Mediastinal fluid as a predictor for esophageal perforation as the cause of pneumomediastinum.

PURPOSE: Pneumomediastinum is not an uncommon finding on emergency CTs and is seen in a variety of settings, including acute trauma, recent repeated emesis and recent thoracic surgery, and after EGD (Kouritas et al. J Thorac Dis 7(Suppl 1):S44-S49, 2015). The purpose of this study was to examine cases of CT pneumomediastinum for distinguishing features which could support or exclude esophageal injury as a cause, which in turn could help guide evaluation of these patients. METHODS: CT chest scans showing pneumomediastinum performed in the emergency department between January 2013 and December

[Boerhaave's syndrome and pleural tuberculosis].

INTRODUCTION: Boerhaave's syndrome is a rare condition involving spontaneous rupture of the oesophagus. The diagnosis is difficult and the prognosis depends on early diagnosis and treatment. CASE REPORT: We report a case of spontaneous rupture of the oesophagus in a 70-year-old woman where the diagnosis was delayed. Initial treatment comprised dual antibiotic therapy and feeding via a jejunostomy. Three months later, direct suture of the oesophagus was performed successfully. A routine pleural biopsy performed during the surgery revealed pleural tuberculosis. Antituberculous treatment was giv

Pneumatocele With Perforation of the Residual Lung Immediately After Thoracoscopic Left Lower Lobectomy: A Case Report.

Cases of pneumatocele formation following pulmonary resection are increasingly reported, yet its underlying pathogenesis and optimal treatment remain unclear. Here, we present a case of pneumatocele perforation occurring immediately after thoracoscopic left lower lobectomy. A 66-year-old man was found to have a suspicious nodule in the left lower lung zone during a routine chest X-ray as part of a medical checkup. CT suggested lung cancer in the left lower lobe, prompting referral to our hospital for further evaluation and treatment. Contrasted CT imaging revealed a ground-glass nodule with a

Building a model for the differential diagnosis of non-tuberculous mycobacterial lung disease and pulmonary tuberculosis: A case-control study based on immunological and radiological features.

BACKGROUND: Although the incidence of non-tuberculous mycobacterial pulmonary disease (NTM-PD) is increasing annually, it is easily misdiagnosed as pulmonary tuberculosis (PTB). This study aimed to screen and identify the immunological and radiological characteristics that differentiate NTM-PD from PTB and to construct a discriminatory diagnostic model for NTM-PD, providing new tools for its differential diagnosis. METHODS: Hospitalised patients diagnosed with NTM-PD or PTB between January 2019 and June 2023 were included in the study. Immunological and radiological characteristics were compa

CT of the chest with model-based, fully iterative reconstruction: comparison with adaptive statistical iterative reconstruction.

BACKGROUND: The recently developed model-based iterative reconstruction (MBIR) enables significant reduction of image noise and artifacts, compared with adaptive statistical iterative reconstruction (ASIR) and filtered back projection (FBP). The purpose of this study was to evaluate lesion detectability of low-dose chest computed tomography (CT) with MBIR in comparison with ASIR and FBP. METHODS: Chest CT was acquired with 64-slice CT (Discovery CT750HD) with standard-dose (5.7 ± 2.3 mSv) and low-dose (1.6 ± 0.8 mSv) conditions in 55 patients (aged 72 ± 7 years) who were suspected of lung dis

Comparison between lymph and non-lymph node resection in patients with stage IA non-small-cell lung cancer: A retrospective study.

OBJECTIVE: This retrospective study aimed to determine the need for lymph node resection during surgical treatment in patients with stage IA non-small-cell lung cancer (NSCLC). MATERIALS AND METHODS: A total of 1428 patients diagnosed with cT1N0M0 1 A stage NSCLC who underwent surgery were divided into two groups: lymphadenectomy (n = 1324) and nonlymphadenectomy (n = 104). The effects of lymph node resection on overall survival (OS) and recurrence-free survival (RFS) and on clinicopathological factors that affected the prognosis of the patients were investigated. RESULTS: The group that und

Spontaneous Pneumomediastinum Presenting as Dysphonia.

Spontaneous pneumomediastinum is a rare condition describing air within the mediastinum from a non-traumatic etiology from an intrathoracic or extrathoracic source. It typically affects young men through a mechanism known as the Macklin effect, in which alveolar air travels along bronchovascular sheaths into the mediastinum. There are known predisposing conditions as well as activities that can precipitate spontaneous pneumomediastinum, which is generally self-limited, but in rare circumstances, can lead to serious complications such as tension pneumomediastinum, mediastinitis, and tension pne

Serious complications were pneumoperitoneum (one case), mediastinal emphysema combined with pleural effusion (two cases). CONCLUSIONS: Endoscopic anterior fundoplication with MUSE was an effective treatment for refractory GERD, but still needs refinement and improvement in safety aspect. Esophageal hiatal hernia may affect t …

Successful pericardial repair and coverage for late pacemaker lead-related atrial perforation and pneumothorax: a case report.

BACKGROUND: Various complications of permanent pacemaker implantation have been reported. However, late pacemaker lead-related myocardial perforation rarely occurs. Conservative treatment is generally selected, if possible, but open heart surgery or catheter lead removal should be considered in symptomatic patients. We herein describe a patient who was successfully treated by pericardial repair and coverage for late pacemaker lead-related atrial perforation and pneumothorax. CASE PRESENTATION: A woman in her 80s who had undergone permanent pacemaker implantation 2 years previously visited our

A Case Report of Spontaneous Pneumomediastinum From an Unusual Cause: Baritone Practice.

Spontaneous pneumomediastinum (SPM) is a rare but potentially life-threatening clinical entity in which free air is introduced into the mediastinum. It most commonly presents in young males and has an incidence of approximately 0.002% of the general population. Symptoms include sudden onset chest pain, dyspnea, neck pain, vomiting, and odynophagia. Physical examination usually reveals subcutaneous emphysema, hoarse voice, tachycardia, tachypnea, and occasionally a Hamman's sign, which is a mediastinal "crunch" sound heard on cardiac auscultation. We present a case of an 18-year-old male barito

Pleomorphic Carcinoma of the Lung: Relationship Between CT Findings and Prognosis.

OBJECTIVE: The objective of our study was to assess the radiologic and clinical findings of pleomorphic carcinoma (PC) of the lung and to evaluate whether there are any characteristic features that can be used to predict prognosis. MATERIALS AND METHODS: Forty-four consecutive patients whose diagnosis of PC was histologically confirmed through resection of the lung tumor were included in this study. The clinical and CT findings of these patients were retrospectively reviewed. Two thoracic radiologists evaluated the CT findings including the size, location, internal characteristics, and margin

Penetrating neck traumas.

AIM OF THE STUDY: Aim of the study is to present our own experiences in the treatment of people suffering from penetrating neck traumas. MATERIAL AND METHODS: In the years 1996-2012, 10 patients with penetrating neck traumas were treated, including 3 women and 7 men. The patients' age ranged from 16 to 55 (the average age being 40.7 years). In 9 cases the wound was caused by cutting or stabbing, while in one case it was inflicted by a gunshot. In 8 patients it was a single cut wound, while one patient suffered from 34 stab wounds to the neck, chest and stomach. Two cut wounds resulted from a
