Long-lasting accumulation of hemopexin in permanently transected peripheral nerves and its down-regulation during regeneration.

We have previously demonstrated that hemopexin is present in the intact sciatic nerve and is overproduced in the distal stump after nerve transection (Swerts et al.: J Biol Chem 267:10596-10600, 1992). To get further insight into the function of this hemoprotein in nervous tissue, we have documented long-term changes in hemopexin levels in permanently degenerated (transected) and regenerating (crush-lesioned) sciatic nerves of adult rats, using immunochemical techniques. As early as a couple of days after nerve transection, the amount of hemopexin was raised in the distal stump and at the end

[Lesions of the radial nerve in fractures of the humeral diaphysis. Apropos of 62 cases].

PURPOSE OF THE STUDY: Evaluate outcome of different techniques used for radial nerve repair after humeral shaft fractures. Identify situations where nerve exploration is required or inversely unneeded. Evaluate the risk of more systematic nerve exploration in case of radial nerve palsy. MATERIAL AND METHODS: Over a 10 year period, we cared for 62 patients with humeral fractures associated with radial nerve palsy who were addressed directly to our unit or referred after prior care. There were 54 men (mean age 30 years, 17 - 66) and 8 women (mean age 40 years, 19 - 80). There were 40 fractures

Regeneration of injured skin and peripheral nerves requires control of wound contraction, not scar formation.

We review the mounting evidence that regeneration is induced in wounds in skin and peripheral nerves by a simple modification of the wound healing process. Here, the process of induced regeneration is compared to the other two well-known processes by which wounds close, i.e., contraction and scar formation. Direct evidence supports the hypothesis that the mechanical force of contraction (planar in skin wounds, circumferential in nerve wounds) is the driver guiding the orientation of assemblies of myofibroblasts (MFB) and collagen fibers during scar formation in untreated wounds. We conclude th

Peripheral nerve-dopamine neuron co-grafts in MPTP-treated monkeys: augmentation of tyrosine hydroxylase-positive fiber staining and dopamine content in host systems.

Previous studies of rats in our laboratory indicate that a molecule or molecules released by Schwann cells exert survival and growth-promoting effects on mesencephalic dopamine neurons. In the present study, we have begun to investigate the potential for Schwann cell augmentation of host dopamine fiber systems and embryonic dopamine neuron grafts in non-human primates. Ten adult male St Kitts African Green monkeys treated with the dopaminergic neurotoxin 1-methyl 4-phenyl 1,2,3,6-tetrahydropyridine one year previously, but behaviorally asymptomatic, served as hosts for implant studies. Improved optic nerve visualization and treatment planning through a dedicated optic nerve MRI protocol.

OBJECTIVE: This study describes an innovative optic nerve MRI protocol for better delineating optic nerve anatomy from neighboring pathology. METHODS: Twenty-two patients undergoing MRI examination of the optic nerve with the dedicated protocol were identified and included for analysis of imaging, surgical strategy, and outcomes. T2-weighted and fat-suppressed T1-weighted gadolinium-enhanced images were acquired perpendicular and parallel to the long axis of the optic nerve to achieve en face and in-line views along the course of the nerve. RESULTS: Dedicated optic nerve MRI sequences provid

Optic nerve fiber count and diameter of the retrobulbar optic nerve in normal and glaucomatous eyes.

BACKGROUND: The caliber of the retrobulbar optic nerve and the count of optic nerve fibers vary considerably in normals. The diameter of the retrobulbar optic nerve also decreases with optic nerve atrophy. This study aimed to determine the relationship between the caliber of the optic nerve and the optic nerve fiber count. METHODS: We counted the optic nerve fibers and measured the diameter of histological cross sections of the optic nerve for 56 normal subjects and 23 patients with absolute glaucoma. RESULTS: The optic nerve fiber count increased significantly (P < 0.0001) by 777,000 fibers

Investigation into the possible cause of subjective decreased sensory perception in the nipple-areola complex of women with macromastia.

Patients with macromastia often comment on a lack of sensation in their nipple-areola complex. A study was designed to investigate the cause of this decreased sensation. Two hypotheses were proposed. First, the decreased sensation could result from neuropraxia of the sensory nerve fibers secondary to traction caused by the heavy breast parenchyma. The second hypothesis proposed that tissue expansion of the nipple and areola by the voluminous breast parenchyma caused a decrease in nerve fibers per surface area and hence decreased sensory perception. Sixty-one patients were assessed in the study

[Primary shortening--secondary lengthening. A new treatment concept for reconstruction of extensive soft tissue and bone injuries after 3rd degree open fracture and amputation of the lower leg].

The main problem in major limb replantation--especially of the lower extremity--is an extensive bone- and soft-tissue loss. The traditional replantation concept tries to preserve the initial limb length; only a small shortening is accepted. To avoid a more extensive shortening, often insufficient debridement at the time of replantation is carried out. After successful revascularisation, bone and soft-tissue defects will be reconstructed according to the principles of staged reconstruction. Especially segmental nerve defects of more than one major peripheral nerve and severe skin and muscle los

Potential of dental pulp stem cells and their products in promoting peripheral nerve regeneration and their future applications.

Peripheral nerve injury (PNI) seriously affects people's quality of life. Stem cell therapy is considered a promising new option for the clinical treatment of PNI. Dental stem cells, particularly dental pulp stem cells (DPSCs), are adult pluripotent stem cells derived from the neuroectoderm. DPSCs have significant potential in the field of neural tissue engineering due to their numerous advantages, such as easy isolation, multidifferentiation potential, low immunogenicity, and low transplant rejection rate. DPSCs are extensively used in tissue engineering and regenerative medicine, including f

Distribution of albumin in the normal monkey eye as revealed by Evans blue fluorescence microscopy.

Since intravenously injected Evans blue binds irreversibly to serum albumin, its distribution reflects albumin exchange between the intravascular and extravascular tissue compartments. In histologic specimens examined by fluorescence, microscopy, extravasated Evans blue--albumin complex was identified within the ciliary body and trabecular meshwork of normal monkey eyes. In eyes fixed by intra-arterial perfusion of fixative, no dye was identified in the choroid, retina, or optic nerve. With immersion fixation, however, some extravasation was seen in the choroid and adjacent optic nerve. In som

Low-Intensity Pulsed Ultrasound Neuromodulation of the Hypoglossal Nerve for the Treatment of Sleep Apnea: An Animal Study.

Electrical stimulation of the hypoglossal nerve is an established therapy for obstructive sleep apnea, as activation of tongue muscles helps maintain airway patency during sleep. However, surgical implantation of electrodes carries inherent risks and limits broader application. In this study, we investigated a non-invasive approach using low-intensity pulsed ultrasound neuromodulation to stimulate the hypoglossal nerve and evaluated its effect on tongue muscle activity and upper-airway function. A 1-MHz ultrasound transducer was applied to the cervical region of anesthetized mice to deliver ac

[Spinal endoscopic technique in the surgical treatment of thoracic ossification of the ligamentum flavum].

Thoracic ossification of the ligamentum flavum (TOLF) is a pathological heterotopic ossification disease in which the fibrous tissue of the ligamentum flavum of the thoracic spine converts into bony tissue, often leading to thoracic spinal stenosis and compression of the thoracic spinal cord nerve. When TOLF patients present with symptoms of spinal cord nerve compression, surgical treatment is usually required, and traditional open surgery is more invasive and carries a higher risk of spinal cord nerve injury. In recent years, domestic and foreign researchers have tried to apply spinal endosco

Sympathetic nerve fiber repulsion: testing norepinephrine, dopamine, and 17β-estradiol in a primary murine sympathetic neurite outgrowth assay.

Loss of sympathetic nerve fibers (SNFs) occurs in inflamed tissue; and select semaphorins, upregulated during inflammation, stimulate repulsion/loss of SNFs. However, it is unknown whether other factors released locally in inflamed tissue, such as norepinephrine, dopamine, and 17β-estradiol, are also repellent. In order to study the effects of hormones on SNF repulsion, an SNF outgrowth assay was used. The repellent activity of semaphorins 3C was weaker than of semaphorin 3F. Tumor necrosis factor α (TNF-α) repelled nerve fibers with moderate to strong effects (from 0-100% repulsion). High con

Myelin repair by Schwann cells in the regenerating goldfish visual pathway: regional patterns revealed by X-irradiation.

In the regenerating goldfish optic nerves, Schwann cells of unknown origin reliably infiltrate the lesion site forming a band of peripheral-type myelinating tissue by 1-2 months, sharply demarcated from the adjacent new CNS myelin. To investigate this effect, we have interfered with cell proliferation by locally X-irradiating the fish visual pathway 24h after the lesion. As assayed by immunohistochemistry and EM, irradiation retards until 6 months formation of new myelin by Schwann cells at the lesion site, and virtually abolishes oligodendrocyte myelination distally, but has little or no effe

Distribution of CGRP-, somatostatin-, galanin-, VIP-, and substance P-immunoreactive nerve fibers in the chicken carotid body.

An immunoperoxidase method was used to investigate and compare the distribution of neuropeptide-immunoreactive (ir) nerve fibers and neurofilament-ir fibers in chick carotid body. The vagus nerve and its branches were intensely immunoreactive with an antiserum against chick neurofilaments. The branches from the vagus and the recurrent laryngeal nerves anastomosed within the connective tissue encircling the carotid body, and then entered the organ to form a network of neurofilament-ir fibers. Immunoreactivities for CGRP, somatostatin, galanin, VIP and substance P were found in the carotid body;

Schwannoma Likely Arising from the Pancreatic Head Nerve Plexus That Mimicked a Mucinous Cystic Neoplasm and Was Treated by Pancreaticoduodenectomy: A Case Report.

INTRODUCTION: Pancreatic and peripancreatic schwannomas are exceptionally rare benign peripheral nerve sheath tumors. Cystic and hemorrhagic degeneration may produce imaging findings resembling pancreatic cystic neoplasms, while an extrapancreatic neural origin can be difficult to recognize preoperatively. Lesions located in the pancreatic head nerve plexus region pose additional surgical challenges because of their close relationship with periarterial nerve plexuses and major vessels. CASE PRESENTATION: A 48-year-old woman was referred for a 5-cm multilocular cystic lesion adjacent to the pa

Neuropathic Pain Due to Iatrogenic Lingual Nerve Lesion: Nerve Grafting to Reduce Otherwise Untreatable Pain.

Iatrogenic lingual nerve lesion is a well-known and unfortunate complication after mandibular third molar removal. Occasionally, the nerve injury can cause severe neuropathic pain. Here, the authors present the history of 2 patients with lingual nerve injury due to mandibular third molar removal, and with severe neuropathic pain in the craniomandibular region. Pharmacotherapy and physiotherapy did not reduce the pain, and ultimately, the lingual nerve was surgically explored. Scar tissue and a lingual nerve neuroma were observed and resected in both patients. In the first patient, the gap betwee

Peripheral nerve elongation by laser Doppler flowmetry-monitored expansion: an experimental basis for future application in the management of peripheral nerve defects.

Nerve grafting often fails to achieve optimal functional results and is always associated with donor-site morbidity. Peripheral nerve elongation by the use of a tissue expander may provide a useful adjunct in the management of segmental nerve loss. In the present study, rabbit sciatic nerve (n = 40) was elongated by expansion while nerve blood flow was monitored by laser Doppler flowmetry. Elongation was possible up to 40 percent with preservation of clinical nerve function. Nerve conduction velocity of the expanded nerves decreased in a linear relation to elongation. The reduction in nerve co

Posttraumatic gluteal compartment syndrome. A case report.

Posttraumatic sciatic nerve palsy associated with severe pain, swelling in the gluteal region, elevated tissue pressures, and diffuse edema of the gluteal musculature documented by computed tomography (CT) occurred in a 22-year-old man. Diffuse muscle swelling was observed intraoperatively, and sciatic nerve function returned within days following surgical decompression. This case represents an acute gluteal compartment syndrome, and this entity should be included in the differential diagnosis of posttraumatic sciatic nerve palsy.
