Newly described weathering pattern in pili annulati hair shafts: a scanning electron microscopic study.

Certain scalp hair shafts from 2 of 10 cases of pili annulati examined by scanning electron microscopy exhibited an unusual weathering pattern. The majority of affected hair shafts showed minor surface abnormalities at regular intervals (nodes) associated with the underlying spaces. However, in a few examples, there was marked damage to the cuticle at the nodes exposing the underlying cortex; in severe cases there was cracking of both cuticle and cortex. These damaged nodes were also associated with trichorrhexis nodosa-like breaks of the hair shaft. This study shows that the nodes in pili ann

Pigmented purpuric dermatosis (Schamberg's purpura) in an infant.

Purpura in an infant is usually an alarming sign of a systemic (infectious, hematologic-oncologic or immunologic) disease. Chronic purpuric dermatoses have not been reported in infants and, therefore, are not considered in the differential diagnosis of purpuras in this age. We report on a female infant with progressive purpura who underwent extensive laboratory investigations to rule out a systemic disease. Based on the laboratory findings and clinical course, the diagnosis of Schamberg's purpura was established.

Dowling-Degos disease associated with hidradenitis suppurativa.

A case of Dowling-Degos disease coexisting with hidradenitis suppurativa is described. We propose that the follicular occlusion inherent in Dowling-Degos disease may predispose to the development of hidradenitis suppurativa.

Minocycline-induced hyperpigmentation.

A 70-year-old man developed hyperpigmentation of his forearms, hands, fingernails, sclerae, ears, and teeth after 9 years of therapy with minocycline for acne rosacea. Minocycline is widely used in the treatment of acne vulgaris and uncommonly produces the side effect of hyperpigmentation. This effect does not appear to be dose-dependent and usually resolves within months to years after discontinuation of therapy. Discoloration of adult teeth, however, is generally permanent.

The role of pulsed laser in the management of cosmetically significant pigmented lesions.

Pulsed laser can be effective in the treatment of naevus of Ota, some cafe au lait macules, lentigines and junctional naevi. It is less useful for compound, intradermal or congenital naevi, chloasma, or post-inflammatory pigmentation. Hydroquinone and broad spectrum sunscreen are routinely used after treatment.

Regional dermatoses in the African. Part I. Facial hypermelanosis.

Acquired facial hyperpigmentation is a common problem among African patients, particularly women, where the causes of the dermatoses are identified largely from circumstantial evidence of exposure to known agents. These include hydroquinone-induced exogenous ochronosis from skin-bleaching creams, mercury deposits from mercury-containing skin-lightening soaps and creams, sulfonamide-related drugs, antimalarials, fixed drug eruptions, clofazimine, and photosensitizing herbal concoctions. The differential diagnosis includes melasma and facial erythema ab igne (local cooks).

The dermatosis of kwashiorkor in young children.

A characteristic dermatosis is often seen in young children suffering from kwashiorkor, one of the severe forms of protein-energy malnutrition (PEM). The skin lesions usually first occur in areas subject to friction or pressure, for example the groin, behind the knees, on the buttocks, and at the elbows; in advanced cases, the dermatosis may be almost anywhere on the body--trunk, limbs, or head. Darkly pigmented patches form, and these may peel or desquamate, rather like old, sun-baked blistered paint. This has led to the terms "peeling paint" or "flaky paint" dermatosis. Underneath these flak

Skin pigmentation associated with minocycline therapy.

A patient on long-term minocycline therapy developed blue-black discoloration on the legs. Skin biopsy specimens from the pigmented areas were examined by light and electron microscopy and energy-dispersive X-ray microanalysis. Pigmented granules were present at all levels of the dermis and subcutaneous fat tissues. Ultrastructural examination showed electron-dense granular material within the cytoplasm of dermal macrophages and energy-dispersive X-ray microanalysis indicated that the granules contained iron. Thyroid tissue obtained by aspiration biopsy showed the presence of fine brown granul

The effect of solvent extraction on the lipids of the stratum corneum in relation to observed immediate whitening of the skin.

Irritant contact dermatitis along with an increased transepidermal water loss can result from exposing the skin to solvents. A study of the interaction of various solvents with human stratum corneum was made using thin-layer chromatography. Comparison of 10 solvents (trichloroethylene, toluene, dimethylsulfoxide, n-hexane, 1,2-propanediol, carbon tetrachloride, methyl ethyl ketone, ethanol, acetone, distilled water) showed that 3 solvents (dimethylsulfoxide, 1,2 propanediol, distilled water) did not extract lipids from intact human stratum corneum. The findings are discussed in relation to the

The skin changes in the Crow-Fukase (POEMS) syndrome. A case report.

A man with a six-year history of intermittent burning sensations in his legs, hypothyroidism, and peripheral neuropathy also had hypertrichosis, hyperpigmentation, Terry nails, and taut thickened skin of the extremities. Discovery of a plasma cell tumor led to recognition of the Crow-Fukase syndrome, and appropriate treatment with melphalan and prednisone.

Hypopigmented mycosis fungoides. Speculations about the mechanism of hypopigmentation.

Hypopigmented, nonatrophic macules are an unusual manifestation of lesions of early mycosis fungoides. Presented herein is a patient with such lesions and an unusual mask-like hypopigmentation of the face. We review both the clinical and microscopic findings in this patient and in previously reported patients with hypopigmented lesions of mycosis fungoides. Finally, speculation is made as to the mechanisms of the hypopigmentation.
