Clinicopathological factors influencing the number of stages of Mohs surgery for basal cell carcinoma.

BACKGROUND: Mohs Micrographic Surgery (MMS) is commonly used to treat high-risk basal cell carcinoma (BCC). OBJECTIVES: Correlate clinicopathologic preoperative features with the number of MMS stages (primary endpoint) and margins (secondary endpoint) required for BCC complete excision. METHODS: We retrospectively analyzed BCCs treated by MMS in a 2-year period at the study's institution. Variables studied included the patient gender, age, immune status, lesion size, location, if it was a primary, recurrent, or persistent tumor, histopathologic characteristics, number of surgical stages, and

Lateral eyelid island flap for reconstruction of large central and medial full-thickness defects of the lower eyelid.

INTRODUCTION: To report our experience with lateral eyelid island flaps for reconstruction of large central and medial full-thickness defects of lower eyelid. METHODS: Retrospective case series of patients with large central and medial full-thickness lower lid defects repaired with medial transposition of the lateral lower lid combined with a temporal myocutaneous flap. Preoperatively the authors collected demographic data, etiology and percentage of the defects. Postoperatively the lids were assessed for flap viability and lower eyelid margin position and contour. RESULTS: The sample was co

Perianal Basal Cell Carcinoma: 35-Year Experience.

BACKGROUND: Basal cell carcinoma of the perianal region is a rare anorectal disease. This condition is not related to exposure to ultraviolet radiation. Because of the low prevalence and poor detection, there is a paucity of data relating to this condition in the literature. Perianal basal cell carcinoma presents surgical challenges different from other anatomic locations and may not share the same prevalence or natural history. Here, we describe the largest series to date on the surgical management of perianal basal cell carcinoma. OBJECTIVE: We aimed to present our 35-year experience in man

Does hydrochlorothiazide increase the incidence of skin, lip and oral cancer in a UK population?

Data sources Data was from The Health Improvement Network (THIN) database from January 1999 to May 2016.Study selection This was a series of population-based, case-control studies looking to evaluate the association between hydrochlorothiazide (HCTZ) exposure and skin, lip and oral cancer in the UK population.Case/control selection Using the THIN database, patients with the following outcomes were grouped: squamous cell carcinoma (SCC) skin cancer; basal cell carcinoma (BCC) skin cancer; melanoma; lip cancer and oral cancer. Patients within the lip cancer and oral cancer groups were accepted w

Transnasal or transglabellar semicircular flap for medial canthal reconstruction.

PURPOSE: To review the results of medial canthal reconstruction with the medial (transnasal or transglabellar) semicircular flap. METHODS: Medical charts of 38 patients who underwent the described procedure were reviewed. After tumor excision, a semicircular flap created along the nasal bridge or glabella was advanced to the canthal defect; if necessary, this flap was combined with other reconstructive methods. RESULTS: The patients (19 male, 19 female; mean age, 66 years) had basal cell (n = 36) or squamous cell (n = 2) carcinomas. The mean tumor diameter was 9.2 mm (range, 3-21 mm). Tumor

Association between hydrochlorothiazide exposure and different incident skin, lip and oral cavity cancers: A series of population-based nested case-control studies.

AIMS: Hydrochlorothiazide-induced photosensitivity may increase squamous cell carcinoma (SCC), basal cell carcinoma (BCC) and lip cancer risk. The aim was to quantify these risks. METHODS: Nested case-control studies using data from the UK THIN database from 01 January 1999 to 01 May 2016. Adults with incident SCC, BCC, melanoma, lip cancer and oral cancer were matched (on age, sex and calendar year of cohort entry) to controls using incidence density sampling. Incidence rate ratios (IRR) for each outcome were calculated for ever and cumulative hydrochlorothiazide exposure, measuring the impa

When basal cell carcinomas became giant: an Italian multicenter study.

BACKGROUND: Giant basal cell carcinoma (GBCC) is a basal cell carcinoma (BCC) enlarged in a diameter more than 5 cm. Since GBCCs are a highly infrequent entity and the occurrence rate is approximately 0.5-1% out of all BCC types, only anecdotal cases are reported, and causes and characteristics inducing development of this tumor are not defined. OBJECTIVES: Evaluate causative factors and clinico-histological characteristics of GBCCs. METHODS: The study is a 6-month, hospital-based case series study performed in 12 Italian dermatologic centers. RESULTS: A total of 59 cases and 458 control BC

Tumor Evolution in Two Patients with Basal-like Breast Cancer: A Retrospective Genomics Study of Multiple Metastases.

BACKGROUND: Metastasis is the main cause of cancer patient deaths and remains a poorly characterized process. It is still unclear when in tumor progression the ability to metastasize arises and whether this ability is inherent to the primary tumor or is acquired well after primary tumor formation. Next-generation sequencing and analytical methods to define clonal heterogeneity provide a means for identifying genetic events and the temporal relationships between these events in the primary and metastatic tumors within an individual. METHODS AND FINDINGS: We performed DNA whole genome and mRNA

Concurrent Paget's disease and basal cell carcinoma of the vulva; a case report.

An 82-year-old Caucasian woman had a long-standing history of recurrent Paget's disease of the right perianal region that was documented by multiple skin biopsies. Histological examination of a skin biopsy from an erythematous raised right perianal area revealed large rounded cells with ample pale staining cytoplasm scattered throughout the epidermis in multifocal nests and a flattened basal layer. A second lesion showed tongues of basaloid cells with peripheral palisading in continuity with the undersurface of the epidermis at multiple points. The individual tumor nests had cytoplasmic melani

Double rhomboidal flap for reconstruction of large surgical defect of the labial commissure.

Closure of perioral surgical defects involving the oral commissures is highly challenging. We describe a 69-year-old male patient with a large basal cell carcinoma of the right perioral region, extending to the right oral commissure. This lesion was radically excised, and the resulting surgical defect was closed using a homolateral double opposing rhomboidal flap. The final result was esthetically very satisfactory, with total preservation of lip function. Double opposing rhomboidal flaps are viable surgical options for the reconstruction of surgical defects involving the perioral area and ora

Analysis of 256 cases of basal cell carcinoma after either one-step or two-step surgery in a Japanese institution.

Basal cell carcinoma (BCC) is a common skin cancer that arises from the cells of the basal layer of the epithelium or from the external root sheath of the hair follicle. In the present report, 256 cases treated surgically between 1999 and 2008 in our department were retrospectively analyzed. The most frequent BCC locations included the face (77.8%), especially the nose (26.9%) and eyelids (21.5%). Incomplete excisions occurred in 21 cases. Two patients experienced local recurrence; one of these patients exhibited a bone metastasis while the other had a metastasis of the parotid gland without t

The development of a Marjolin's cancer in a human immunodeficiency virus-positive hemophilic man and review of the literature.

BACKGROUND: The malignant potential of chronic ulcers and scars secondary to a variety of injuries is well characterized in the medical literature. Furthermore, it has been reported that human immunodeficiency virus (HIV)-positive patients have a higher incidence of basal cell carcinoma (BCC) than the general population. OBJECTIVE: To describe a case of an unusually aggressive morpheaform BCC that developed in a 28-year-old HIV-positive patient. The tumor developed in a scar 25 years after the initial trauma (Marjolin's ulcer). METHODS: Description of a case and review of the literature of M

Basal cell carcinoma occurring at the site of a strawberry hemangioma.

A twenty-seven-year-old white woman presented with a basal cell carcinoma at the former site of a strawberry hemangioma. The strawberry hemangioma had been treated in infancy with dry ice. The lesion spontaneously regressed and left no visible scar. To our knowledge, this is the first reported association of basal cell carcinoma and strawberry hemangioma.

Occurrence and recurrence of basal cell carcinoma of the head and neck in Negroid and albinoid Africans.

A study of sixty patients with basal cell carcinoma of the head and neck region carried out over a six-year period (1979-1985) is hereby presented. Sixteen (26.72 per cent) were albinos and 44 (73.28 per cent) negroids. Forty-eight (80 per cent) were outdoor workers. The negroid patients presented between the 3rd and 4th decades while the albinos presented a decade earlier. The commonest site involved in the head and neck was the forehead. The midface showed the highest recurrence rate in both groups, even after adequate excision. The frequency of recurrence in tumours presenting with a size o

Trends in the presentation and treatment of basal cell carcinomas.

Analysis of 1,620 basal cell carcinomas treated at the Cleveland Clinic Foundation from 1981 to 1983 confirms that the majority of basal cell carcinomas occur on the face and that the relative risk for recurrent tumor is high in certain locations, especially the nose. Alternately, tumors found on the neck/scalp, trunk, and arms have a low relative risk of recurrence when compared to all other anatomic locations. More primary and fewer recurrent tumors now presenting to our unit have been referred for Mohs surgery as primary therapy. This results in higher cure rates for treatment of all basal
