Do No Harm: A Multifactorial Approach to Preventing Emergency Department Falls-A Quality Improvement Project.

INTRODUCTION: Patient falls in the emergency department are a unique patient safety issue because of the often challenging nature of the environment. As there are a variety of potential causative factors for patient falls in the emergency department, this project employed a multifactorial approach to prevent patient falls in a Level 1 trauma center emergency department (adult only) in an urban tertiary care teaching hospital. METHODS: This project was a single-unit quality improvement intervention that compared postintervention monthly unit-level data to historic monthly rates on the same uni

[Specific mortality rates at the Algeciras hospital during 1995-1996].

BACKGROUND: In-hospital mortality has been proposed as an indicator of health care quality. It is also a fact that should be used for epidemiological monitoring and health planning. The aim of this work is to analyse the mortality in Algeciras Hospital, belonging to the Andalusia Health Authority. METHODS: A descriptive study was carried out of the patients discharged from hospital between January 1995 and December 1996, using discharge data and/or their medical record. The structure of the joint basic minimum data report on hospital discharge was used. The periods of hospitalisation were gro

Identification of Prenatal Opioid Exposure Within Health Administrative Databases.

BACKGROUND: Health administrative data offer a vital source of data on maternal prenatal opioid exposure (POE). The impact of different methods to estimate POE, especially combining maternal and newborn records, is not known. METHODS: This population-based cross-sectional study included 454 746 hospital births with linked administrative data in Ontario, Canada, in 2014-2017. POE ascertainment included 3 sources: (1) prenatal opioid prescriptions, (2) maternal opioid-related hospital records, and (3) newborn hospital records with neonatal abstinence syndrome (NAS). Positive percent agreement w…

A strategy for the reduction of formaldehyde concentration in a hospital pathology laboratory.

OBJECTIVES: Formaldehyde is routinely used in pathology laboratories. The threshold limit value of formaldehyde determined by American and Japanese organizations is 0.1 ppm, which is similar to the indoor air quality guideline value (0.08 ppm). Therefore, maintaining low formaldehyde concentrations in the workplace is imperative. The purpose of this study was to reduce the concentration of formaldehyde in a hospital pathology laboratory, in which approximately 15 000 pathological diagnoses are conducted yearly, using hardware and software interventions. METHODS: Although this laboratory had v

A model for developing and pre-testing a multi-media teaching program to enhance the self-care behavior of diabetes insipidus patients.

A patient-education program was designed by members of the Neurology Nursing Service. The program was designed to provide patients with diabetes insipidus with the information needed to help them manage their condition in the hospital and at home upon discharge. The program was approved by the Nursing Department's Patient Education Committee. The nurses worked closely with the hospital's patient-education coordinator and television-production staff to produce a videotape for educating patients about managing diabetes insipidus. A literature review of patient-education resources on diabetes ins

A scoping review of models of care and services for nausea and vomiting in pregnancy and hyperemesis gravidarum.

BACKGROUND: Nausea and vomiting in pregnancy and hyperemesis gravidarum have significant physical and psychological impacts on pregnant women. Women are often unable to participate in everyday care and work activities, potentially affecting individual wellbeing and ability to contribute to the economy. Research priorities for hyperemesis gravidarum have been established, however little is known about the current landscape of literature on the health and social care provided to affected women. To inform decision makers on effective models of care and identify remaining evidence gaps, this scopi

[Hospital morbidity determinants in a region of São Paulo State (Brazil)].

INTRODUCTION: Many changes in the morbidity of patients attended to the Country's hospitals have occurred in recent years, wether resulting from demographic, epidemiological, financial and technological factors or from those related to the structure of the services themselves. METHODS: The change in diagnoses occurring over this period was analyzed using two data banks relating to hospital discharges during 1975 and 1988, in the Vale do Paraíba, Southeastern Brazil, taking into account the factors mentioned above. Both data banks covered the same variables: hospital, dwelling, patient's age a

Compliance with a standard for the emergency department management of epileptics who present after an uncomplicated convulsion.

We conducted a retrospective chart review to determine whether the treatment of uncomplicated convulsions in emergency department patients with a history of epilepsy complied with a clinical standard and whether the degree of compliance with the standard was related to the treating physician's specialty or postgraduate year of training. The standard specified items to be included in the medical record and appropriate diagnostic tests. A medical record score was defined by the presence of seven essential and seven desirable items obtained from the history and the physical examination. Only anti

Admissions to a rural hospital in the West Nile District of Uganda over a 27 year period.

An analysis is presented of data on all 30 129 inpatient admissions to a mission hospital in the West Nile District of Uganda in the 27 year period from July 1951 to August 1978. For most of this period the hospital was staffed by the same two doctors. For each patient admitted, a record was made of their age (adult or child), sex, place of residence, duration of stay in hospital, diagnosis and vital status at discharge. The annual number of admissions increased steadily from around 300 in 1952 to over 1600 in 1966 and subsequently declined to about 900 in 1977. Sixty-five per cent of admissio

Impact of Secure Electronic Health Record Chat on Physician Communication in the Emergency Department.

INTRODUCTION: Effective communication in the emergency department (ED) is essential for safe patient care. Electronic health record (EHR)-integrated secure messaging may improve communication workflows, but limited evidence exists comparing clinician satisfaction across modalities in the ED. This study evaluated physician satisfaction with an EHR-integrated secure messaging tool compared to existing communication methods for non-emergent communication. METHODS: We conducted an electronic pre-post survey study at an urban academic ED following implementation of an EHR-integrated secure messagi

Clinical agents fail silently on patient identity.

BACKGROUND: LLMs now power clinical agents that plan, call tools, and write into EHRs in routine clinical workflows today. Before clinical deployment, we must know whether agents detect patient identity faults or write through them. METHODS: We built a record environment from publicly available MIMIC-IV emergency department data. Agents copied ICD-10-CM codes from visit headers into patient records using Extract and Store tools, with "UNKNOWN" allowed when uncertain. Missing Store calls were counted as non-writes. Each run presented ten visits from one patient (clean), then one visit was tamp

Evaluating the Performance of Large Language Models for Generating Emergency Department Discharge Instructions.

BACKGROUND: Clear discharge instructions are essential for safe emergency department (ED) care, yet creating patient-centered, health literate materials remain challenging. Large language models (LLMs) may improve patient communication, but their role in the ED remains underexplored. OBJECTIVE: The objective of this study was to evaluate the feasibility of using LLMs to generate ED discharge instructions and to compare readability across models. METHODS: We generated discharge instructions for 20 ED diagnoses across Emergency Severity Index (ESI) levels using GPT-4, GPT-4o, GPT-5.2, Gemini 2

A taxonomy of key performance errors associated with hyperangulated video laryngoscopy for emergency intubation.

BACKGROUND: Video laryngoscopy (VL) is now ubiquitous in emergency airway management. Hyperangulated video laryngoscopy (HAVL) differs from standard geometry VL (SGVL) in blade design, technique, and required microskills, yet the two are often regarded similarly. Despite widespread use, no comprehensive taxonomy of HAVL-specific performance errors exists. METHODS: We conducted an observational study of video-recorded endotracheal intubations (ETIs) performed in an urban academic emergency department (2020-2024). Fifty HAVL videos were first analyzed to derive 20 performance errors, followed b

Improving hand hygiene in hospitals: A comparative study using body-worn cameras and direct observation.

BACKGROUND: Hand hygiene (HH) prevents infections, but traditional monitoring is limited by office hours and the Hawthorne effect. We used body-worn cameras in ICU to compare video with direct observation. METHODS: After ethics approval, staff wore a GoPro™ on the upper abdomen during patient care. A trained observer simultaneously documented opportunities and performance. A blinded researcher analyzed the video. Both methods were compared on opportunities, compliance, performance, and duration. RESULTS: Seventeen paired video and observer data sets captured 166 HH opportunities and 147 even
