Preoperative analysis of the Hill-Sachs lesion in anterior shoulder instability: how to predict engagement of the lesion.

BACKGROUND: It has been reported that engagement of the Hill-Sachs lesion affects postoperative recurrence of anterior shoulder instability. … lesions, the mean width was 52% (range, 27%-66%) and the mean depth was 14% (range, 8%-20%) of the humeral head diameter on axial images. The corresponding measurements on coronal images were 42% (range, 16%-67%) and 13% (range, 5%-24%), respectively. In cases of nonengaging lesions, the corresponding measurements were 40% (range, 0%-71%) and 10% (range, 0%-2 …

Outcomes after operative treatment of elbow contractures in the pediatric and adolescent population.

BACKGROUND: The role of elbow contracture release in the very young is unclear, with existing studies reporting conflicting results. This study evaluated the long-term results after open elbow contracture release in patients aged younger than 18 years. METHODS: Between 1994 and 2012, 32 patients underwent open elbow contracture release at a mean age of 13.8 years (range, 5-18 years), and their outcomes were reviewed. The primary cause was traumatic in 30 patients (4 radial head/neck fractures, 5 intra-articular distal humeral fractures, 11 extra-articular distal humeral fractures, 10 complex

Unusual occurrence of avascular necrosis with bilateral involvement and ankylosing spondylitis, meningioma and Hodgkin lymphoma.

This case report aimed to reveal the multidisciplinary aspect and discuss an association of disorders in a complex case with Hodgkin disease, meningioma, avascular necrosis with bilateral involvement and ankylosing spondylitis. We report the case of a 47-year-old patient, diagnosed with Hodgkin lymphoma at the age of 14. Two decades later, she was diagnosed with frontal chordoid meningioma, which was surgically removed, without radiotherapy. She was re-operated 10 years later for recurrence of meningioma. At the age of 39, she was diagnosed with bilateral avascular necrosis of the humeral head…

One-stage surgical treatment of neglected simultaneous bilateral locked posterior dislocation of shoulder: a case report and literature review.

Simultaneous bilateral locked posterior dislocation of the shoulder is a rare injury. Herein, we present a 59-year-old male case with a three-month history of an irreducible locked bilateral posterior dislocation of the shoulders with an associated large impression fracture on the anteromedial aspect of both humeral heads after a grand mal type epileptic seizure. Plain X-ray and computed tomograph revealed a defect on the right side more than 40% of the articular surface, and on the left side, 30%. He was treated with a one-stage operation with a reconstruction of femoral head osteochondral al…

Biceps tendon disorders in athletes.

It has been proposed that the long head of the biceps functions as a humeral head depressor and stabilizer. In addition, in many overhead sports, the biceps helps to accelerate and decelerate the arm. With improper training or fatigue, inordinate stresses can be placed on the biceps as it attempts to compensate for other muscles. This can lead to attrition and failure, either within the tendon substance or at its origin. Bicipital problems in athletes usually occur in conjunction with other types of shoulder disorders, such as rotator cuff impingement and glenohumeral instability, making deter…

Posterior superior impingement of the rotator cuff on the glenoid rim as a cause of shoulder pain in the overhead athlete.

A case of impingement of the deep surface of the supraspinatus tendon on the posterior superior rim of the glenoid and mild anterior laxity has been presented. The partial-thickness tear of the undersurface rotator cuff tendon, degenerative tear of the posterior superior labrum, and osteochondral impression fracture of the humeral head have been documented. The purpose of this case report is to present the pathological findings associated with posterior superior glenoid rim impingement and emphasize its role as a cause of shoulder pain in the overhead athlete. In addition it is important to st…

Revitalizing Recovery: Unveiling the Transformative Impact of Physiotherapy in Rehabilitating a Neglected Case of Shoulder Hemiarthroplasty.

The shoulder joint has a complex anatomy and biomechanics. It is a ball and socket joint made by the articulation surface of the humeral head (ball) and glenoidal fossa (socket) of the scapula. Shoulder arthroplasty is done when parts of the shoulder joint are severely affected and damaged beyond repair. The damaged parts are replaced with artificial parts. Prosthetic implants are typically made of metal or plastic material. Implants come in various sizes and shapes. There are three types of surgical arthroplasty: total shoulder arthroplasty, partial shoulder arthroplasty, and reverse arthropl…

Simultaneous Bilateral Proximal Humerus Fractures Treated with Single-Stage Bilateral Reverse Shoulder Arthroplasty.

INTRODUCTION: Simultaneous bilateral fractures of the proximal humerus are infrequent, and simultaneous bilateral three- or four-part fractu … ure, according to Neer's classification. The right shoulder had a risk of avascular necrosis of the humeral head. For the left shoulder, the fracture type had caused ischemia of the humeral head. Single-stage bilateral RSA was performed 9 days after the injury. An abduction pillow was applied for 5 weeks postoperatively. Passive motion exercises were permitted …

Treatment of acute shoulder infection: can osseous lesion be a rudder in guideline for determining the method of débridement?

BACKGROUND: There is no standard to determine the most appropriate method of operation for the treatment of acute septic arthritis of the shoulder joint. METHODS: We retrospectively reviewed 57 patients who underwent arthroscopic or open débridement for acute shoulder infection between 2001 and 2015. Arthroscopic débridement was performed in 27 patients, and open débridement in 30 patients. According to the presence of bone erosion and/or marginal erosion of cartilage of the humeral head on plain radiographs and magnetic resonance imaging (MRI) images, the cases were classified into 3 groups …

Anterior shoulder dislocations in sports.

Anterior shoulder dislocations, primary and recurrent, are among the most disabling injuries to the shoulder that can plague the athlete. The diagnosis is easily made by the following: the physical appearance of the shoulder; loss of capability by the athlete to internally and externally rotate the shoulder with the elbow at his side; by evaluating the mechanism of injury; and x-rays. Anterior shoulder dislocations should be reduced as soon as possible after diagnosis, to minimise the stretching effect on the neurovascular structures while the humeral head is dislocated. The reduction is not d…

Arthroscopic Anterior and Posterior Glenohumeral Capsular Augmentation With Gracilis Allograft.

Multidirectional instability in patients with connective tissue disorders such as Ehlers-Danlos syndrome (EDS) is difficult to treat due to poor tissue quality and frequent failure of standard repairs. We describe an arthroscopic technique using gracilis tendon allografts to perform an anterior and posterior capsular augmentation in the revision setting with subscapularis deficiency. Allografts are anchored to the glenoid and humeral head in a sling configuration, restoring capsular restraint and stability. This method builds upon prior posterior-only augmentation by providing circumferential …

Editorial Commentary: The Trillat Procedure May Be (Rarely) Indicated Instead of Shoulder Latarjet for Recurrent Instability, Irreparable Cuff Tear, No Pain, Preserved Active Motion, and No Critical Glenoid Bone Loss.

The Latarjet shoulder bone block procedure for recurrent instability has largely replaced the Trillat procedure. Both procedures stabilize the shoulder by a dynamic "sling effect." Latarjet increases the anterior glenoid width or "jumping distance," whereas the Trillat prevents the humeral head anterosuperior migration. The Latarjet violates the subscapularis (albeit to a minimal degree), whereas the Trillat procedure only lowers the subscapularis. One clear indication for the Trillat procedure is recurrent shoulder dislocation associated with functioning irreparable cuff tear in patients with…

MRI appearance of multiple displaced osteochondritis dissecans cartilage fragments in the bicipital tendon sheath of a juvenile dog.

A 1-year-old Bernese Mountain dog presented with an acute onset of left thoracic limb lameness. Magnetic resonance imaging (MRI) of the left shoulder was performed, showing a subchondral bone defect in the caudomedial aspect of the humeral head. In addition, several round hypointense structures were visible in the biceps tendon sheath. A left shoulder arthroscopy was performed, which confirmed an osteochondritic lesion. Exploration of the biceps tendon sheath via a small open approach allowed retrieval of the fragments, which likely migrated from the joint. Histopathology confirmed the structu…

[Treatment of subscapularis tendon lesions].

The further development of minimally invasive techniques in recent years, enables adequate intraoperative visualization, mobilization and stable reconstruction even of larger subscapularis tears resulting in good to excellent clinical and structural results. Chronic tears of the subscapularis tendon with high grade muscle atrophy and advanced fatty infiltration can be treated with a muscle tendon transfer (e.g. pectoralis major or latissimus dorsi transfer). If pseudoparalysis and/or signs of anterosuperior decentration of the humeral head or defect arthropathy are present, in most cases a rev…
