Nonmalignant disorders of lymphocytes.

OBJECTIVE: To review the etiology, pathophysiology, clinical features, and laboratory findings in nonmalignant disorders of lymphocytes. DATA SOURCES: Current literature. STUDY SELECTION: Determined by author. DATA EXTRACTION: Determined by author. DATA SYNTHESIS: Assessing qualitative and quantitative changes in lymphocytes provides important information regarding immune reactions, disease conditions, and responses to therapy. Quantitative nonmalignant disorders encompass those conditions in which the total number of lymphocytes may be either increased (lymphocytosis) or decreased (lympho

Laboratory-acquired sporotrichosis.

A case of laboratory-acquired sporotrichosis is described that was associated with research activities involving isolates of Sporothrix schenckii from the 1988 sporotrichosis epidemic in the USA. Infection occurred in the absence of apparent trauma or other predisposing factors. The possibility that S. schenckii can invade healthy and intact skin is suggested.

An outbreak of Streptococcus pyogenes infection associated with calcium oxalate urolithiasis in guinea pigs (Cavia porcellus).

An outbreak of Streptococcus pyogenes infection occurred in a colony of 800 Dunkin-Hartley guinea pigs resulting in 364 (46%) deaths involving breeders, sucklings, weaners, but mainly adults used as a source of blood meals for haematophagus flies (Glossina palpalis). Clinical signs included bleeding from the nose, mouth and vagina before death. Necropsy revealed pneumonia with consolidation of one or both lungs, haemopericardium and haemothorax. There were yellowish-grey deposits in the urinary bladder of more than 50% of both affected breeders and adults used for feeding haematophagus flies.

Q fever epidemiology in Roumania.

A review on Q fever epidemiologic aspects in Roumania from the first case report in 1947 up to the actual cases is presented. It includes the dynamics of the number of cases, features of epidemics, results of surveys on slaughterhouse workers and laboratory-acquired cases, results of serologic surveys in domestic animals, and of wild life studies. Special attention is given to the sporadic character of the present cases in Roumania with emphasis on the seasonal repartition of cases, occupation of subjects, and urban versus rural proportion of cases. The necessity of elucidation of the mechanis

Improving bacterial smear detection with Wright-Giemsa staining: A multicenter study in clinical microbiology laboratories.

Improvement in bacterial detection in smear is needed because pathogens in smears and positive blood cultures (PBCs) can be missed by microscopy if Gram staining is used. This study assessed the efficacy of Wright-Giemsa staining in remedying bacterial detection failures. We conducted a prospective multicenter study involving 5667 sputum specimens and 5502 PBCs collected from five distinct centers. When initial Gram staining did not identify bacteria, Wright-Giemsa staining was performed. The study also analyzed cases of missed detections and the undetected microorganism types. Among the sputu

Clinical and Laboratory Profile of People Living with HIV/AIDS with Oral Kaposi Sarcoma.

The aim of this article was to evaluate the clinical and laboratory profile of people with oral Kaposi's sarcoma (KS) associated with AIDS (KS-AIDS), followed-up at a public university hospital in Salvador, Bahia, Brazil, in the past 10 years. We identified patients diagnosed with KS-AIDS, presenting oral manifestation from January 2007 to December 2017. We searched, in the hospital information systems, the patient demographics, diagnostic data, treatment, image studies, and oral photographic records. Of the 39 cases of KS-AIDS identified at the institution, 14 (22.8%) presented oral lesions.

Laboratory-based surveillance in Latin America: attributes and limitations in evaluation of pneumococcal vaccine impact.

Disease surveillance data are needed to monitor trends in disease activity, inform decision-making in public health and evaluate disease prevention/control measures. The Sistema Regional de Vacunas (SIREVA) supports laboratory-based surveillance of invasive pneumococcal disease (IPD) in Latin American countries, providing information on identification, distribution, and anti-microbial susceptibility of pneumococcal strains. We estimated the proportion of pneumococcal meningitis and sepsis/bacteremia cases captured by SIREVA, by comparing the number of SIREVA-reported isolates in Argentina, Bra

Targeting Asymptomatic Bacteriuria in Antimicrobial Stewardship: the Role of the Microbiology Laboratory.

This minireview focuses on the microbiologic evaluation of patients with asymptomatic bacteriuria, as well as indications for antibiotic treatment. Asymptomatic bacteriuria is defined as two consecutive voided specimens (preferably within 2 weeks) with the same bacterial species, isolated in quantitative counts of ≥105 CFU/ml in women, including pregnant women; a single voided urine specimen with one bacterial species isolated in a quantitative count ≥105 CFU/ml in men; and a single catheterized urine specimen with one or more bacterial species isolated in a quantitative count of ≥105 CFU/ml i

The C-reactive protein.

C-reactive protein (CRP) was identified in 1930 and was subsequently considered to be an "acute phase protein," an early indicator of infectious or inflammatory conditions. Since its discovery, CRP has been studied as a screening device for inflammation, a marker for disease activity, and as a diagnostic adjunct. Improved methods of quantifying CRP have led to increased application to clinical medicine. In the emergency department (ED), CRP must be interpreted in the clinical context; no single value can be used to rule in or rule out a specific diagnosis. We conclude that CRP has limited util

[Laboratory diagnosis of intestinal micro-parasitosis in AIDS patients: state of the art].

BACKGROUND: The diarrhea, often associated with growing thin and weight loss, is important for nutritional state and quality of life in AIDS patients. It was demonstrated that patients with AIDS who present diarrhea have a greater degree of immunosuppression than those without diarrhea, predisposing the gastrointestinal tract to the action of protozoa, bacterial, viral, fungal pathogens which may cause morbidity and death. HIV-patients are particularly susceptible to protozoa infections. Cryptosporidium infection is among the most common causes of enteric disease but Microsporidium (Enterocyto

Mycotic morbidity--an occupational risk for mycologists.

Classical mycologists may be at a greater risk for infection with organisms under investigation than medical mycologists. The methods of infection of human beings by systemic mycotic pathogens is known. These principles can be used to develop good laboratory practices for classical mycological investigators. Newly recognized fungal pathogens and a more susceptible population may cause more laboratory infections. Minimal safety practices to prevent mycotic morbidity are described.

[An outbreak of "wet tail" in hamsters due to Tyzzer's organism (author's transl)].

In a colony of Golden hamsters, many animals were found to show signs of "wet tail", and spontaneous deaths occurred during two months. With this outbreak of wet tail and death, etiologic and pathologic studies were made on 11 cases supplied from the colony. Gross pathologic examination disclosed hemorrhagic enteritis in 10 and necrotic foci in the liver in 5 out of 10 cases examined. A number of bacilli morphologically suggestive of Tyzzer's organisms were detected on smears of cut surfaces of the affected intestines and livers. After inoculation with homogenate of the intestines from the mor
