[Fractures of the midfoot].

The immediate reduction of luxations and severe dislocations is necessary to protect the soft tissue. The soft tissue lesions determine the timing and partially the options of the operative treatment too. The conventional standard X-rays are sufficient to classify most of the fractures. Conservative treatment or minimal-invasive intramedullary techniques of fixation are sufficient in the treatment in most of the fractures. Screw- and plate-osteosynthesis are a good choice only in intra-articular fractures or sometimes in fractures of the first or fifth metatarsals. The clinical outcome is almo

Preoperative planning technique for anterior pelvis percutaneous intramedullary screw fixation.

Useful techniques have been previously described for the planning of safe transsacral-transiliac screws, but to our knowledge, no straightforward technique has been described for anterior pelvic percutaneous fixation. As this method of anterior ring fixation has demonstrated biomechanical and clinical benefits, we propose a preoperative planning technique for determining the corridor characteristics of the patient's superior pubic ramus/anterior column for fracture fixation with percutaneous intramedullary screws. This technique helps the surgeon assess what diameter of solid or cannulated scr

Femur lengthening with monoplanar external fixator associated with locked intramedullary nail.

OBJECTIVE: This study aimed to demonstrate that the lengthening technique of an external fixator associated with locked intramedullary nail is an efficient method that decreases the duration of the external fixation and improves the rehabilitation period. METHODS: From January of 2005 to May of 2014, 31 patients with mean lower limb discrepancy of 5.31 cm were treated. The etiologies of the deformity were femur fracture sequelae, infection, hip development dysplasia, polio, and congenital short femur. RESULTS: The mean duration of external fixation was 2.47 months (external fixation index of

Benefits of early intramedullary nailing in femoral metastases.

PURPOSE: Pathological fractures of the long bones are common complications of metastatic disease; however, the outcome of different surgical techniques for the treatment of these fractures has not been clearly defined. The aim of this study was to evaluate differences in prophylactic and therapeutic intramedullary nailing in femoral metastasic implants. METHODS: Sixty-five patients with metastasis of the femur were analysed retrospectively (37 females; 28 males) between 1995 and 2011 (follow-up 15 months). Forty-four presented with pathological fractures and 21 impending fractures (Mirel ≥7).

Fractures in children.

The literature published in the past year on fractures in children has helped us to increase our understanding of the natural history of fracture healing in children and introduced new techniques for fracture treatment. There is an impetus for operative fixation of femur fractures in children. The possibility is raised of growth derangement as a result of intramedullary nailing. Supracondylar fractures of the elbow continue to be a frequent topic for discussion. The benefits of adequate reduction and internal fixation have been reinforced. The incidence of nerve palsy, especially that of the a

Pseudoaneurysm after pertrochanteric femur fracture: a case report.

Vascular injury as a consequence of dislocated bone fragments after hip fractures is a rare complication. We report a case of a 78-year-old male who developed a pseudoaneurysm of the profunda femoris artery during rehabilitation after intramedullary fixation for a pertrochanteric hip fracture. As this procedure was minimally invasive, open reposition and fixation of the dislocated lesser trochanter were not performed. The pseudoaneurysm was diagnosed through duplex ultrasound and CT angiography and treated with thrombin injections and an endovascular Wallgraft covered stent. The imaging featur

Simultaneous ipsilateral revision total hip arthroplasty and revision total knee arthroplasty with entire femoral allograft.

Treatment of periprosthetic fractures of both total hip arthroplasty and total knee arthroplasty has been well described in the literature. Treatments used have included closed treatment, revision arthroplasty with or without bone graft, open reduction internal fixation with plating, or intramedullary fixation [1-4]. However, treatment of periprosthetic fractures that involve a femur that has both a proximal total hip prosthesis and a distal total knee prosthesis has not been thoroughly described in the literature. We present a persistent distal femoral nonunion of a periprosthetic fracture, w

Challenges of perioperative pain control in opioid use disorder patients following intramedullary nail fixation.

BACKGROUND: Outcomes of patients with opioid use disorder undergoing elective procedures have been well studied, but research is lacking in the orthopaedic trauma population. AIM: The aim was to compare perioperative pain and morphine equivalents required by patients with versus without opioid use disorder following intramedullary nail fixation of femoral or tibial fractures. METHODS: We conducted a retrospective review of all patients with isolated femoral or tibial diaphyseal fractures treated with intramedullary nail fixation. Outcomes were compared between patients with diagnosed opioid

Acute Correction of Multiplanar Proximal Tibial Deformity Utilizing Fixator-Assisted Intramedullary Nailing.

UNLABELLED: Proximal tibial deformities, particularly varus deformities, are relatively common in adolescents and young adults. The etiology of these deformities is often untreated or undercorrected infantile and adolescent Blount disease. Other less common etiologies include metabolic bone disease-associated deformities, posttraumatic and iatrogenic growth disturbance, and deformity related to surgical treatment or radiation for tumors1-3. We apply the principles of fixator-assisted acute deformity correction, mostly described for use at the distal aspect of the femur, as well as the principl

Complications After Surgical Management of Proximal Femoral Metastasis: A Retrospective Study of 417 Patients.

BACKGROUND: Proximal femoral fractures resulting from metastatic disease often require surgical management. Few studies have compared surgical techniques, and physicians' preferred strategies vary. This study compared revision and complication rates among surgical strategies. METHODS: The study consisted of a retrospective review of electronic medical records of 417 consecutive patients with proximal femoral metastasis or multiple myeloma who underwent intramedullary nailing (n = 302), endoprosthetic reconstruction (n = 70), and open reduction and internal fixation (n = 45) between 1999 and 2

The Brooker-Wills interlocking nail in the treatment of femoral fractures.

Interlocking nails have expanded the indication for intramedullary fixation to almost any type of fracture of the shaft of the femur. Distal locking with the Brooker-Wills femoral nail is obtained by a wing fixator. This simplifies the operative technique and diminishes exposure to radiation. Adequate control of angulation, shortening and rotation could be achieved in a first series of 25 patients, thus leading to firm bone healing and to early functional recovery.

Retrospective evaluation of perioperative and short term clinical outcomes in appendicular long bone skeleton fractures repaired via the string of pearls (SOP) locking plate system.

BACKGROUND: Internal plate fixation and, more recently, locking plate fixation are commonly used in the repair of fractures in small animal surgery. This retrospective study reviewed the use of the String of Pearls locking plate system in the fixation/repair of appendicular long bone skeleton fractures in 31 small animal veterinary patients (33 fractures). RESULTS: Major complications necessitating revision surgery occurred in 7/33 (21%), with implant failure as the inciting cause in all cases. Variables corresponding to an unsuccessful outcome were evaluated, and a correlation was found with

Technical Trick: Supplemental Medial Column Screw Fixation of Distal Femur Fractures Treated With a Laterally Based Locked Plate.

Given known failure rates after lateral plating of distal femur fractures, there is an increasing interest in augmenting fixation to improve outcomes. The addition of medial plates or intramedullary nails have been described with promising results, decreasing nonunion and varus collapse rates. However, the use of dual implants increases implant costs, adds surgical complexity, and requires a second surgical approach that may increase morbidity. A supplemental, percutaneously placed, medial column screw may provide a less invasive means of improving stability and achieving fracture union compar

Nailing vs. plating in comminuted proximal ulna fractures - a biomechanical analysis.

BACKGROUND: Comminuted proximal ulna fractures are severe injuries with a high degree of instability. These injuries require surgical treatment, usually angular stable plating or double plating is performed. Nailing of proximal ulna fracture is described but not performed regularly. The aim of this study was to compare a newly developed, locked proximal ulna nail with an angular stable plate in an unstable fracture of the proximal ulna. We hypothesize, that locked nailing of the proximal ulna will provide non-inferior stability compared to locked plating. METHODS: A defect fracture distal to

Flexible intramedullary nailing for stabilization of displaced midshaft clavicle fractures: technique and results in 87 patients.

BACKGROUND: Elastic, stable intramedullary nailing (ESIN) with titanium nails is a promising minimally treatment for displaced midclavicular fractures, which may be an alternative to plate fixation (ORIF) or even nonoperative treatment. We describe the surgical technique and outcome in 87 patients. METHODS: The nail was inserted at the medial inferior end of the clavicle in 83 patients and in the acromial end in 12 patients. An open fracture reduction via an additional small incision was necessary in 53 patients and closed manoeuvre was successful in 42. Implant removal was performed in 82 pa
