Intestinal polypoid arteriovenous malformation: unusual presentation in a child and review of the literature.

AIM: We report a jejuno-jejunal intussusception with a polypoid arteriovenous malformation as a lead point in a 12-year-old boy, presenting with lower abdominal pain and non-bloody non-projectile vomiting. METHODS: A computed tomography scan of the abdomen and pelvis showed proximal jejuno-jejunal intussusception in the right upper quadrant. Exploratory laparotomy revealed a 5.5 × 2.5 × 2 cm polypoid mass within the wall of the jejunum. Consequently, jejunal segment resection was performed with end to end jejunostomy. Our case is distinctive because it involves a rare vascular lesion at an at

Inflammatory fibroid polyp or Vanek's tumour.

We report a case of a 65-year-old woman who presented with recurrent episodes of severe, postprandial abdominal pain followed by projectile vomiting. Gastroscopy revealed a large polyp in the prepyloric region. During peristalsis, the polyp was repeatedly 'passing' through the pylorus into the duodenal bulb, hence obstructing the lumen. The polyp was eventually removed in a piecemeal fashion. Histopathologic examination revealed an inflammatory fibroid polyp (known also as Vanek's tumour). A brief review on inflammatory fibroid polyps follows.

[The Peutz-Jeghers syndrome--a case review].

INTRODUCTION: The authors present a case review of a localized Peutz-Jeghers syndrome of the caecum, emulating a carcinoma. METHODS: The patient who presented with insignificant family history with a diagnosed stenosing tumor of his appendix and histological findings of highly suspected adenocarcinoma, was indicated for surgical revision and right-sided hemicolectomy. RESULTS: The patient underwent laparoscopically assissted right-sided hemicolectomy and his postoperative course was adequate. The final histological finding diagnosed solitary hamartoma of the Peutz-Jeghers polyp. CONCLUSION:

Non-familial juvenile polyposis with histological evidence of adenomatous transformation.

A 14-year-old male presented with abdominal pain, diarrhoea and a sensation of something prolapsing through the anus during defecation, and was found to have diffuse colonic polyposis. There was no evidence of mucocutaneous hyperpigmentation and family history was negative, suggesting a diagnosis of non-familial juvenile polyposis. Histological analysis of multiple endoscopic biopsies showed features typical of juvenile or retention type (hamartomatous) lesions: dilated cystic glands lined by mucocus-secreting epithelium and prominent, inflamed and congested lamina propria. However, admixed wi

A case of Cronkhite-Canada syndrome whose major complaint, taste disturbance, was improved by zinc therapy.

A patient whose major complaint was taste disturbance and who was diagnosed as having Cronkhite-Canada syndrome was prescribed zinc sulfate. Improvement in taste disturbance was noted after 3 weeks of treatment, followed by gradual improvement in skin and gastrointestinal symptoms. Cronkhite-Canada syndrome can be considered a zinc-deficiency disorder caused by gastrointestinal polyposis.

Anorectal malformation: a new anatomic variant resembling an H-type fistula.

A new anatomic variant of an anorectal malformation resembling an H-type fistula was observed in a newborn girl. This variant consisted of a high-grade stenosis of the anal canal below the level of the levator muscle with a blind-ending fistulous tract originating from the stenotic segment and running toward the posterior wall of the vestibulum. The embryogenesis of this malformation remains to be seen.

Inflammatory cloacogenic polyps in children.

Inflammatory cloacogenic polyp (ICP) is a rare lesion arising in the region of the anorectal transitional zone. It is likely caused by occult internal prolapse. Most cases reported in the literature are in the adult population. This is a report of ICP in four children. Awareness of this entity in children is important because of both the propensity for recurrence or persistence of the polyps if the underlying etiology is not corrected, as well as the long-term implications of internal intussusception: procidentia, descending perineum syndrome, and ultimately, incontinence.

Leiomyomatous colonic polyp.

Leiomyomatous colonic polyps are rare. We report a patient with an asymptomatic 15-mm submucosal polyp in the sigmoid colon. The tumor was successfully removed by endoscopy. Histopathological examination revealed a leiomyoma originating in the muscularis mucosa.

Triple carcinomas in Cronkhite-Canada syndrome.

The present report describes a 69-year-old man displaying the clinical features of the Cronkhite-Canada syndrome. After taking medicine for the common cold, he suffered hypogeusia and watery diarrhea, eruptions on the lower extremities and an 8 kg loss in body weight. All his finger and toenails began to fall out. He underwent an upper gastrointestinal examination, upon which multiple polyps of the stomach were detected. Three years later, he again developed diarrhea, bloody stools, body weight loss and eruptions on the lower extremities. An upper gastrointestinal series showed a diverticulum

[Solitary rectal polyp and endoscopic polypectomy in a pediatric population: 3 years' experience].

During defecation, rectal polyps represent one most frequent causes of bleeding in infancy. The advent of endoscopic polypectomy has resulted very useful in their treatment. From January 1987 to March 1990, 113 children with rectal bleeding were evaluated; 22 of them had solitary rectal polyp, between 5 to 15 cm from anal margin at rectoscopy. 6 patients were excluded because they had autoamputation of the polyp, 16 patients, 9 girls and 6 boys between 2 and 11 years of age were studied with blood tests, series stools, contrast study with barium, colonoscopy with polypectomy and polyp histolog

Colonoscopic polypectomy in children.

Three children, 30 to 36 months of age, had solitary juvenile polyps in the sigmoid colon and rectal bleeding of varying severity, In all three patients, the polyps were removed without the use of general anesthesia through a standard 100-cm colonoscope used to examine adult patients. No untoward effects were encountered. Colonoscopic polypectomy is an effective therapeutic procedure and obviates the need for either anxious waiting in the mildly symptomatic patient or colotomy and polypectomy in the child with profuse rectal bleeding.
