Bottoms Up: A History of Rectal Nutrition From 1870 to 1920.

From the 1870s through the early 20th century, physicians frequently relied upon nutritive enemata to succor patients suffering from bowel obstructions and other disorders of the gastrointestinal system. Far from extraordinary or outlandish, this therapy was used on paupers and presidents alike, including on Garfield and McKinley after their assassination attempts. The medical milieu of the late 19th century provided particularly promising circumstances for its practice, with the rise of allopathic medicine generally-and surgery especially-coinciding with flourishing research on the physiology

The Role of GI Peptides in Functional Dyspepsia and Gastroparesis: A Systematic Review.

Functional dyspepsia (FD) and gastroparesis (GP) are common disorders of the upper gastrointestinal tract. The pathophysiology of these conditions is likely to be heterogenous, and factors such as altered motility, sensitivity and response to nutrition have been identified as putative underlying mechanisms. Motility, sensitivity as well as responses to nutrition can be influenced or mediated by peptide hormones and serotonin released from the gastrointestinal mucosa. This review summarizes the role of GI peptides in functional dyspepsia and gastroparesis. In most studies, the levels of somatos

Successful treatment of extreme acute lead intoxication.

Anorexia and bulimia nervosa are characterized by unbalanced eating patterns that include inadequate dietary intake of various nutrients. Conservation mechanisms resulting from starvation and/or self-prescribed nutrient supplements can result in laboratory values that appear within normal limits. These artificially inflated values drop to dangerous levels in some patients once rehydration and refeeding begin. Electrolyte status must be closely monitored during this time to prevent complications. Other micronutrient deficiencies can be corrected with adequate dietary intake, but patients with e

[Chronic functional dyspepsia or irritable colon].

An attempt is made to synthesize the fundamental information concerning the pathogenesis of chronic functional digestive disorders, which should be designated as "chronic functional dyspepsia". At present the term "irritable colon" does not seem justified since the disorder affects the entire gastrointestinal tract. In the development of this disorder three types of fundamental pathogenic factors apparently participate: 1. Deficient protein nutrition leading to hypotrophy of the intestinal mucosa. 2. Alterations of intestinal motility related fundamentally to neurotic dysfunction. These are hy

The Impact of Anorexia Nervosa and the Basis for Non-Pharmacological Interventions.

Anorexia nervosa is a psychiatric disorder with an unknown etiology that is characterized by an individual's preoccupation with their weight and body structure while denying the severity of their low body weight. Due to the fact that anorexia nervosa is multifaceted and may indicate the coexistence of genetic, social, hormonal, and psychiatric disorders, a description of non-pharmacological interventions can be used to ameliorate or reduce the symptoms of this condition. Consequently, the purpose of the present narrative review is to describe the profile's context in the anorexic person as wel

Metabolic profiles of cow's blood; a review.

The term 'metabolic profile' refers to the analysis of blood biochemical parameters that are useful to assess and prevent metabolic and nutritional disorders in dairy herds. In the higher standards of milk production, the priority in modern breeding is keeping dairy cows in high lactation and healthy. The proper analysis, as well as control. of their feeding and metabolic status is immensely important for the health condition of the herd. The disproportion between the genetically determined ability for milk production and the limitations in improving the energy value of the ration may be the c

[Chronic intestinal pseudo-obstruction: pathogenesis, diagnosis and therapy].

Chronic intestinal pseudo-obstruction (CIPO) is a rare disease in which a severe intestinal motility disorder impairs transit of chyme so that patients suffer from symptoms of a mechanical ileus without mechanical obstruction. CIPO may be a primary or secondary disorder due to muscular, neurologic, metabolic or endocrine disorders, but may also occur postinfectiously, postoperatively, following abdominal radiation or be caused by drugs or noxae. In severe cases, the typical history of (repeated) symptoms of mechanical obstruction leading to unsuccessful laparotomies will give key clues for dia

Role of carnitine in disease.

Carnitine is a conditionally essential nutrient that plays a vital role in energy production and fatty acid metabolism. Vegetarians possess a greater bioavailability than meat eaters. Distinct deficiencies arise either from genetic mutation of carnitine transporters or in association with other disorders such as liver or kidney disease. Carnitine deficiency occurs in aberrations of carnitine regulation in disorders such as diabetes, sepsis, cardiomyopathy, malnutrition, cirrhosis, endocrine disorders and with aging. Nutritional supplementation of L-carnitine, the biologically active form of ca

Women's Self-Assessment of Quality of Life and Menopausal Symptoms: An Online Survey of 26,000 Women in German-Speaking Countries.

Menopausal symptoms can substantially impair women's quality of life, yet large-scale, population-based data from German-speaking regions are lacking. We used data of a cross-sectional online survey among 26,338 women in Germany, Austria, and Switzerland, using validated instruments such as the Menopause Rating Scale (MRS II), Insomnia Severity Index (ISI), and Alcohol Use Disorders Identification Test (AUDIT-C). Additional questions covered weight change, employment, nutrition, and physical activity. We investigated correlations and group differences using descriptive statistics and univariat

Nutritional therapy for MRSA sepsis complicated with pulmonary embolism in children under limited energy and nutrient conditions: a case report.

The nutritional treatment of methicillin-resistant Staphylococcus aureus sepsis combined with pulmonary embolism presents considerable challenges due to the risks associated with tube placement, coagulation disorders, severe infections, digestive limitations, and fluid restrictions in pediatric patients. This report discusses the case of an approximately 13-year-old female patient admitted with symptoms of right lower limb pain, fever, and cough accompanied by shortness of breath. The patient was assessed to be at moderate risk of malnutrition. In the early stages of treatment, permissive low-

[Perception of taste and smell in normal and pathological aging: an update].

The physiological and pathological aging on sense organs is widely studied in the visual and hearing modalities. By contrast, taste and smell changes are widely overlooked. These symptoms are rarely evoked in the elderly and often neglected in clinical practice. Studies in this population are rare and show contradictory results. In this update, we describe the perception of taste and smell in aging. In a first part, we present studies about the aging of smell and taste senses. While taste is remarkably spared during aging, olfaction decreases dramatically since 5th decade of life, frequently r

Symptomatic acquired zinc deficiency in at-risk premature infants: high dose preventive supplementation is necessary.

Zinc is a cofactor for several enzymes involved in many metabolisms. Zinc deficiency induces various disorders such as acrodermatitis enteropathica, either inherited or acquired. We report three cases of premature infants (24-31 wks gestational age) with low birthweight (650 to 940 g) and enteropathy, two of whom presented with necrotizing enterocolitis. All infants were fed by total parenteral nutrition. At a chronological age ranging from 73 to 80 days, all infants developed a periorificial dermatitis. Before the onset of the first signs, they had received zinc supplementation ranging from 1

[A case report of severe hypophosphatemia in the course of refeeding syndrome].

This is a case report of 53 year old male who underwent pancreatoduodenectomy for chronic pancreatitis. Two months after the operation the symptoms of severe malnutrition caused by nutrition digestive disorders developed. The patient was admitted to the hospital with the symptoms of severe malnutrition and general poor medical condition. The patient was put on total parenteral nutrition (TPN) after the preliminary treatment of water-electrolytes imbalance. After 3 days of TPN a severe hypophosphatemia occurred (serum level < or = 0.25 mmol/l). The refeeding syndrome developed despite the use o

Understanding pediatric intestinal pseudo-obstruction: implications for nurses.

Pseudo-obstruction is actually a group of disorders resulting in bowel dysmotility in the absence of anatomic obstruction. Because of varied pathologic abnormalities of the GI tract involved in the disorder, pseudo-obstruction has been reported in the literature under different names, including hypoganglionosis, chronic adynamic ileus, pseudo-Hirschprung's disease, visceral neuropathy, visceral myopathy, and megacystis-microcolon-intestinal hypoperistalsis syndrome. Although a rare condition, its effects are severe, disabling, and life-threatening in pediatric clients. Prognosis is inversely c

[Extensive small intestinal resection in newborn infants].

This retrospective study analyzes the management of 83 infants who had undergone extensive small bowel resection as newborns between 1970 and 1987. Resection was performed for atresia (n = 34), volvulus (n = 21), laparoschisis (n = 10), vascular enteropathy (n = 10) and other disorders in 8 cases. The patients were divided into two groups: Group I (33 children) has less than 40 cm and Group II (50 children) 40-80 cm of residual small bowel respectively. Survival depended on the length of residual small bowel (Group I: 63.6%, Group II: 92%) and on their date of birth (born before 1980: 65%, aft

The role of specific nutrients in preventing immune system and blood cell disorders: an umbrella review.

INTRODUCTION: Nutritional deficiencies affecting the immune and haematopoietic systems represent a well-known global public health challenge: only the iron deficiency anaemia affects 1.62 billion individuals, especially in vulnerable populations. However, the protective effect that nutrition might give on disorders of these systems is still poorly understood. This umbrella review aims to synthesise the available evidence on the effectiveness of nutritional interventions in the primary prevention of blood and immune disorders, with a focus on the role of essential micronutrients and bioactive c

Occurrence of nutritional hypocalcaemic rickets-related dilated cardiomyopathy in a child with concomitant rickets and infantile-onset Pompe disease.

Infantile-onset Pompe disease is a lysosomal storage disorder characterised with hypertrophic cardiomyopathy, respiratory insufficiency, and hypotonia. Dilated cardiomyopathy is an extremely rare and curable complication of nutritional hypocalcaemic rickets. A 3-month-old female infant was referred to our paediatric ICU with a 4-day history of fatigue, tachypnoea, tachycardia, hypoxia, and respiratory failure. According to the laboratory, radiology, and echocardiography findings, she was first diagnosed with nutritional hypocalcaemic rickets-related dilated cardiomyopathy, but vitamin D and el

Enteral nutrition in surgical patients.

PURPOSE: Malnutrition is well-recognized as a potential cause of increased morbidity and mortality in surgical patients; however, enteral and parenteral nutritional support given pre- and postoperatively have been shown to decrease these rates. We conducted a prospective study to assess the short-term efficacy of oral dietary supplementation in malnourished patients undergoing major abdominal surgery. METHODS: Sixty patients undergoing major abdominal surgery were divided according to nutritional risk index into borderline (BM), moderately (MM), and severely (SM) malnourished categories. The
