Linear scleroderma is a subtype of localized scleroderma generally observed in children, and may produce secondary bone and joint deformities. Its localization at the foot or ankle is rarely reported. A complete review of the literature reveals 21 cases of foot or ankle deformity due to linear scleroderma to which we add one case. The clinical and radiological aspects of the deformity, its pathogenesis, histological findings as well as orthopaedic management are presented and discussed.

OBJECTIVE: Hand-Foot-and-Mouth Disease (HFMD) is a mild exanthematous illness seen worldwide, affecting mainly children under ten years of age. The causative agents were initially Coxsackie virus type A 16 and related serotypes. The situation changed drastically about thirty years ago with the advent of a new aetiological agent, Enterovirus type 71 (EV 71), which has caused very large outbreaks with severe complications and many deaths.
METHODS: The authors report an outbreak of papulovesicular lesions on the skin and oral mucosa compatible with the diagnosis of HFMD in children in and around…

Directed injections of the foot and ankle performed under fluoroscopic guidance have both considerable diagnostic value and the potential for targeted therapy by accurately delivering corticosteroid and/or anesthetic to the source of pain. Both large and small joints in the foot and ankle are readily accessed, offering precise information to the foot and ankle surgeon who may be contemplating arthrodesis or other intervention. In addition to its role in pain management, ankle arthrography may be performed in conjunction with MRI or CT, with improved evaluation of articular cartilage and capsul…

An opening and a major stage of the diabetic septic foot surgical treatment is the primary surgical processing that is usually combined with amputation of separate fingers or with various types of transmetatarsal amputations. Out of the total of 379 operated patients 35.7% underwent amputation of separate fingers, combined with resection of the metatarsal bone head. 18.3% of the patients underwent transmetatarsal amputations by Sharp with plantar lambo shaping, and 14.9% had oblique resections with dermatological plastic surgeries. As a result of this treatment the foot amputations were reduce…

We present a case of ipsilateral dislocation of the knee, ankle and midfoot. No previous report of this combination of injuries was found in a review of the literature. In spite of the type of injury, no neurovascular compromise or complications turned up. Closed reduction of the dislocations was possible and performed under general anaesthesia. At the last examination, 4 years after injury, the alignment of the joints was preserved, and the range of motion of the three joints resembled that of the opposite side. Without fractures, the closed reduction and temporary immobilisation of all the a…

Reconstruction of the diabetic foot presents significant challenges for the surgeon. The goals of treatment are correction of deformity as well as elimination of infection, with production of a stable, plantigrade foot. Certainly not all patients with Charcot deformities require reconstruction. Many can be effectively managed with proper shoe modifications with orthoses. Bracing is also effective in providing support and in preventing further deformity. Surgical treatment is indicated for patients with recurrent ulceration or an unstable foot. Reconstruction of the Charcot foot should eliminat…

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a rare disease of uncertain etiology usually affecting the synovium of weightbearing joints.
METHODS: We retrospectively evaluated 11 patients who were diagnosed and treated for PVNS of the ankle and foot over a 13-year period with a minimum of 2-year followup. Four patients with ankle joint PVNS and one patient with PVNS of the fifth metatarsophalangeal joint were seen initially at our institution and were treated with surgery alone. Six patients with ankle joint PVNS were referred to our institution for recurrent PVNS lesions; two of th…

This review covers the perioperative management of trauma to the foot and ankle. The goal when treating these injuries is to return the patient to a sensate, plantigrade, painless, and functioning foot and ankle. Depending on the nature of the trauma, realistic outcomes should be established for the patient, family, and surgeon. The importance of early recognition and treatment of foot and ankle injuries has been established and is paramount for the overall recovery of traumatized patient.

The complexity of foot and ankle anatomy and function is unique in the musculoskeletal system. Understanding the complex anatomy alone is a daunting task, not to mention transferring that understanding to the two-dimensional planes encountered on imaging studies. When evaluating sports injuries in the foot and ankle, the interpreting radiologist must take into account the type of activity, vector of stress, and inherent characteristics of the involved structures. A strong working relationship with the health care providers managing patient care, ideally orthopedists, is essential. But in this …

Although reconstruction of complex distal lower extremity trauma has been studied extensively, the subject has rarely been evaluated in the pediatric population. The authors review their experience with 18 patients ranging from 2 to 18 years of age, who were admitted to the trauma service of the Children's Hospital of the University of Pittsburgh between 1991 and 1999 and required plastic surgery evaluation for the management of foot and ankle injuries. The mechanism of injury included motor vehicle accidents (6), lawnmower injuries (6), gunshot wounds (2), crush injuries (2), burns (1), and c…

Foot compartment syndrome is a serious potential complication of foot crush injury, fractures, surgery, and vascular injury. The purpose of this article is to summarize and review the existing literature on this entity. Long-term sequelae of foot compartment syndrome (FCS) include contractures, deformity, weakness, paralysis, and sensory neuropathy. These complications are poorly tolerated, and often necessitate multiple procedures for rehabilitation. Therefore, the threshold for considering compartment syndrome and performing fasciotomy must be low to minimize such outcomes. The existence of …

Fibrosarcoma is an uncommon, malignant soft-tissue tumor that is rarely found as a primary neoplasm in the foot. A case report is presented that demonstrates a large, locally invasive fibrosarcoma of the plantar aspect of the foot with initial symptoms consistent with plantar fasciitis. Below-the-knee amputation was performed as curative treatment.

Both chronic as well as acute foot and ankle pain are common indications for visits to general practitioners, rheumatologists, and orthopedic surgeons. The causes for foot and ankle pain are varied and can include osseous pathology (fractures), yet the far greater majority are secondary to soft-tissue injuries and inflammation. Regional corticosteroid injections, traditionally performed using anatomic landmarks, can be inaccurate and miss intended targets. The use of ultrasound for guidance for interventional radiologic procedures is well known, including guidance for vascular as well as visce…

Conventional radiographs in conjunction with clinical examination remains the primary method for evaluating the acute athletic injury. In most cases, suspected acute tendon and ligament injuries are initially treated based on physical examination. Magnetic resonance (MR) imaging, with its multiplanar capability and superb soft tissue contrast, is quickly becoming the method of choice for evaluating chronic foot and ankle pain and further defining the extent of tendon and ligament injuries. This article reviews the common acute and chronic (overuse) foot and ankle athletic injuries with an emph…

Hindfoot pain from tendon pathology is common and seen in a wide range of patients from young athletes to older sedentary individuals. Magnetic resonance (MR) is an excellent technique for imaging tendons and for identifying injuries that can be treated with surgery. MR also demonstrates the presence of bone marrow edema, which is a cause of pain and often a marker for adjacent tendon injury. Finally, MR can reveal other etiologies that cause similar hindfoot pain, such as osteochondral injuries and impingement. In this article, we review the normal MR appearance of the ankle tendons and tendo…

Over the years patients with relatively minor compressive injuries seem to have a different morbidity, largely because of a constellation of neurologic findings, including dysesthesia and hyperesthesias. These are usually multifocal, and often extend far beyond the point of impact of the crushing force. Neuroischemia may play a role in the development of chronic pain after crush injuries to the foot, either through direct trauma to the peripheral nerves or by intraneural or extraneural fibrosis. This direct trauma to the nerve may cause chronic neuritis, which then triggers a sympathetically m…

This case-control study examines whether foot problems are risk factors of fractures of five sites among people aged 45 years or older at six Kaiser Permanente Medical Centers in northern California. From October 1996 to May 2001, interviewers collected information through a standardized questionnaire. Incident cases of distal forearm (n = 1,000), foot (n = 827), proximal humerus (n = 448), shaft of the tibia/fibula (n = 168), and pelvis (n = 172) fractures and 1,913 controls from the same medical centers were included. After adjustment for potential confounders and for each additional foot pr…
