Some oncologic considerations in the treatment of lip cancer.

We evaluated the treatment and results of 46 patients with lip cancer of commissure origin or extension. The patients were evaluated by clinical stage and pathologic findings. Treatment was predominantly by surgery for T1 and T2 lesions (31 of 37 patients), whereas combined resection and radiation therapy was used for larger tumors. The results showed control of both very large and very small lesions, but T2 lesions had repeated recurrences. We therefore recommended aggressive therapy for T2 and higher classifications of lip cancer with commissure involvement.

Implication of the New AJCC pT Classification of SCC of the Lip Comparing With Other Oral Subsites.

OBJECTIVE: To determine the implication of the new AJCC staging system for pT classification in a cohort of patients with SCC of the lip mucosa and compare it to other oral cavity sites. METHODS: Retrospective cohort of 744 patients treated between 2002 and 2017, by the Head and Neck Surgery Department of the University of Sao Paulo. RESULTS: Of 95 lip patients, 42 had pT upstage (58.1% of pT1 to pT2-3 and 50% of pT2 to pT3). Similar DFS/OS observed for those pT1 maintained or upstaged to pT2-3, pT2 patients upstaged to pT3 presented worse OS (49.4% versus 92.3%, P = .032). The comparison be

Facial sensory loss--a need for suspicion.

Three patients with facial sensory loss were referred by their general dental practitioners. In the absence of local or systemic factors to account for such symptoms, the dental surgeon should be alert to the possibility of sinister underlying pathology, possibly remote from the jaws. Even in the face of negative special tests, patients should be thoroughly investigated and referred accordingly. Following referral, an exhaustive search should be undertaken for possible causes, particularly for the possibility of metastatic disease.

Regional epidemiological features of lip, oral cavity, and oropharyngeal cancer.

Lip, oral cavity, and oropharyngeal cancer are among the most common forms of the disease in the world. These types of cancer display significant geographic, ethnic, and socioeconomic variations. We examined the cases of cancer of the lip, oral cavity, and oropharynx diagnosed in the Department of Otolaryngology at the University of Uludağ School of Medicine during the last 5 years, July 1990 to June 1995, and recorded the epidemiological features of these tumors. The Department of Otolaryngology treated a total of 26,225 in- and outpatients during the 5-year period. 320 of these patients (1.2

Squamous cell carcinoma of the lip and oral cavity in The Netherlands; an epidemiological study of 740 patients.

The epidemiological data of 740 patients with primary squamous cell carcinoma (SCC) of the lips or oral cavity is reported. The mean age was 63 years. The mean onset of oral cancer in women was 7 years later than in men. Patients under the age of 40 years were mainly males. The male-to-female ratio ranged from 8.3 for the lower lip to 0.9 for the upper alveolar ridge with an overall ratio of 1.8. In the case of multiple oral SCCs the tongue was the most frequently affected site. No significant association was found between the age or gender of patients and the size of the tumour. However, a si

Swedish agricultural workers. A group with a decreased risk of cancer.

A Cancer-Environment Registry was established in Sweden in 1978. A study of the register revealed that for persons engaged in agricultural work, the risk for most types of cancer was lower than the respective national averages. For cancer of the trachea, bronchus, lung and pleura, the ratio of observed to expected numbers of cases is 0.39 (observed 934, expected 2375). The difference is statistically significant. For laryngeal cancer the ratio is 0.38 (observed 94, expected 248), which likewise is significant. Primary cancer of the liver and tongue also show less than expected frequency. The r

Treatment and outcome for 2,111 patients with vermilion lip squamous cell carcinoma: A nationwide, population-based study from the SweHNCR.

BACKGROUND AND IMPORTANCE: The major risk factor for vermilion lip squamous cell carcinoma (vlSCC) is excessive sunlight exposure. Population-based studies on treatment and outcome are lacking. METHODS: This is a nation-wide, population-based study of treatment and outcome from prospectively recorded data for 2,111 vlSCC registered in the Swedish Head and Neck Cancer Register (SweHNCR) diagnosed 2008-2022. RESULTS: The age standardized incidence increased with 34 % between 2008 and 2022, from 1.14 to 1.53 per 100,000 (19 % for males and 55 % for females). Median age at diagnosis was 76 years

Intra-Lesional Injection of Triamcinolone to Palatoplasty Scar to Aid Reversal of Transverse Maxillary Relapse after Orthognathic Surgery.

A 32-year-old female with a repaired right unilateral cleft lip and palate underwent several surgical and orthodontic procedures during the rehabilitation process of her condition. Nine years after this extensive treatment she underwent transverse relapse of her maxilla and requested a consultation for its correction as she felt her speech and chewing were negatively affected. She presented with a transverse maxillary arch collapse on the cleft side with significant palatal scarring secondary to multiple palate procedures. A course of maxillary expansion and dental alignment with fixed orthodo

[A Patient with Lower Lip Verrucous Carcinoma Treated with a Tongue Flap for Functional and Esthetic Reconstruction].

Surgery is generally indicated for the treatment of lower lip carcinoma. However, surgery can lead to esthetic as well as functional issues, with disturbances in eating and articulation. If the defect involves less than one-third of the width of the lower lip, primary closure is possible. If the defect is wider, reconstructive local flaps may be necessary for both esthetic and functional recovery. This report describes a case of lower lip verrucous carcinoma in which a tongue flap was used to treat a defect involving more than half the width of the lower lip following resection. The patient wa

Sling suspension using the anterior third of the temporalis muscle extended with tendon graft to correct oral incontinence after free flap reconstruction in cases of a major defect of the lower lip muscle.

BACKGROUND: Oral incontinence is a common complication after free flap reconstruction after ablation of oral cancers. The postoperative incontinence was corrected with strong suspension to the temporalis muscle in cases of major or total defect of lower lip muscles. METHODS: Eight patients with oral cancer with previous skin flaps for reconstruction of the lower lip had incontinence because of major or total loss of the lower lip muscle. A tendon graft was used as a suspension sling sutured to the anterior third of the bilateral temporalis muscle. RESULTS: Oral continence without drooling wa

Extension of the facial artery musculomucosal flap to reconstruct a defect of the soft palate.

Reconstruction after resection of a large oropharyngeal tumour poses a difficult challenge. This case illustrates an extension of the previously described facial artery musculomucosal (FAMM) flap, whereby a defect of the soft palate is replaced with a similar trilaminar structure in the form of irradiated, redundant lower lip. This allows not only the treatment of the palatal defect, but also management of dribbling secondary to a marginal mandibular palsy.

Parotid gland biopsy compared with labial biopsy in the diagnosis of patients with primary Sjogren's syndrome.

OBJECTIVE: To assess the value of the parotid biopsy as a diagnostic tool for primary Sjögren's syndrome (pSS), and to compare the parotid biopsy and the labial biopsy with regard to diagnostic value and biopsy-related morbidity. METHODS: In 15 consecutive patients with pSS and 20 controls, the parotid biopsy was assessed as a diagnostic tool based on the presence of lymphocytic foci, benign lymphoepithelial lesions and lymphoid follicles. These new histological criteria were compared with established diagnostic criteria for the labial biopsy in 35 consecutive patients suspected for pSS who u

Midline cleft lip and nasal dermoids over five generations: a distinct entity or autosomal dominant Pai syndrome?

A family with midline nasal dermoids in four members over five generations is described. Two of these individuals also had a midline cleft lip. The pattern of inheritance appears to be autosomal dominant with variable expression. To our knowledge, no familial cases of midline clefting with nasal dermoids have previously been reported in the literature. Cranial imaging to look for intracerebral extension of the dermoid is recommended in such cases.

Burkitt's lymphoma: an unusual cause of lip numbness.

A 38-year-old man was referred by his dental surgeon with a painful swelling in the right mandibular sulcus and an ipsilateral numb lip. Dental infection secondary to caries was initially diagnosed but biopsy revealed this to be a Burkitt's lymphoma. After intense chemotherapy, the patient is still in remission three years later. This case is a reminder that malignancy should always be high on a clinician's list of differential diagnoses of jaw swelling associated with neurological abnormalities and prompt referral is essential.

Reconstruction of concomitant lip and cheek through-and-through defects with combined free flap and an advancement flap from the remaining lip.

Massive facial defects involving the oral sphincter are challenging to the reconstructive surgeon. This study presents the authors' approach to simultaneous reconstruction of complex defects with an advancement flap from the remaining lip and free flaps. From January of 1997 to December of 2001, 22 patients were studied following ablative oral cancer surgery. Their ages ranged from 32 to 66 years. Nineteen patients had buccal cancer, two patients had tongue cancer, and one patient had lip cancer. In all cases, the disease was advanced squamous cell carcinoma. Nine patients underwent composite
