Three-dimensional analysis of upper airway morphology in skeletal Class III patients with and without mandibular asymmetry.

OBJECTIVE: To compare the three-dimensional (3D) morphology of the upper airway in skeletal Class III patients with and without mandibular asymmetry and to investigate the possible underlying correlations between the morphology of the upper airway and mandibular deviation. MATERIALS AND METHODS: Cone-beam computed tomography images of 54 subjects with skeletal Class III malocclusion (ANB angle ≤ 0.4°, Wits ≤ -5.5°) were taken and 3D upper airway models were reconstructed using Dolphin 3D software. According to the distance (d) from symphysis menti to the sagittal plane, all subjects were divi

An in vitro study in separating tensile loads during maxillo-mandibular fixation using wire and/or elastics.

Intermaxillary fixation (IMF) or maxillo-mandibular fixation (MMF) is a fundamental process in stabilizing the maxilla and mandible through dental maximum intercuspation (MIP) during the management of trauma, orthognathic surgery, and reconstruction. Despite the availability of several techniques in achieving adequate maxillo-mandibular fixation, concerns have been raised regarding the sufficiency of using only latex elastics to counter displacing forces during reduction and fixation. To address this, an in vitro study was conducted to compare the efficacy of three maxillo-mandibular fixation

Morphological variability of inferior alveolar nerve in low-grade craniofacial microsomia.

Craniofacial microsomia (CFM) involves asymmetric hypoplasia and dysmorphogenesis of the facial skeleton. Certain aspects of CFM may be treated by surgical osteotomy and distraction osteogenesis (DO). Mandibular osteotomy places the inferior alveolar nerve at risk. The aim of this study was to investigate radiological landmark relationships to the anatomy of the inferior alveolar nerve in CFM. Application of this understanding will aid intraoperative protection of the inferior alveolar nerve. Six subjects with similar presentations of hemifacial microsomia were selected. Three-dimensional reco

Radiological and Histological Findings of Primary Pleomorphic Rhabdomyosarcoma Arising from the Mandibular Gingiva: A Case Report and Literature Review.

Background: Pleomorphic rhabdomyosarcoma (RMS) is a rare, adult-predominant high-grade sarcoma that usually arises in the deep soft tissues of the extremities. Primary oral pleomorphic RMS is exceptionally rare, and detailed CT and MRI characteristics of oral pleomorphic RMS remain sparsely documented. Case Presentation: A 66-year-old woman presented with a two-month history of lower anterior tooth pain and progressive mandibular swelling, initially misdiagnosed and treated as a dental infection. CT and MRI revealed a 3.8-cm heterogeneously enhancing mass centered in the mandibular gingiva, wi

[Current problems in the diagnosis and treatment of injuries to the temporomandibular joint].

Complex examination of 2103 patients with temporomandibular joint (TMJ) injuries was carried out. This examination included contrast arthrotomography, computed tomography (CT) and MR tomography of the TMJ. It gas been established, that in mandibular condyle fractures not only bone structures of the TMJ, but also soft tissue ones (the articular disk and capsular-ligamentous apparatus) are being injured. Our investigation has shown that condylar fractures in the region of condylar base and neck are combined with the injuries of the articular disk and capsular-ligamentous apparatus in 54.1 +/- 5.

Subtle Morphological Changes in the Mandible of Tabby Mice Revealed by Micro-CT Imaging and Elliptical Fourier Quantification.

X-linked hypohidrotic ectodermal dysplasia (XLHED) is a genetic disorder due to a mutation of the EDA gene and is mainly characterized by an impaired formation of hair, teeth and sweat glands, and craniofacial dysmorphologies. Although tooth abnormalities in Tabby (Ta) mutant mice - the murine model of XLHED - have been extensively studied, characterization of the craniofacial complex, and more specifically the mandibular morphology has received less attention. From 3D micro-CT reconstructions of the left mandible, the mandibular outline observed in lateral view, was quantified using 2D ellipt

Fibula free flap for mandible reconstruction.

OBJECTIVES: Microvascular mandibular reconstruction should be considered the procedure of choice for bone replacement in radiated tissue beds, orocutaneous fistulae with segmental bone loss, osteoradionecrotic mandibles, and for immediate composite tissue reconstruction. The free vascularized fibula has significant potential for the reconstruction of the mandible. We evaluated our clinical experience in 21 cases of mandible reconstruction with fibular flaps. PATIENTS AND METHODS: Between 1997 and 2002, we performed 7 free osseous fibula flaps and 14 osteocutaneous fibula flaps for a total of …

Custom facial reconstruction for osteosarcoma of the jaw.

Osteosarcoma accounts for most primary bone cancers in children and young adults. High-grade lesions are typically managed with a combination of chemotherapy and wide-margin surgical excision. Although this malignancy typically affects the metaphyseal region of long bones, it also can be seen in the axial skeleton. Of axial locations, tumors in the head and neck can be particularly troubling to treat. Segmental bone loss after resection of malignant mandibular tumors continues to present important challenges to the reconstructive surgeon. Recent advancements in 3-dimensional modeling have faci

Extended and unusual indications in jaw reconstruction with the fibula flap: An overview based on our 30-year experience.

Since the introduction of fibula flap as a reconstructive technique, an evolution of indications has been observed. Our first report of a traumatic mandibular reconstruction using fibula flap was in 1992. The vast majority of indications for surgery, are: malignant tumors, benign neoplasms, osteoradionecrosis and traumas. Nevertheless, extended indications have been described such as the treatment of dentoalveolar defect without bone discontinuity or reconstruction of maxilla defect up to type III (A and B), according to Cordeiro's classification. Unusual indications include cleft palate malfo…

Salvage Secondary Reconstruction of the Mandible with Vascularized Fibula Flap.

Primary restoration of the mandibular continuity remains the standard of care for defects, and yet several constraints preclude this objective. Interim reconstructions with plate and nonvascular bone grafts have high failure rates. The secondary reconstruction, when becomes inevitable, remains a formidable task. This retrospective study evaluates various issues to address secondary reconstruction. Twenty-one patients following mandibulectomy presented with various complications between 2012 and 2016 were included in the study. The profile of primary reconstruction includes reconstruction plate
