A case of intrahepatic cholangiocarcinoma with intrabile duct tumor growth is presented. A 70-year-old female with prolonged obstructive jaundice and cholangitis was diagnosed with a mass lesion in the left lobe of the liver accompanied by an intrabile duct component which caused hilar bile duct obstruction. Percutaneous transhepatic biliary drainage (PTBD) was performed via three routes with intensive care of the drainage catheters over a 2-month period to alleviate jaundice and cholangitis. Extended left hepatic lobectomy with combined resection of extrahepatic bile duct and regional lymph n

Authors collected three cases of neuromas situated in the wall of extrahepatic bile ducts, which occluded lumen and caused jaundice. In two cases without any evidence of previous surgery or cholelithiasis, neuroma was localized in ductus choledochus. After excision of ductus choledochus and cholecystectomy, we performed hepaticojejunostomy Roux--en Y. In the last patient, 7 years after cholecystectomy, neuroma localized in hepatic duct was treated by T-tube. In all patients with mechanical jaundice, before operation we had never correctly diagnosed this very rare cause of obstruction of extrah

Three infants with idiopathic perforation of the extrahepatic bile ducts were treated at this hospital during the last 6 years. Two patients presented with jaundice and biliary ascites, with high bilirubin in the ascitic fluid, and one patient with peritonitis. The site of perforation was repaired and abdomen drained in one patient. Simple drainage was performed in two others, with good overall results. Perforation of the bile ducts should be suspected in every neonate and infant with ascites and jaundice. Confirmation is obtained by paracentesis and bilirubin measurement in the ascitic fluid

Platelet-activating factor acetylhydrolase (PAF-AH) is an enzyme that catalyzes hydrolysis of PAF and is important for regulation of serum PAF concentration. Twenty-seven patients with hyperbililubinemic hepatobiliary diseases (eight patients with liver cirrhosis, five with acute hepatitis and 14 with obstructive cholestasis) were studied. Serum PAF-AH activity was elevated in hyperbilirubinemic condition associated with liver cirrhosis, obstructive cholestasis or acute hepatitis. There was a significant correlation of serum PAF-AH activity with serum cholesterol (r=0.891, P=0.014 in liver cir

Hemodynamic response to laryngoscopy and intubation is usually transient, but it may be more pronounced and unpredictable in certain group of patients. Bradycardia and asystole during laryngoscopy is usually a rare manifestation compared to hypertension and tachycardia. Anesthesiologists should be more vigilant and take special precautions to avoid such life-threatening complications during laryngoscopy. Here, we report recurrent asystole on multiple occasions during laryngoscopy in a patient with obstructive jaundice.

93 patients with chronic pancreatitis were treated endoscopically: 44 had transpapillar procedures and 49 patients had transmural operations. The transmural procedures were applied in case of pancreatic pseudicysts, whereas transpapillar endoscopic operations were applied in cases of the obstructive jaundice, pseudocyst, connected of the main pancreatic duct, virsungolithyasis or ductal pancreatitis with pain syndrome. The endoscopic procedures by chronic pancreatitis proved to be the reliable alternative to the traditional surgery.

Majority of malignant neoplasms arising from the extrahepatic bile duct are adenocarcinomas. Carcinoid tumors at this site are extremely rare. We report a 67-year-old woman with malignant carcinoid tumor of the common bile duct. She presented with obstructive jaundice of 1 week's duration. Abdominal CT and ERCP revealed a common bile duct mass. She underwent Whipple's operation and was diagnosed as malignant carcinoid tumor histologically and immunohistochemically.

Primary and isolated lymphoma of the head of pancreas with secondary involvement of the lungs and obstructive jaundice, in a 31-year-old postpartum woman, is described. The histological diagnosis was diffuse histiocytic lymphoma. Chemotherapeutic treatment resulted in complete remission of the tumor. During therapy a pseudocyst of the pancreas developed and a gastrocystostomy was performed. This is the second reported case of primary pancreatic lymphoma in the English medical literature.

A new syndrome is described of which the salient features are (1) congenital hypoplasia and stenoses of the pulmonary arteries, sometimes with associated cardiovascular malformations; (2) neonatal liver disease, commonly with obstructive jaundice and resembling biliary atresia or neonatal hepatitis, but sometimes apparent only as mild persistent hepatic dysfunction; and (3) various minor congenital anomalies, including an odd facies. There is a familial tendency suggesting autosomal dominant inheritance, with variable penetrance.

A neonate with a large chorioangioma, multiple skin angiomata and prolonged cholestasis is described. Hepatic angiomata are thought to be responsible for the cholestasis but in some reported cases the jaundice could have been due to extrahepatic biliary obstruction caused by angiomata.

Obstruction of the extrahepatic bile ducts by primary or metastatic carcinoma can sometimes be diffiult to relieve. Frequently there is an inadequate length of normal duct proximal to the obstruction to permit an anastomosis. A by-pass anastomosis between a dilated right hepatic biliary radicle and the gallbladder is a simple and satisfactory method of relieving the jaundice. This can be accomplished by using a T-tube as a stent; a vein graft over the T-tube limb in the liver seems to improve the result.

Twenty-four patients with clinical evidence of obstructive jaundice were examined by percutaneous transhepatic cholangiography (PTC) and needle biopsy (NB) of liver. The presence of extrahepatic bile duct obstruction was confirmed by surgery in 21 cases. PTC and the combination of both methods were superior to NB alone in the differential diagnosis between extra- and intrahepatic biliary obstruction. Sampling bias added to the difficulties of NB in distinguishing beween these two types of obstruction. The combination of both precedures proved most useful in three cases with intrahepatic obstru

Tract dilation is an essential step in completing endoscopic ultrasonography-guided hepaticogastrostomy (EUS-HGS). EUS-HGS using a 22-gauge needle is currently attracting attention; however, the complexity of subsequent dilation and guidewire exchange is problematic. A-60-year-old man with duodenal cancer was referred to our center for the drainage of obstructive jaundice. As endoscopic retrograde cholangiopancreatography was not feasible because of duodenal obstruction, EUS-HGS was performed.

A 3-year-old Rhodesian Ridgeback was examined because of recurrent pancreatitis of 2 months duration. The dog had signs of abdominal pain and jaundice. Blood biochemical findings were consistent with extrahepatic bile duct obstruction, but on abdominal ultrasonography no cause of obstruction was identified. At surgery a pancreatic pseudocyst was found in the body of the pancreas. Cystoduodenostomy, cystic omentalization and biliary diversion resulted in excellent long-term recovery.

One of the most common causes of extrahepatic cholestasis is bile duct obstruction by gallstones, bile duct strictures in chronic pancreatitis involving the head of the pancreas, or tumors in the region of the pancreas, bile ducts or gallbladder. While choledocholithiasis usually gives rise to classical clinical signs (obstructive jaundice, typical pain, and fever in the case of cholangitis), tumors often become symptomatic only when far advanced. In addition to laboratory parameters, diagnostic imaging techniques, in part with a therapeutic intervention option (ERCP), are of central importanc

The most common and serious complication of hydatid cyst of the liver is rupture into the biliary tract causing obstructive jaundice, cholangitis and abscess. The traditional treatment of biliary-cystic fistula is surgery and recently endoscopic sphincterotomy. We report a case of complex heterogeneous cyst rupture into the biliary tract causing biliary obstruction in which the obstruction and cyst were treated successfully by percutaneous transhepatic endobiliary drainage. Our case is the second report of percutaneous transbiliary internal drainage of hydatid cyst with rupture into the biliar

Spontaneous perforation of extrahepatic bile duct is a rare entity, but is the second cause of surgical jaundice in the newborn period after biliary atresia. We review the etiological theories and three patients with this anomaly. Centrolobulillar cholestasis was present in all cases. Parietal ischemic necrosis was seen in one case, obstructive lesions (extrahepatic biliary atresia) in two and malformative lesions (choledochal cyst) in the last patient. A developmental error and a small circumscribed area of duct wall weakness have been the most accepted factors involved in its pathogenesis. H

BACKGROUND: Hepatocellular carcinoma with biliary ductal invasion is rare and associated with a significantly lower survival rate.
CASE PRESENTATION: We present an unusual case of a patient with hepatocellular carcinoma and biliary invasion, who had his diagnosis confirmed by histological analysis from tissue extracted by endoscopic retrograde cholangiopancreatography. An 87-year-old male presented with a 1-day history of right upper quadrant pain and jaundice. His past medical history included recurrent gallstone cholangitis and a previous cholecystectomy. An abdominal CT demonstrated a dila

It is established that laparoscopic cholecystostomy presents the most acceptable method of gall bladder decompression in acute cholecystitis patients of advanced age comprising a high-risk group for surgery. Cholecystostomy efficiency reaches 97.2%. In case the procedure is not feasible in acute cholecystitis patients with intrahepatic position of the gall bladder or perivesicular adhesions the preference should be given to transhepatic drainage of the gall bladder whose effectiveness is 88.5%. An expedient method of laparoscopic decompression of the biliary tracts in mechanical jaundice due t
