patients were treated surgically. Each one has undergone hysterectomy, and the opening of the fistula into the bladder has been operated by incision of the bladder in two cases and through extracystic approach in one case. Mechanism of development, symptoms, various types, diagnosis, and therapy of the bladder-uterine fistulas have been discussed.

Four cases of superficial peri-anal fistula occurring in early infancy are described. They were all successfully treated by complete excision of the fistula. Histological examination of the excised fistulae revealed a mixture of columnar, transitional and stratified squamous epithelium lining the tracts in three cases and non-specific connective tissue in the fourth. It is postulated that the presence of these inappropriate types of epithelium (columnar and transitional) along the length of the fistulous tract is evidence of a congenital origin for peri-anal fistulae presenting in the first fe…

Thorough understanding of the pathophysiology of anal fissures and fistulas is essential for the physician treating these conditions. An acute fissure often heals in a month with conservative management; long-standing disease is relieved by lateral internal sphincterotomy. An acute fistulous abscess must be drained by unroofing the abscess rather than by simple incision, which fosters recurrence. Chronic fistula in ano requires fistulotomy, which can be done with a local anesthetic. Fistulas with multiple external orifices should be treated by a specialist in rectal surgery.

Perilymph fistulas may manifest themselves either by otologic symptoms, as in cases of small fistulas, or primarily through neurologic symptoms, as with large fistulas. A case history is presented of a 3-year-old girl with recurrent bouts of meningitis who was found to have a large perilymph fistula. She had a right undeveloped cochlea and labyrinth, as well as multiple defects of the medial wall of her middle ear. The only way of preventing the recurrent bouts of bacterial meningitis in cases of large perilymph fistulas is surgical repair. Our conclusion is that, considering the problems invo…

Twenty-one patients had surgical procedures for symptomatic cogenital lacrimal fistulas. The symptoms were epiphora from birth in 18 patients and late-onset epiphora in three cases. Thirteen patients had nasolacrimal obstruction at the upper duct or sac level. There were 21 dacryocystorhinostomies with fistula excision and canalicular intubation, and one patient had excision of the fistula tract only. The fistulas originated from the tear sac in only four cases and from the common canaliculus in 17; in one, the upper and lower canaliculus and the lacrimal fistula each had a separate opening in…

A review is given on frequency, etiology, anatomopathology and physiopathology of vesico-intestinal fistulas. Symptomatology and diagnosis with stress on adjuvant technical investigations are studied. In conclusion the principles for a surgical treatment are discussed.
