Targeting the B-Cell Activating Factor and a Proliferation-Inducing Ligand Pathway in IgAN: From Disease Pathogenesis to Clinical Trial Evidence.

Immunoglobulin A nephropathy (IgAN) is an immune-mediated glomerular disease driven by dysregulated mucosal B-cell immunity, leading to the overproduction of galactose-deficient IgA1 (Gd-IgA1) and the formation of immune complexes with anti-Gd-IgA1 IgG and IgA immunoglobulins. These immune complexes deposit in the renal mesangium, triggering mesangial activation, proliferation, and downstream inflammatory injury. Central to this process are members of the tumor necrosis factor (TNF) superfamily, a proliferation-inducing ligand (APRIL) and B-cell activating factor (BAFF). These ligands are cruc

Prospective Pilot Study of Plasma Cell-Targeted Therapy With Bortezomib in Refractory IgA Nephropathy.

INTRODUCTION: IgA nephropathy (IgAN) is a leading cause of kidney failure, characterized by galactose-deficient IgA1 (Gd-IgA1) deposition and immune complex formation. Aberrant trafficking of IgA+ plasma cells and autoantibody production (IgG or IgA) contribute to disease pathogenesis. Proteasome inhibitors such as bortezomib may modulate B or plasma cell activity and reduce pathogenic antibody production. METHODS: This open-label, prospective, uncontrolled trial evaluated bortezomib in adults with biopsy-confirmed IgAN, proteinuria > 1.5 g/d, and estimated glomerular filtration rate (eGFR) ≥

NKT cell-dependent PspA-specific IgA production caused by mucosal administration of a novel nanoparticle pneumococcal vaccine.

Pneumonia is one of the leading causes of death worldwide, with Streptococcus pneumoniae being a major causative agent of community-acquired pneumonia. In immunocompromised infants and the elderly, this pathogen can invade the bloodstream and cerebrospinal fluid, leading to invasive pneumococcal diseases (IPDs), such as bacteremia and meningitis. Currently, vaccines targeting the capsular polysaccharides of pneumococcus are available to prevent pneumococcal infection. However, these vaccines have limitations, including incomplete coverage of all serotypes. Therefore, there is a need for next-g

Intranasal inoculation of an MVA-based vaccine induces IgA and protects the respiratory tract of hACE2 mice from SARS-CoV-2 infection.

Current vaccines have greatly diminished the severity of the COVID-19 pandemic, even though they do not entirely prevent infection and transmission, likely due to insufficient immunity in the upper respiratory tract. Here, we compare intramuscular and intranasal administration of a live, replication-deficient modified vaccinia virus Ankara (MVA)-based Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) spike (S) vaccine to raise protective immune responses in the K18-hACE2 mouse model. Using a recombinant MVA expressing firefly luciferase for tracking, live imaging revealed luminescen

Serum levels of IgM, IgD, IgA and IgG before and during rejection of renal allotransplants.

In recipients of renal allotransplants the relationship of IgM, IgA and IgG serum levels to 26 rejections was investigated, as well as the relationship of IgD and 22 rejection. For IgM and IgG a significant decline was found in the whole group (p less than 0.05) and in the sub-group of rejections after a period longer than one month following transplantation (p less than 0.05 in IgM and p less than 0.02 in IgD). As compared with values before rejection, a decline of IgM occurred in 11 rejections which amounted to more than 20% and a decline of IgD in 10 rejections by more than 50%. Changes of

Proton Pump Inhibitor (PPI)-Domperidone Induced Anemia and Galactorrhea: A Case Report.

Gastrointestinal disorders are increasingly common among young adults, particularly medical students exposed to chronic stress and irregular dietary habits. Proton pump inhibitors (PPIs) are widely prescribed for the management of gastroesophageal reflux disease (GERD), gastritis, and related acid-peptic disorders. However, prolonged PPI therapy has been associated with impaired iron absorption and may contribute to iron deficiency anemia (IDA). We report the case of 26-year-old female, unmarried, postgraduate medical student with iron deficiency anaemia and galactorrhea. The patient presented

A novel frameshift variant in the ADA2 gene of a patient with a neurological phenotype: a case report.

BACKGROUND: Adenosine deaminase 2 (ADA2) deficiency is an inherited autoinflammatory syndrome caused by a defect in the ADA2 gene. Most common manifestations include peripheral vasculopathy, early-onset stroke, immunodeficiency, and haematological manifestations. Patients with pathogenic variants that are more detrimental to ADA2's enzymatic function (e.g. frameshift) have been reported to be prone to developing hematological phenotype. We report here the case of a 13-year-old Caucasian girl with a novel frameshift variant in the ADA2 gene and a clinical phenotype of early-onset stroke. CASE

DOCK8 mutation diagnosed using whole-exome sequencing of the dried blood spot-derived DNA: a case report of an Iraqi girl diagnosed in Japan.

BACKGROUND: Dedicator of cytokinesis 8 (DOCK8) deficiency (MIM #243700) is a rare disease, leads to a combined primary immunodeficiency (PID), and accounts for the autosomal recessive-hyper immunoglobulin E syndrome (AR-HIES). DOCK8 deficiency status characterizes by recurrent infections, atopy, and risk of cancer. Lymphoproliferative disease complicating PID, is difficult to diagnose. Our aim is to present a rare case of PID, and to the best of our knowledge, she is the first case of DOCK8 deficiency from Iraq. The genetic diagnosis was carried out in Japan using dried blood spot-based DNA tr

IMPACT OF VITAMIN D METABOLISM DISORDERS ON THE DEVELOPMENT OF AUTOIMMUNE KIDNEY DISEASES: A SYSTEMATIC REVIEW.

INTRODUCTION: Vitamin D plays a crucial role in immune regulation and renal physiology. It acts as a modulator of both the innate and adaptive immune systems. Vitamin D enhances the differentiation of monocytes into macrophages, thereby strengthening their phagocytic and chemotactic functions. Its deficiency has been associated with the pathogenesis of autoimmune renal diseases; however, the clinical and molecular mechanisms underlying this association remain insufficiently understood. AIM: This systematic review focused on systemic lupus erythematosus with lupus nephritis (LN), IgA nephropat

A Tale of Two Immunodeficiencies: A Case of Multiple Myeloma Associated with Profound Immune Defect Mimicking Common Variable Immunodeficiency Syndrome.

INTRODUCTION: Multiple myeloma (MM) is a clonal plasma cell disorder commonly associated with secondary immune deficiency. By contrast, common variable immunodeficiency (CVID) is a primary immunodeficiency characterized by low serum levels of immunoglobulins (IgG, IgA, and/or IgM) and inability to produce specific protective antibodies in response to infections and immunizations. Besides a defective immune system and susceptibility to infections, CVID is associated with autoimmune disorders, gastrointestinal tract inflammation, granulomatous disease, and malignancies. Although MM and CVID both

Phenotypic and clinical heterogeneity associated with monoallelic TNFRSF13B-A181E mutations in common variable immunodeficiency.

Mutations in the TNFRSF13B (TACI) gene have been reported to be associated with Common Variable ImmunoDeficiency (CVID). Of 48 patients evaluated within the immunodeficiency clinic, 39 had CVID, 6 had symptomatic IgA deficiency (IgAD) with or without IgG2 and IgG4 subclass deficiency, while 3 had unclassified immune dysregulatory disorders. In all 48 patients TACI genetic testing was performed, and monoallelic mutations were observed in 4 of the 39 CVID patients (10.26%), an incidence comparable to other studies. Of the 6 IgAD patients, 1 had a heterozygous TACI mutation (16.67%), while of the…

Novel mutations in a Japanese patient with CD19 deficiency.

Common variable immunodeficiency (CVID) is a heterogeneous disorder characterized by recurrent bacterial infections, hypogammaglobulinemia and low to normal numbers of circulating B cells. Mutations in the ICOS, TACI and CD19 genes have recently been identified in <10% of CVID patients. We, herein, describe two novel CD19 gene disruptions in an 8-year-old Japanese boy, who had been clinically diagnosed as having CVID at the age of 5 years. Flow-cytometric analysis demonstrated absence of CD19 and reduced CD21 expression on CD20-postive peripheral blood B cells. Mutation analysis of CD19 reveal
