A Machine Learning Ensemble Classifier for Early Prediction of Diabetic Retinopathy.

The main complication of diabetes is Diabetic retinopathy (DR), retinal vascular disease and it leads to the blindness. Regular screening for early DR disease detection is considered as an intensive labor and resource oriented task. Therefore, automatic detection of DR diseases is performed only by using the computational technique is the great solution. An automatic method is more reliable to determine the presence of an abnormality in Fundus images (FI) but, the classification process is poorly performed. Recently, few research works have been designed for analyzing texture discrimination ca

[Quality of care to prevent and treat postpartum hemorrhage and pre-eclampsia/eclampsia : an observational assessment in Madagascar's hospitals].

BACKGROUND: In Madagascar, where more than half of women give birth without skilled attendants, quality improvement of health services may ameliorate community perception of facility-based delivery care, thereby increasing the skilled birth attendance rate. For women who do deliver in a facility, a higher quality of services will lead to better outcomes, moving the country closer to reaching Millennium Development Goals 4 and 5. To guide the quality improvement processes at health facilities in Madagascar, this study assessed the quality of care at facilities with respect to interventions addr

Fast-track anesthesia in a pregnant female patient undergoing liver transplantation: a case report.

BACKGROUND: Liver transplantation (LT) is a critical life-saving intervention that can improve outcomes in patients with acute liver failure (ALF). However, consensus regarding LT management during the second trimester of pregnancy is lacking. For pre-delivery LT, anesthesia and perioperative management are crucial factors influencing fetal survival. Recently, fast-track anesthesia (FTA) has been applied in adult LT, with favorable outcomes. CASE PRESENTATION: A 41-year-old female patient at 22-weeks of gestation was diagnosed with ALF and hepatic encephalopathy. The patient’s condition did no

Understanding the underuse: Barriers to vacuum-assisted birth among clinicians in Tanzania: An exploratory qualitative study.

BACKGROUND: Vacuum-assisted birth (VAB) is a critical obstetric intervention for saving the lives of the mothers and newborns, particularly in cases of prolonged second-stage labor, maternal exhaustion, or fetal distress. Despite its proven benefits, VAB remains underutilized, contributing to an increased rate of unnecessary cesarean sections, some of which could be avoided with timely vacuum extraction. This study aimed to explore the barriers to the use of VAB among clinicians at a tertiary hospital in Tanzania. MATERIALS AND METHODS: An exploratory qualitative study using in-depth intervie

Impacted fetal head at cesarean delivery.

PURPOSE: Impacted fetal head (IFH) can be defined as the deep engagement of the fetal head in the maternal pelvis at the time of cesarean delivery that leads to its difficult or impossible extraction with standard surgical maneuvers. In this narrative review, we aimed to ascertain its incidence, risk factors, management and complications from the perspective of the anesthesiologist as a multidisciplinary team member. METHODS: Databases were searched from inception to 24 January 2023 for keywords and subject headings associated with IFH and cesarean delivery. RESULTS: IFH has an incidence of

Maternal and neonatal outcomes of occiput posterior vaginal delivery.

AIM: The occiput posterior (OP) position is the most common fetal malposition associated with poorer maternal outcomes compared to the occiput anterior (OA) position. However, there are no equivalent reports for women in Japan. This study aimed to investigate the maternal and neonatal outcomes of OP delivery in Japan. METHODS: This retrospective study was conducted at a tertiary hospital in Japan. Singleton pregnant women who delivered vaginally at and after 37 weeks of gestation between January 2000 and May 2023 were included. Participants were divided into OA and OP groups and pregnancy out

Recovery of pelvic floor muscle strength and endurance 6 and 12 months postpartum in primiparous women-a prospective cohort study.

INTRODUCTION AND HYPOTHESIS: To date there has been scant knowledge on the natural recovery of the pelvic floor muscles (PFMs) after childbirth. The aims of the present study were to investigate whether PFM variables at 6 and 12 months postpartum had returned to mid-pregnancy levels and assess risk factors for reduced recovery at 12 months postpartum. METHODS: This was a prospective cohort study following 235 nulliparous pregnant women from mid-pregnancy to 12 months postpartum. Vaginal resting pressure (VRP), PFM strength and endurance were assessed by manometry at 22 weeks, 6 and 12 months

Musculoskeletal findings on MRI among postpartum women with persistent pelvic pain.

INTRODUCTION AND HYPOTHESIS: Persistent postpartum pelvic pain affects one in six women, and its source is often unexplained in the absence of obvious clinical findings. Musculoskeletal injuries during childbirth are common and can be detected using MRI or US; however, pelvic imaging is not standard of care in evaluating women with persistent pain. We hypothesize that clinical symptoms in women with unexplained persistent postpartum pelvic pain will correlate with musculoskeletal abnormalities identified on MRI in > 50% of cases. METHODS: Retrospective cohort study of women with persistent po

Cesarean delivery rates and obstetric culture - an Italian register-based study.

INTRODUCTION: Cesarean delivery rates are rising due to multiple factors, including less use of operative vaginal delivery and vaginal birth after cesarean delivery, which often reflect local obstetric practices. Objectives of the study were to analyze the relations between cesarean delivery, these practices, and perinatal outcomes. MATERIAL AND METHODS: We included all deliveries in the 72 hospitals of Lombardia, a region in northern Italy, during the year 2013. The delivery certificate was used as data source. Pearson's correlation coefficient and logistic regression were used for statistic

Characterizing levator-ani muscle stiffness pre- and post-childbirth in European and Polynesian women in New Zealand: a pilot study.

INTRODUCTION: The influence of levator-ani muscles on second-stage labor is poorly understood. The ability of these muscles to stretch without damage may affect birth outcomes, but little is known about material properties, effects of pregnancy and/or ethnicity on levator-ani stiffness. There are strong associations between muscle damage and subsequent pelvic floor disorders. This study aimed to quantify levator-ani muscle stiffness during the third trimester of pregnancy and postpartum in European and Polynesian women. Associations between stiffness, obstetric variables, and the risk of intra

Joint hypermobility, obstetrical outcomes, and pelvic floor disorders.

INTRODUCTION AND HYPOTHESIS: Benign joint hypermobility syndrome may be a risk factor for pelvic floor disorders. It is unknown whether hypermobility impacts the progress of childbirth, a known risk factor for pelvic floor disorders. Our objective was to investigate the association between joint hypermobility syndrome, obstetrical outcomes, and pelvic floor disorders. Our hypotheses were: (1) women with joint hypermobility are less likely to experience operative delivery and prolonged second-stage labor; and (2) pelvic floor disorders are associated with benign hypermobility syndrome, controll

Risk factors for perinatal asphyxia at Queen Elizabeth Central Hospital, Malawi.

The aim of the study was to identify maternal risk factors for perinatal asphyxia in Malawi. Records of 100 mothers who delivered neonates with Apgar scores less than 6 at 5 minutes of birth during March to September 1998 were analyzed. The majority of the mothers were primigravidas (79%) and were within the normal childbearing ages of 20 to 34 years (61.2%). Sixty-one percent of the mothers started antenatal care at 20 to 28 weeks' gestation. Sixty-five percent of the mothers developed obstetric and medical problems that contributed to perinatal asphyxia, and of these, 12 mothers (18.5%) had

Adverse obstetric outcome in low- and high- risk pregnancies: predictive value of maternal serum screening.

OBJECTIVE: To determine whether the relationship between adverse pregnancy outcome and elevated maternal serum alpha-fetoprotein (MSAFP) and/or maternal serum hCG levels in women whose fetuses have no chromosomal abnormalities or neural tube defects is restricted to pregnancies with a priori elevated risk for pathology or also present in low-risk pregnancies. METHODS: The outcomes of pregnancy in two groups of patients with elevated MSAFP and/or maternal serum hCG values were compared with the outcomes of a reference group with normal serum values. The first study group consisted of 83 women
