Trimethylaminuria: causes and diagnosis of a socially distressing condition.

Trimethylaminuria is a disorder in which the volatile, fish-smelling compound, trimethylamine (TMA) accumulates and is excreted in the urine, but is also found in the sweat and breath of these patients. Because many patients have associated body odours or halitosis, trimethylaminuria sufferers can meet serious difficulties in a social context, leading to other problems such as isolation and depression. TMA is formed by bacteria in the mammalian gut from reduction of compounds such as trimethylamine-N-oxide (TMAO) and choline. Primary trimethylaminuria sufferers have an inherited enzyme deficie…

Listerine: past, present and future--a test of thyme.

Listerine, a mouthrinse composed of a mixture of essential oils, was created in 1879 and was originally formulated as a surgical antiseptic. In spite of its known antimicrobial properties it was thought of as a product in search of a use and promoted as a deterrent for halitosis and as a floor cleaner. In the last several years Listerine has emerged as a bona fide therapeutic agent for reduction of plaque induced oral diseases. In contrast to the inconsistent history of Listerine, systemic antibiotics discovered in the 1940's were heralded as miracle drugs. However, the value of prophylactic u…

Herpes simplex virus infection, with particular reference to the progression and complications of primary herpetic gingivostomatitis.

Primary herpetic gingivostomatitis (PHGS) represents the clinically apparent pattern of primary herpes simplex virus (HSV) infection, since the vast majority of other primary infections are symptomless. PHGS is caused predominantly by HSV-1 and affects mainly children. Prodromal symptoms, such as fever, anorexia, irritability, malaise and headache, may occur in advance of disease. The disease presents as numerous pin-head vesicles, which rupture rapidly to form painful irregular ulcerations covered by yellow-grey membranes. Sub-mandibular lymphadenitis, halitosis and refusal to drink are usual…

Interpreting and using clinical trials.

In 1754, aboard HMS Salisbury, James Lind conducted a simple, controlled clinical trial. He took 12 patients with "pale and bloated skin, listlessness, an aversion to exercise, swollen gums, halitosis, ecchymotic mucous membranes, and limb edema" and allocated them to receive treatment with one of six different therapies. Since the patients receiving two of his six chosen interventions had such a dramatic recovery, he felt ethically obligated to end his trial and administer these treatments to all the remaining sailors. Today we fully recognize the impact that the controlled clinical trial can…

Complications of paediatric tonsillectomy post-discharge.

A post-operative survey of 291 children was conducted to assess morbidity found at home in the first five days post-tonsillectomy. Analgesic requirement (92.4 per cent), signs of distress (90.4 per cent), otalgia (69.1 per cent), halitosis (66.7 per cent) were common. Physical or behavioural changes (36.8 per cent) and secondary haemorrhage (8.9 per cent) were also prominent features. Nausea was reported in 59 children (20.3 per cent) and delayed return to a normal diet which in turn predisposed to secondary haemorrhage. Return to normal diet was independent of pain. General Practitioners were…

Stannous Fluoride Toothpaste Stabilized With Nitrate and Phosphates (SNaP) Reduces Oral Malodor: A Randomized Clinical Study.

BACKGROUND: Oral malodor, whether from systemic disease, dietary sources, or bacteria in the oral cavity, can negatively impact patients' quality of life. Oral malodor due to bacteria in the oral cavity can be managed by mechanically or chemically removing bacteria. Dentifrices are ideal vehicles to deliver therapeutic active ingredients that promote and maintain oral health since most consumers brush their teeth daily. Consumer preference drives consistency in oral hygiene routine. This study first identified a favorite flavor via consumer flavor testing and then measured the clinical efficac

The effect of fixed appliances on oral malodor from beginning of treatment till 1 year.

BACKGROUND: Orthodontic appliances can enhance plaque accumulation, and this can cause gingival inflammation. Halitosis of oral origin is associated with microbial metabolism on the tongue and in the saliva, dental plaque, and the amount of volatile sulfide-containing compounds. This study used a Halimeter to investigate fixed orthodontic therapy-associated increases in the oral malodor over a year. METHODS: Thirteen orthodontic patients with Angle Class I malocclusions receiving fixed orthodontic therapy formed the study group, and 12 dental students without any dental treatment formed the c…

Resting salivary flow independently associated with oral malodor.

BACKGROUND: Dryness of the oral cavity is considered one cause of oral malodor. However, it is unclear which of the factors regulating the wetness of the oral cavity are involved in oral malodor development. This study investigated the effects of salivary flow and oral mucosal moisture on oral malodor. METHODS: The study population comprised 119 patients (48 men and 71 women, mean age of 50.6 ± 15.4 years) with complaint of oral malodor. After the oral malodor level had been evaluated by the organoleptic test and gas chromatography, the rates of stimulated saliva and resting saliva and the mo

Alternative medicine in periodontal therapy--a review.

Periodontal diseases continue to be most commonly occurring oral diseases in modern times. Many therapeutic modalities have been tried and tested to relieve these problems. The conventional therapy--scaling and root planing (SRP)--stands out to be the most used mode of treatment, and other treatments remain applicable as adjuncts to SRP, including acupuncture, acupressure, and aromatherapy. The present article discusses the applications of the abovementioned therapeutic modes and their relevance in current scenarios. Alternative medicine may be preferred as an adjunct to conventional periodont

Salivary β-galactosidase activity affects physiological oral malodour.

OBJECTIVES: Previous reports have associated salivary β-galactosidase activity with non-periodontopathic oral malodour. In this study, we investigated the localization of β-galactosidase and elucidated the relationship between its enzymatic activity and physiological oral malodour. STUDY DESIGN: Fifty-six patients complaining of halitosis were separated into two groups: periodontally healthy and periodontitis. Saliva samples from the subjects were separated by centrifugation, and the level of β-galactosidase activity was measured in the supernatant, pellet lysate, and whole saliva using the c…

Comparison of the use of the Halimeter and the Oral Chroma™ in the assessment of the ability of common cultivable oral anaerobic bacteria to produce malodorous volatile sulfur compounds from cysteine and methionine.

OBJECTIVE: To compare the use of the Halimeter and the Oral Chroma™ to assess the ability of common oral anaerobic bacteria isolated from the Kuwaiti population to produce volatile sulfur compounds (VSCs). MATERIALS AND METHODS: Broth cultures of common anaerobes isolated from supragingival plaque were centrifuged and pellets resuspended in phosphate buffer (pH 7.7) with an optical density OD(550) of 0.3. 100 μl of this suspension and 870 μl of buffer were added in 2 sterile 15-ml head space vials. Reaction was initiated by addition of 30 μl of 33 mML-methionine and L-cysteine, respectively,

Pilot study of a breath test for volatile organic compounds associated with oral malodor: evidence for the role of oxidative stress.

BACKGROUND: We performed a pilot study of a new method to identify the volatile organic compounds (VOCs) in breath associated with oral malodor, using gas chromatography and mass spectroscopy (GC/MS). METHODS: Oral cavity breath was collected from seven patients with oral malodor. Breath samples (150 ml) were concentrated onto sorbent traps and analyzed by GC/MS. RESULTS: Organoleptic scores ranged from 3.0 to 4.0 (mean = 3.3) on a scale of 0-5. Twenty-four of 30 (80.0%) of the most abundant oral malodor volatile organic compounds (OMVOCs) were alkanes and methylated alkanes. These VOCs are

Negative correlation between oral malodour and numbers and activities of sulphate-reducing bacteria in the human mouth.

The majority of cases of oral malodour are thought to be due to bacterial activities in the mouth, but many of the bacterial species responsible have not been identified. Volatile sulphide compounds have been proposed as constituents of oral malodour. Therefore, the relation between intensity of odour and numbers of bacteria in the mouth that are sulphide-producing from sulphate was investigated. Numbers of such dissimilatory sulphate-reducing bacteria (SRB) and sulphide reduction rates were evaluated in samples from different oral sites in relation to measures of oral malodour. Results showed

Psychiatric patients presenting with primarily somatic symptoms: report of two cases.

Two recent cases of psychiatric disorders, presenting primarily with physical symptoms are described, which suggest that both psychotic and psychoneurotic disorders can present with somatic symptoms for which no adequate physical cause could be defined. It is emphasised that such cases require specialist psychiatric evaluation and assessment for the appropriate management to be initiated. Recent studies have stressed the frequency with which psychiatric illness or psychological distress is presented in most African patients as somatic illness, especially in depressive illness, psychotic disord
