From Vitality to Vital Exhaustion and Other States of "Tense Tiredness": A New Biopsychosocial Risk Domain.

Fatigue is a common prodromal symptom for various medical conditions, including acute myocardial infarction. Fatigue is also the core component of vital exhaustion, which consists of a specific triad: excessive fatigue, increased irritability, and feelings of demoralization. In this issue of Psychosomatic Medicine, Frestad and Prescott present a meta-analysis of 16 studies, involving 53,337 participants, which found vital exhaustion to be associated with an increased risk of incident coronary heart disease (CHD) and recurrent cardiac events among individuals with established CHD. After discuss

What holds the egalitarian processes together? The case for norms.

This commentary argues that the process-based account of egalitarianism overlooks a central component: norms. Norms are socially enforced and morally grounded behavioral standards that enable and regulate egalitarian processes. I show that many processes discussed by the authors rely on underlying norms, and argue that incorporating norms as a distinct category clarifies individual motivation and the relational flexibility of egalitarian systems.

The Surgeon as a Professional: Changes and Challenges Over Time.

OBJECTIVE: In this article, we seek to use a case-study discussion of a woman seeking treatment guidance for an elective, complex surgical ailment to discuss how professionalism has changed within the past century and where it may be headed with specific regard for the medical profession and surgeons. BACKGROUND: Traditionally, professionals were those who possessed a certain knowledge base, committed to an ╗altruistic mission through serving the general populace while adhering to certain performance standards that were established by other members of the field. In the 20th century, we saw ce

Children's and Adults' Views of Punishment as a Path to Redemption.

The current work investigated the extent to which children (N=171 6- to 8-year-olds) and adults (N = 94) view punishment as redemptive. In Study 1, children-but not adults-reported that "mean" individuals became "nicer" after one severe form of punishment (incarceration). Moreover, adults expected "nice" individuals' moral character to worsen following punishment; however, we did not find that children expected such a change. Study 2 extended these findings by showing that children view "mean" individuals as becoming "nicer" following both severe (incarceration) and relatively minor (time-out)

The informalization of doctor-patient relations in a Finnish setting: New social figurations and emergent possibilities.

This article features data drawn from interviews with doctors working in the Finnish occupational health-care system. These are used to explore the value of an Eliasian approach towards interpreting and assessing the moral meanings and social dynamics of relationships between health practitioners and their patients. We attend to spiralling 'formalizing' and 'informalizing' processes and how these are operating to reconfigure doctor-patient relationships. We document some of the ways in which Finnish doctors are adapting to these processes. While data drawn from a British context suggest both d

'Delusional' consent in somatic treatment: the emblematic case of electroconvulsive therapy.

Even more than for other treatments, great importance must be given to informed consent in the case of electroconvulsive therapy (ECT). In a percentage of cases, the symbolic connotation of the treatment, even if mostly and intrinsically negative, may actually be a determining factor in the patient's motives for giving consent. On an ethical and medicolegal level, the most critical point is that concerning consent to the treatment by a psychotic subject with a severely compromised ability to comprehend the nature and objective of the proposed therapy, but who nonetheless expresses his consent,

Re-examining the Ethics of Genetic Counselling in the Genomic Era.

Respect for patient autonomy has served as the dominant ethical principle of genetic counselling, but as we move into a genomic era, it is time to actively re-examine the role that this principle plays in genetic counselling practice. In this paper, we argue that the field of genetic counselling should move away from its emphasis on patient autonomy and toward the incorporation of a more balanced set of principles that allows counsellors to offer clear guidance about how best to obtain or use genetic information. We begin with a brief history of how respect for patient autonomy gained such emp

[Neuroethics: new quality of medical ethics?].

During the last decade there has been a striking growth in interest in ethical issues arising from the development of neurosciences. It was as late as 2002 that the new discipline, called neuroethics, started. It was intended to be a new area of interdisciplinary discourse on moral dilemmas connected with recent advances in, broadly understood, neurosciences. Ten years after its launch neuroethics possesses a distinct body of knowledge and an institutional basis for its further development. As a very young discipline, however, neuroethics is still in a state offlux. Two essential theoretical c

Free riders and pious sons--why science research remains obligatory.

John Harris has previously proposed that there is a moral duty to participate in scientific research. This concept has recently been challenged by Iain Brassington, who asserts that the principles cited by Harris in support of the duty to research fail to establish its existence. In this paper we address these criticisms and provide new arguments for the existence of a moral obligation to research participation. This obligation, we argue, arises from two separate but related principles. The principle of fairness obliges us to support the social institutions which sustain us, of which research

[Conceptualization of Narrative Bioethics in Healthcare].

Ethical dilemmas in real-world medical settings expose the limitations of bioethics, represented by the "four-principle framework," in dynamic and personalized clinical decision-making.The development of narrative medicine provides more space for showcasing the unique context of life, alleviating the predicament of normative ethics. However, current ethical practices in narrative medicine have not responded to the theoretical problem of the lack of ontology, moral subject, and reflective process in principlism, calling for the further exploration of the moral issues inherent in the narrative process

Ethical Dilemmas Relating to the Management of a Newborn with Down Syndrome and Severe Congenital Heart Disease in a Resource-Poor Setting.

Decision-making regarding treatment for newborns with disabilities in resource-poor settings is a difficult process that can put parents and caregivers in conflict. Despite several guidelines that have helped to clarify some of the medical decision-making in Ghana, there is still no clear consensus on the specific moral criteria to be used. This article presents the case of a mother who expressed her wish that her child with Down syndrome should not have been resuscitated at birth. It explores the ethical issues at stake in both her misgivings about the resuscitation and her unwillingness to c

Third party assisted conception: an African perspective.

The central importance of reproduction in all human cultures has given rise to many methods and techniques of assisting reproduction or overcoming infertility. Such methods and techniques have achieved spectacular successes in the Western world, where processes like in vitro fertilization (IVF) constitute a remarkable breakthrough. In this paper, the author attempts to reflect critically on assisted reproduction technologies (ART) from the background and perspective of African culture, a culture within which human reproduction is given the highest priority but which also exhibits a highly ambi

Commentary on "Genetic harm: bitten by the body that keeps you?

Several important issues are raised and illuminated in "Genetic Harm"; not least, in its detailed discussion of specific genetic disorders. In particular, it focuses on a type of disorder whose ill effects are not manifested at birth, but only at a later stage in life. The conclusion, with its significant implications for practice, seems quite valid: a moral duty should be recognized to genetically (or otherwise, if feasible) cure an embryo of that which is expected to (later) cause such prospective suffering. Yet the reasons given for that conclusion, as well as much of the argument throughou

Female psychiatrists on screen: Cinematic stereotypes and therapeutic authority, 1935-2021.

Throughout the 20th century, women entered psychiatry, yet cinematic representations remained structured by masculinised archetypes such as Dr Evil, Dr Wonderful, and Dr Dippy. This article interrogates depictions of female psychiatrists in nineteen films and one paradigmatic television series (1935-2021) through a historically grounded qualitative analysis. It identifies two gendered figures - Dr Correction and Dr Trapped - that reconfigure the masculine triad into a moral continuum of care, control and vulnerability. By situating these tropes within psychiatry's shifting professional and mor

Contested illness, contested identity: How women with fibromyalgia construct legitimacy online.

This research explores the lived experiences of women with fibromyalgia (FM) and the various ways they go about legitimizing a contested medical condition. Through ethnographic observation in a private online community, "Fibro Women Canada," and in-depth interviews with its members, we explore how women work to be seen as legitimate pain patients in the eyes of their healthcare providers. We argue that FM produces a dilemma of legitimacy, a dilemma of identity, and a dilemma of morality for its sufferers. In the face of these dilemmas, Fibro Women Canada functions as a backstage environment th

Supported Home Ownership and Adult Independence in Milan: The Gilded Cage of Family Housing Gifts and Transfers.

This article analyses practices of intergenerational support for homeownership among different generations of families in Milan, Italy, highlighting the role of housing in family welfare relations and life-course transitions. It makes use of an original dataset of qualitative interviews investigating homeownership pathways and the negotiations of support that they pre-suppose. The article explores the meanings and moral reasoning behind the decision to accept (or not) support in context of contemporary discourses surrounding the liquidity and availability of housing and finance. It highlights

[Legitimidad de la histerectomía para resolver el "problema" de la higiene menstrual en adolescentes con discapacidad intelectual].

Menstrual hygiene in adolescents with intellectual disability (ID) represents an extra burden for parents or primary caregivers, especially in developing countries, where social institutions, including the health system, lack the capability to help this group of teenagers and their families; hence, hysterectomy to eliminate menstrual bleeding is considered a morally acceptable resource. Hysterectomy to solve the "problem" of menstrual hygiene reflects obstacles that affect the care provided by the physician to the adolescent with ID: on one hand, the criterion of social value about a "poor qua

Shared predictors of youthful gambling, substance use, and delinquency.

Given that gambling, alcohol misuse, other drug use, and delinquency are correlated, it is hypothesized that these problem behaviors have shared antecedents. Measures from 3 explanatory domains--sociodemographic factors (age, race, and socioeconomic status), individual factors (impulsivity and moral disengagement), and socialization factors (parental monitoring and peer delinquency)--were tested for links to problem behaviors in 2 longitudinal samples of adolescents. Black youth had lower levels of problem behaviors than Whites. Impulsivity was a significant predictor of alcohol misuse for fem

Revealing the secrets of the body: medical tests as legal evidence in personal status disputes in modern Egypt.

The study seeks to answer the following question: In the field of personal status, what kinds of medical tests have been considered reliable and acceptable by Egyptian courts of law, what kinds have been less accepted, and why? The main finding is that, on the one hand, the judges (qadis) have welcomed tests that seek to determine age or to discover physical diseases and/or mental disorders. On the other hand, they have been reluctant to introduce tests to determine virginity and pregnancy tests, and even more so to introduce paternity tests that might bring into light manifestations of moral
