A Life Threatening Pitfall in Ear Surgery: Extracranial Sigmoid Sinus.

AIM: The aim of this article is to imply the significance of temporal bone computed tomography imaging before temporal surgeries. CASE: A 74-years-old patient was admitted to emergency department with dizziness and nausea. The neurologic examination showed a spontaneous nystagmus, whereas otologic examination revealed a left tympanic membrane perforation with mild discharge. A temporal bone computed tomography imaging was scheduled to exclude cholesteatoma and perilymphatic fistula. Computed tomography detected an anterior sigmoid sinus with middle fossa defect and subcutaneous course of the

Mastoidectomy and mastoid obliteration with autologous bone graft: a quality of life study.

BACKGROUND: A mastoid cavity resulting from a canal wall down mastoidectomy can result in major morbidity for patients due to chronic otorrhea and infection, difficulty with hearing aids and vertigo with temperature changes. Mastoid obliteration with reconstruction of the bony external ear canal recreates the normal anatomy to avoid such morbidity. Few have the studied the quality of life benefit that this procedure confers. METHODS: This retrospective observational study was conducted to determine if mastoid obliteration with autologous cranial bone graft following mastoidectomy improves qua

Psychosocial effects of otoplasty in children with prominent ears.

This study aimed to investigate changes experienced by children during the pre- and postoperative periods of prominent ear corrective surgery. A total of 30 patients with prominent ears, sometimes called "lop ears" or "cup ears," ranging in age from 6 to 14 years were consecutively enrolled in this study. Half of the patients (n = 15, 50%) were male. The inclusion criteria specified children with prominent ears and reports of evident anatomic deformity. Clinical evaluations, routine laboratory tests, and interviews were performed in the pre- and postoperative periods. To assess the dissatisfac

[Correction of vestibular disorders in patients with otitis media purulenta chronica complicated by labyrinthitis].

A vestibulogic examination was made in 66 patients with otitis media purulenta chronica complicated with labyrinthitis. The patients were divided into two groups: group 1 received surgical treatment in an acute period of labyrinthitis, group 2 was treated conservatively with delayed operation. The patients were examined at admission, 2 weeks, 1, 3, 6 and 12 months after the treatment. It was found that compensation of the vestibular function in group 1 took place 6 months after temporal bone surgery, in group 2-9 months after delayed operation, i.e. 1 years after hospitalization.

[Surgical removal of unilateral vestibular schwannomas after failed Gamma Knife radiosurgery].

One of the primary criticismes of vestibular schwannoma (VS) radiosurgery is that the risk of surgical morbidity is increased for patients whose tumor progresses after the procedures. We reviewed the French experience of operated patients after failed Gamma Knife radiosurgery. From July 1992 to January 2002, 25 out of the 1000 treated patients underwent another treatment procedure for a gamma knife failure. Excluding the NF2 patients, 21 patients have been operated and the present study shows the data collected for 20 of them. In order to analyze the difficulties observed during the surgery, a

Syndrome of recurrent meninigitis due to congenital perilymph fistula with two different clinical presentations.

Recurrent meningitis secondary to a congenital labyrinthine anomaly is a rare clinical entity, diagnosis of which is dependent upon certain clinical, radiological and intraoperative features. In the following report we describe two children with congenital labyrinthine fistula and recurrent meningitis whose clinical presentation, radiological features and intraoperative findings were dissimilar and thus, illustrative of two different ways of presentation of this rare disorder. While one had a classical Mondini defect and unilateral hearing loss, the other had normal audiometric and radiographi

Unruptured translabyrinthine meningocele without CSF otorrhea.

Labyrinthine meningocele can be classified into translabyrinthine and perilabyrinthine type. We describe a case of rare unruptured translabyrinthine meningocele (TLM). It is rare to encounter an unruptured TLM because it is usually diagnosed after rupture as a labyrinthine fistula, cerebral spinal fluid otorrhea, and subsequent meningitis. We provide for the first time an intraoperative photo and video of a case of an unruptured TLM that developed through an inner ear malformation in a single-side deaf child, which was preoperatively misdiagnosed as congenital cholesteatoma in preoperative tem

Recurrent phosphaturic mesenchymal tumour of the temporal bone causing deafness and facial nerve palsy.

OBJECTIVE: We describe the first reported case of a phosphaturic mesenchymal tumour, mixed connective tissue variant, invading the temporal bone. CASE REPORT: A female patient presented with increasing deafness. On examination there appeared to be a mass behind an intact tympanic membrane. Further radiological investigation showed a vascular mass occupying the middle ear, mastoid and internal auditory meatus. This was surgically resected and revealed to be a benign phosphaturic mesenchymal tumour, mixed connective tissue variant. The tumour recurred a year later, presenting as facial nerve pa

Early non-EPI DW-MRI after cholesteatoma surgery.

OBJECTIVE: Middle ear cholesteatoma may lead to the erosion of the bony structures of the temporal bone, possibly causing intra- and extracranial complications. Surgical treatment is mandatory, and due to possible residual/recurrent disease, the use of reliable diagnostic methods is essential. Our study aimed to evaluate the reliability of non-EPI DW-MRI for the follow-up of cholesteatoma after surgery. METHODS: In a study group including 53 consecutive patients who underwent surgery for cholesteatoma at a tertiary university hospital, an imaging protocol was applied, including non-echo plana
