Neurotoxic effects of combined exposures to aluminum and mercury in early life (infancy).

Aluminum and mercury are environmentally ubiquitous. Individually they are both neurotoxic elements with shared neuro-pathogenic pathways: oxidative stress, altered neurotransmission, and disruption of the neuroendocrine and immune systems. In the infant, Al and Hg differ in type of exposure, absorption, distribution (brain access), and metabolism. In environmentally associated exposure (breast milk and infant formulas) their co-occurrences fluctuate randomly, but in Thimerosal-containing vaccines (TCVs) they occur combined in a proprietary ratio; in these cases, low-doses of Thimerosal-ethylm…

[Feeding infants and young children with acute diarrhea].

Acute gastroenteritis remains a common and often severe illness among infants and children throughout the world. The management of a child with acute diarrhea includes rehydration and maintenance fluids with oral rehydration solutions (ORS), combined with continued age-appropriate nutrition. However, although substantial data support the role of continued nutrition in improving gastrointestinal function and anthropometric, biochemical, and clinical outcomes, the practice of continued feeding during diarrheal episodes has been difficult to establish as accepted standard of care. Recommendations

Renal function in sick very low birthweight infants: 2. Urea and creatinine excretion.

Plasma urea and creatinine concentrations and urea and creatinine clearances and excretion were measured in a sample of 40 infants of 25.5-33 weeks' gestation, birth weight 720-2000 g, between the ages of 0.5 and 33 days. Creatinine excretion rate was between 60 and 120 mumol/kg/day in the first five postnatal weeks (mean 90.5) and was independent of sex or growth retardation. This can be used in clinical practice to estimate instantaneous urine flow rate V, if the creatinine concentration is measured in a randomly voided urine sample, from the formula V = 90.5/urine creatinine, with 95% confi

Is it safe to breastfeed while taking methylphenidate?

QUESTION: My patient has narcolepsy and is currently breastfeeding her 3-month-old infant. Lately she has had difficulties adjusting to caring for her baby, especially staying alert with the demands of breastfeeding. If she starts taking methylphenidate again, should I advise her to switch to formula? ANSWER: Methylphenidate is excreted in breast milk only in small amounts, and to date there have been no reports of breastfed infants demonstrating any adverse effects. Based on the available data, methylphenidate appears to be compatible with breastfeeding; however, the long-term neurodevelopme

Nasopharyngeal reflux and neonatal apnea.

Although oropharyngeal dysfunction has been reported as a cause of dysphagia in newborns, to our knowledge oropharyngeal dysfunction and neonatal apnea have not been associated previously. We studied two infants in whom apnea and bradycardia developed primarily while feeding. The diagnosis was established by observation of the swallowing mechanism using cinefluoroscopy while the infant ingested a dilute barium meal. Thickened formula feedings and use of a special nipple resulted in complete cessation of apnea and bradycardia. Careful attention to the details that surround episodes of apnea is

The role of nutrition in promoting growth in pre-term infants with bronchopulmonary dysplasia: a prospective non-randomised interventional cohort study.

BACKGROUND: Pre-term infants who develop bronchopulmonary dysplasia (BPD) are at risk of postnatal growth failure. It has been reported that energy expenditure is higher in infants with BPD than in those without BPD. The aim of the study was to evaluate whether increasing the enteral energy intake of pre-term infants with BPD can improve post-natal growth. METHODS: This prospective, non-randomised interventional cohort study was designed to assess growth in 57 preterm infants with BPD (gestational age <32 weeks, birth weight <1500 g, and persistent oxygen dependency for up to 28 days of life)

Premature infants fed mothers' milk to 6 months corrected age demonstrate adequate growth and zinc status in the first year.

The objective of this investigation was to describe zinc status to 12 months corrected age in premature infants fed their mother's milk in relation to nutritional management in hospital and post-hospital discharge. Twenty-five premature infants fed their mother's milk in hospital were randomized to receive either a multi-nutrient fortifier (MNF), providing protein, calcium, phosphorus and zinc (MM + MNF) or calcium and phosphorus alone (MM + CaGP). Twelve preterm infants fed a preterm formula (PTF) served as a comparison group. At 35 weeks post-menstrual age zinc retention was determined using

Copper nutrition during infancy and childhood.

Full-term human infants are believed to possess adequate copper stores to last through weaning regardless of the copper content of the diet they are fed. This may not be generally true, however; a combination of low copper intake and low bioavailability from the diet may lead to copper deficiency. More information is needed on the bioavailability of copper from different infant diets, but it appears that copper is well absorbed from breast milk compared with infant formula. Several dietary factors that may affect copper absorption in infants, such as protein sources, amino acids, phytate, asco…

Breast milk beta-glucuronidase in breast milk jaundice.

Breast milk jaundice (BMJ) is the most common etiology of prolonged unconjugated hyperbilirubinemia in a newborn infant. The pathogenesis of BMJ has been studied by many investigators, yet the etiology still remains uncertain. A prospective study was done to examine the role of breast milk beta-glucuronidase in BMJ in healthy full-term Chinese newborns. There were 71 infants in the formula-fed group (FF group), 28 in the breast-fed group (BF group) and 27 in the mixed feeding group (MF group). Study infants were followed at the well-baby clinic for at least two months. Enzyme activity in mater

Infant feeding decisions among people living with human immunodeficiency virus in the United States: a pilot study.

INTRODUCTION: The benefits of breastfeeding for the lactating person and infant have been well established; however, for those living with HIV, the option to provide human milk has been contraindicated. In January 2023, the US national guidance was revised so breastfeeding can be supported through a shared decision-making process for virally suppressed individuals living with HIV. Few research studies have explored maternal experiences related to infant feeding decisions among people living with HIV (PWH) in the US. METHODS: A qualitative, modified critical ethnographic method was used to con

[Infant botulism in France, 1991-2009].

Infant botulism is caused by the ingestion of spores of Clostridium botulinum and affects newborns and infants under 12 months of age. Ingested spores multiply and produce botulinum toxin in the digestive tract, which then induces clinical symptoms. A single French case was described in the literature prior to 1991. We describe the cases of infant botulism identified in France between 1991 and 2009. All clinical suspicions of botulism must be declared in France. Biological confirmation of the disease is provided by the National reference laboratory for anaerobic bacteria and botulism at the Pa

The protective role of enteral IgA supplementation in neonatal gut origin sepsis.

Preterm infants and infants unable to breast feed are particularly susceptible to gut origin sepsis. Many studies have shown the benefits of breast milk in decreasing the incidence of bacterial infections in neonates. Little in vivo work has focused on prevention of neonatal gut origin sepsis with breast milk components. The aim of this study was to determine whether supplementation of a standard neonatal formula with exogenous, luminally administered, human secretory IgA protects against gut origin sepsis in a newborn rabbit model. Sixty New Zealand white rabbit pups were delivered by cesarea

Feed thickener for infants up to six months of age with gastro-oesophageal reflux.

BACKGROUND: Gastro-oesophageal reflux (GOR) is common in infants, and feed thickeners are often used to manage it in infants as they are simple to use and perceived to be harmless. However, conflicting evidence exists to support the use of feed thickeners. OBJECTIVES: To evaluate the use of feed thickeners in infants up to six months of age with GOR in terms of reduction in a) signs and symptoms of GOR, b) reflux episodes on pH probe monitoring or intraluminal impedance or a combination of both, or c) histological evidence of oesophagitis. SEARCH METHODS: We used the standard search strategy

Lactose Intolerance versus Cow's Milk Allergy in Infants: A Clinical Dilemma.

Due to its very early introduction, cow's milk is one of the first foods that can cause adverse reactions in human beings. Lactose intolerance (LI) and cow's milk allergy (CMA) are the most common adverse reactions to cow's milk. While LI is due to insufficient small intestinal lactase activity and/or a large quantity of ingested lactose, CMA is an aberrant immune reaction to cow's milk proteins, particularly casein or β-lactoglobulin. However, the clinical manifestations of LI and CMA, particularly their gastrointestinal signs and symptoms, are very similar, which might lead to misdiagnosis o

Development of a high-throughput method for the comprehensive lipid analysis in milk using ultra-high performance supercritical fluid chromatography combined with quadrupole time-of-flight mass spectrometry.

Milk lipids are one of the most complex materials in nature and are associated with many physiological functions, hence it is important to comprehensively characterize lipids profiles to evaluate the nutritional value of milk. A quick method was developed by ultra-high performance supercritical fluid chromatography coupled to electrospray ionization quadrupole time-of-flight mass spectrometry (UHPSFC-ESI-QTOF-MS) to analyze the non-polar and polar lipids profiles of cow, goat, buffalo, human milk, and infant formulas in 7 min. All chromatographic conditions were carefully optimized and their e…

Evaluating the reflux suppression properties of Gaviscon Infant powder with different milk formulations using an in vitro model of the infant stomach.

Gastroesophageal reflux (GER) is common in babies and infants and is characterized by effortless regurgitation (with or without vomiting), which is a common symptom of other infant conditions, including cow's milk protein allergy (CMPA). Various milk formulations are commercially available to help manage GER and CMPA symptoms (extensively hydrolyzed [eHF], partially hydrolyzed [pHF], and amino-acid formulas [AAF]). We aimed to understand if an alginate product (Gaviscon Infant powder) worked as intended in the presence of different milk formulations. A previously evaluated artificial infant st
