Gitelman syndrome: diagnostic challenges and therapeutic strategies.

Gitelman syndrome (GS), characterized as an autosomal recessive tubulopathy due to SLC12A3 mutations, disrupts the sodium-chloride cotransporter (NCC) within the renal distal convoluted tubule. This condition results in a clinical presentation marked by hypokalemia, hypomagnesemia, metabolic alkalosis, and hypocalciuria. Despite the well-defined biochemical characteristics associated with GS, achieving an accurate diagnosis can be formidable due to the variability in phenotypic expression and the potential for overlapping with other acquired electrolyte disorders. Emerging evidence identifies

Minerals and Human Health: From Deficiency to Toxicity.

Minerals are essential nutrients that play critical roles in human health by regulating various physiological functions. Examples include bone development, enzyme function, nerve signaling, and the immune response. Both the deficiencies and toxicities of minerals can have significant health implications. Deficiencies in macrominerals such as calcium, magnesium, and phosphate can lead to osteoporosis (associated with falls and fractures), cardiovascular events, and neuromuscular dysfunction. Trace mineral deficiencies, such as iron and zinc. Selenium deficiency impairs oxygen transport, immune

Development of Multiple Autoimmune and Endocrine Disorders Following COVID-19 Vaccination: A Case Report.

Coronavirus disease 2019 (COVID-19) vaccines have been instrumental in mitigating the pandemic, but rare post-vaccination immune-related events have been reported. Due to the brief trial period and the pressing need to begin vaccination, adverse events have been documented globally in real-time data format. There have been recent reports of new inflammatory diseases or flare-ups of immune-mediated diseases after vaccinations all over the world. Despite the low prevalence of vaccination-related side effects, it is still important to report and identify the endocrine diseases linked to the COVID

Case report of belt electrode-skeletal muscle electrical stimulation for acute heart failure with severe obesity: a novel therapeutic option for acute phase rehabilitation.

BACKGROUND: Belt electrode skeletal muscle electrical stimulation (B-SES) is an emerging therapy anticipated to yield more favorable outcomes than conventional neuromuscular electrical stimulation (NMES), owing to its larger stimulation area. However, information on its efficacy and safety in patients with heart failure remains limited. CASE PRESENTATION: A 43-year-old man with a body mass index of 41 kg/m2 was admitted to our hospital for acute heart failure due to dilated cardiomyopathy. The patient required prolonged catecholamine support owing to poor cardiac function, and heart transplan

Monitoring spinal muscular atrophy with three-dimensional optoacoustic imaging.

BACKGROUND: Spinal muscular atrophy is a progressive neuromuscular disorder and among the most frequent genetic causes of infant mortality. While recent advancements in gene therapy provide the potential to ameliorate the disease severity, there is currently no modality in clinical use to visualize dynamic pathophysiological changes in disease progression and regression after therapy. METHODS: In this prospective diagnostic clinical study, ten pediatric patients with spinal muscular atrophy and ten age- and sex-matched controls have been examined with three-dimensional optoacoustic imaging an

The role of neuromuscular ultrasound in diagnostics of peripheral neuropathies induced by cytostatic agents or immunotherapies.

A relevant number of cancer patients who receive potentially neurotoxic cytostatic agents develop a chemotherapy-induced peripheral neuropathy over time. Moreover, the increasing use of immunotherapies and targeted agents leads to a raising awareness of treatment-associated peripheral neurotoxicity, e.g., axonal and demyelinating neuropathies such as Guillain-Barré-like syndromes. To date, the differentiation of these phenomena from concurrent neurological co-morbidities or (para-)neoplastic nerve affection as well as their longitudinal monitoring remain challenging. Neuromuscular ultrasound (

Physicians' attitudes and perceptions of neuromuscular blocker infusions in ARDS.

PURPOSE: The perceptions and practices of ICU physicians regarding initiating neuromuscular blocker infusions (NMBI) in acute respiratory distress syndrome (ARDS) may not be evidence-based amidst the surge of severe ARDS during the SARS-CoV-2 pandemic and new practice guidelines. We identified ICU physicians' perspectives and practices regarding NMBI use in adults with moderate-severe ARDS. MATERIALS AND METHODS: After extensive development and testing, an electronic survey was distributed to 342 ICU physicians from three geographically-diverse U.S. health systems(n = 12 hospitals). RESULTS:

Neurodegenerative diseases detection using distance metrics and sparse coding: A new perspective on gait symmetric features.

Gait rhythm fluctuations are of great importance for automatic neurodegenerative diseases (NDDs) detection. They provide a cost-effective and noninvasive monitoring tool in which their parameters are related to neuromuscular function. This study investigated a new solution based on a set of new symmetric features and sparse non-negative least squares (NNLS) coding classifier. Dynamic gait series warping (DGSW), Euclidean, Manhattan, Minkowski, Chebyshev, Canberra distances, and cosine function were used to quantify the amount of divergence between the left and right stride, swing, and stance i

Clinical implication of maximal voluntary ventilation in myotonic muscular dystrophy.

Patients with myotonic muscular dystrophy type 1 (DM1) tend to exhibit earlier respiratory insufficiency than patients with other neuromuscular diseases at similar or higher forced vital capacity (FVC). This study aimed to analyze several pulmonary function parameters to determine which factor contributes the most to early hypercapnia in patients with DM1.We analyzed ventilation status monitoring, pulmonary function tests (including FVC, maximal voluntary ventilation [MVV], and maximal inspiratory and expiratory pressure), and polysomnography in subjects with DM1 who were admitted to a single

Validation of an ambient measurement system (AMS) for physical activities in a paediatric population.

Ambient measurement systems (AMSs) can enable continuous assessment of functional performance at home, increasing the availability of data for monitoring of neuromuscular disease. An AMS passively measures movement whenever someone is in range of the sensor, without the need for any wearable sensors. The current study evaluates the performance of an AMS for three metrics associated with functional assessments in Duchenne muscular dystrophy (DMD): ambulation speed, rise-to-stand speed and arm-raise speed. Healthy paediatric subjects performed a series of functional tasks and were graded by both

Cisatracurium dosing in a patient with hyperthermia.

PURPOSE: A case report involving varying cisatracurium dosing requirements in a hyperthermic patient undergoing prone ventilation who subsequently received active cooling as part of targeted temperature management is presented. SUMMARY: Cisatracurium is known to be primarily metabolized via pH- and temperature-dependent Hofmann elimination. Previous reports in the literature described cases of decreased dosing requirements for both cisatracurium and its parent compound, atracurium, for patients in hypothermic states. While augmented atracurium dosing requirements in hyperthermic states have b

Identifying evidence of cardio-renal syndrome in patients with Duchenne muscular dystrophy using cystatin C.

Patients with Duchenne muscular dystrophy (DMD) develop dilated cardiomyopathy and are at risk for kidney injury. Creatinine based estimated glomerular filtration rate (eGFR) is limited by low muscle mass with low serum creatinine levels in DMD. We assessed the relationship between cardiac function, modified Schwartz eGFR and cystatin C eGFR in patients with DMD. Ninety-three patients with DMD were screened for renal dysfunction in an outpatient neuromuscular clinic. Patients with new nephrotoxic medications, recent hospitalization or decompensated heart failure were excluded from the analysis

New insight into neurodegeneration: the role of proteomics.

Recent advances within the field of proteomics, including both upstream and downstream protocols, have fuelled a transition from simple protein identification to functional analysis. A battery of proteomics approaches is now being employed for the analysis of protein expression levels, the monitoring of cellular activities and for gaining an increased understanding into biochemical pathways. Combined, these approaches are changing the way we study disease by allowing accurate and targeted, large scale protein analysis, which will provide invaluable insight into disease pathogenesis. Neurodegen

Serological diagnostics in myasthenia gravis based on novel assays and recently identified antigens.

Myasthenia gravis (MG) is the most common immune-mediated disorder of the neuromuscular junction with a prevalence of 200-300/million population and its study has established paradigms for exploring other antibody-mediated diseases. Most MG patients (~85%) have autoantibodies against the muscle acetylcholine receptor (AChR-MG), whereas about 6% of MG patients have autoantibodies against the muscle specific kinase (MuSK-MG). Until recently no autoantibodies could be detected in the remaining patients (seronegative MG). Probably, the most sensitive assays for the detection of the autoantibodies

[A case of neonatal congenital myasthenic syndrome caused by MUSK gene variants].

A 3-month-old male infant was admitted because he had required continuous invasive ventilation since birth. He was delivered at 33+4 weeks' gestation without spontaneous respiration; endotracheal intubation with positive-pressure ventilation was initiated, and Apgar scores were 3, 6, and 6 at 1, 5, and 10 minutes, respectively. Two attempts to wean from mechanical ventilation failed. Clinical findings included ptosis, hypotonia, and reduced spontaneous movements. Ultrasonography showed decreased diaphragmatic motion. Monitoring with neurally adjusted ventilatory assist showed low electrical ac

Muscle electrical activity, functional mobility level, and handgrip strength in intensive care unit patients: a single-center observational study.

Intensive care unit-acquired weakness (ICU-AW) is a consequence of critical illness and is associated with functional limitations. Objective assessment using handgrip strength (HGS) and surface electromyography (sEMG) may support monitoring of early rehabilitation in the ICU. This study aimed to evaluate associations between sEMG parameters, mobility level, and HGS in ICU patients. In this single-center observational study, ICU patients were assessed when they met criteria for early rehabilitation using bilateral sEMG and handgrip strength measurement, then classified into four functional mobi
