General practice in the German Democratic Republic (1949-1990) : A discipline between threat and professionalization.

BACKGROUND: In the 1950s the socialist health policy in East Germany did not follow a clear-cut course with regard to outpatient medical care. Whilst state-run policlinics gradually took the place of doctors in private practice, the required qualifications of physicians working in outpatient care remained unclear. After preparatory lobbying by committed physicians from the outpatient sector, the 1960 Weimar Health Conference finally paved the way for the preservation and professionalization of general practice in East Germany. AIM: The article analyzes the formation of general practice as a s

[Rescue system establishment for neurological patients in case of natural disastrous emergency].

Big earthquake and subsequent gigantic tsunami killed more than 20,000 peoples in Tohoku area of Japan on March 11, 2011. Neurological patients were one such victim because they are usually very vulnerable to such a huge tragedy due to their physical disability including artificial ventilator-support. On occasion of the last tsunami, most cases showed "all or nothing" to lose life or to survive, and there were only a little cases who needed emergency surgical treatment. In the very early period, some neurological patients required electric power to keep their life-supportive ventilator at evac

[Medical certificates: law, deontology and practice].

At the start of the 21st century, in a society that gives the impression of switching century every five years because of its never-ending evolution, the general practitioner is confronted every day to more complex situations, including problems encountered at the legal level. These last few years, the legal interrogations relevant to medicine and its progress became considerably more extensive in Belgium, as in many other countries. The medical law and its regulation are the subject of many front titles in the press and of animated political debates. The main issues concern the organ transpla

From Engel's Bio-Psycho-Social model to the personalized health determinants model: a comprehensive framework and illustrative operationalization for precision health.

Engel's Bio-Psycho-Social (BPS) model (1977) reframed healthcare by integrating biological, psychological, and social perspectives. Despite its influence, the model has been criticized for insufficient specificity in domains critical to precision health, including nutrition, lifestyle, socioeconomic, environmental, and structural factors. To address these limitations, we propose the Personalized Health Determinants Model (PHDm), a comprehensive nine-dimension framework, Biological, Psychological, Social, Cultural, Environmental, Economic, Political, Spiritual, and Lifestyle, synthesized from E

Barriers and enablers in implementing integrated primary health care: A qualitative assessment in West Sumbawa District, Indonesia.

In 2023, Indonesia introduced an integrated primary health care (IPHC) model called Integrasi Pelayanan Kesehatan Primer (ILP) as part of the national primary care transformation agenda. ILP seeks to strengthen the network of public primary care providers across sub-district, village, and community levels with a five-cluster model organized around a life-cycle approach, replacing the previously fragmented, program-based model. West Sumbawa District (WSD) in West Nusa Tenggara Province was chosen as a national pilot district by the Ministry of Health (MOH), with ILP implementation expanding fro

Coastal canaries.

The winter storms of January and February 2026 caused severe coastal damage along southwest England, reviving memories of the destructive 2013/14 winter and exposing persistent weaknesses in UK coastal resilience policy. Using the Slapton Sands gravel barrier system, in Devon, as a case study, this article argues that repeated damage to the village of Torcross and the A379 road section that runs along the barrier illustrates a broader national cycle of escalating coastal risk despite decades of scientific understanding and strategic planning through Shoreline Management Plans (SMPs). Although

From criminalization to care: a comparative rights-based policy review of HIV responses in South Asia.

BACKGROUND: South Asia faces concentrated HIV epidemics rooted in legal and social marginalization of key populations. Laws criminalizing same-sex relations, sex work, and drug use, combined with gaps in anti-discrimination protections and funding constraints for civil society organizations, undermine progress toward the UNAIDS 95-95-95 targets. This review applies a rights-based approach (RBA) to compare national policies and outcomes across India, Nepal, Pakistan, and Sri Lanka, and offers actionable regional guidance. METHODS: A comparative analysis was performed using a five-dimension RBA

Social determinants of health among people working on informal gold mines in French Guiana: a multicentre cross-sectional survey.

INTRODUCTION: Social determinants of health, such as living and working conditions, economical and environmental context and access to care, combine to impact the health of individuals and communities. In French Guiana (FG), the persons working in informal artisanal and small-scale gold mining in the rainforest are a particularly vulnerable population which lives in precarious conditions and far from the health system. Previous studies have demonstrated their high morbidity due to infectious diseases. This study aims to describe the social determinants of health in this specific population. M

Study protocol for development of an options assessment toolkit (OAT) for national malaria programs in Asia Pacific to determine best combinations of vivax radical cure for their given contexts.

INTRODUCTION: Recent advances in G6PD deficiency screening and treatment are rapidly changing the landscape of radical cure of vivax malaria available for National Malaria Programs (NMPs). While NMPs await the WHO's global policy guidance on these advances, they will also need to consider different contextual factors related to the vivax burden, health system capacity, and resources available to support changes to their policies and practices. Therefore, we aim to develop an Options Assessment Toolkit (OAT) that enables NMPs to systematically determine optimal radical cure options for their gi

Elimination of dog-mediated human rabies: the burden of human rabies in Africa.

Hope exists for the elimination of dog-mediated human rabies in Africa. Momentum is gathering towards this goal, with an increasing number of successful demonstration projects showing that elimination is feasible. The Pan African Rabies Control Network is bringing Africa together against rabies, supported by the World Health Organization, the World Organisation for Animal Health and the Food and Agriculture Organization of the United Nations, which have a combined resolution to eliminate human deaths from dog-transmitted rabies by 2030. Furthermore, the inspiring examples of both rinderpest an

Reducing corruption in a Mexican medical school: impact assessment across two cross-sectional surveys.

BACKGROUND: Corruption pervades educational and other institutions worldwide and medical schools are not exempt. Empirical evidence about levels and types of corruption in medical schools is sparse. We conducted surveys in 2000 and 2007 in the medical school of the Autonomous University of Guerrero in Mexico to document student perceptions and experience of corruption and to support the medical school to take actions to tackle corruption. METHODS: In both 2000 and 2007 medical students completed a self-administered questionnaire in the classroom without the teacher present. The questionnaire

Reconstructing infectious disease prevention mechanisms in carceral settings in transitional states: lessons from the Chinese experience.

AIMS: This study aims to examine how infectious disease prevention mechanisms are constructed and operationalized within carceral settings in transitional states, using China as a representative case. It focuses on the discursive and structural strategies employed by stakeholders to navigate institutional challenges in epidemic control and to delineate responsibilities across health and justice sectors, while also considering the ethical implications of these strategies and upholding the dignity and rights of individuals affected. METHODS: Guided by the epidemiological triangle model, the stu

Peer Support in Online Women's Health Communities: Mixed Methods Formative Analysis of Reddit Discourse.

BACKGROUND: Stigmatized women's health issues, such as polycystic ovary syndrome (PCOS) and endometriosis, are often marginalized or dismissed in traditional clinical settings. This drives individuals to seek peer support in anonymous online communities such as Reddit. While these digital platforms host critical discussions, they are often designed as static information repositories, failing to account for the complex emotional, temporal, and cultural dynamics that shape users' support needs. There is a disconnect between the lived experiences of users-particularly feelings of clinical dismiss

Prevalence and Trends of Hepatitis C, Hepatitis B, and Human Immunodeficiency Viruses Over Half a Decade Among Healthy Blood Donors Across Sindh, Pakistan.

INTRODUCTION: Pakistan has a high prevalence of viral hepatitis and these transfusion-transmitted illnesses (TTIs) pose a major hazard to the health of patients who need blood transfusions, which has a negative impact on the affordability and accessibility of safe blood products in underfunded or less strengthened healthcare systems. While selecting a donor for blood donation, he/she must be healthy enough to donate 500 mL of whole blood, but some of them who were considered the healthiest community were caught to be reactive while getting screened for hepatitis B virus (HBV), hepatitis C viru

Delayed, fragmented, inequitable: a global qualitative systematic synthesis of fertility care pathways and determinants.

BACKGROUND: Infertility is a prevalent reproductive health condition that affects one in six people worldwide. Despite recent increasing recognition of the global burden of infertility, a large fertility care gap remains, with limited understanding of the determinants of care-seeking among those who require infertility services to achieve their desired family size. OBJECTIVE AND RATIONALE: To date, there has been no comprehensive global thematic synthesis specifically focused on the qualitative evidence regarding determinants of fertility care-seeking. This review aims to identify and to syst

[Fifty years of public service for Quebec community psychiatry services. Part I].

This essay comprises 2 parts. It aims to recognize the public service of psychiatrists of the Département de psychiatrie de l'Université de Montréal who served at the provincial level of the Ministry of Health and Social Services for deinstitutionalisation of policies and organisation of services, at the service of people with severe mental disorders. First with Dr. Camille Laurin post-face of the 1962 book Les fous crient au secours! Infectious diseases in Afghanistan: Strategies for health system improvement.

BACKGROUND AND AIM: Afghanistan is grappling with a severe health crisis marked by a high prevalence of infectious diseases, particularly tuberculosis, malaria, HIV, and the added strain of the COVID-19 pandemic. The nation's healthcare system, already fragile, faces formidable challenges. Socioeconomic constraints, including limited resources and financial barriers, hinder healthcare accessibility, leading to delayed or inadequate care. Environmental factors, such as poor sanitation and crowded living conditions, exacerbate the transmission of diseases, especially waterborne illnesses. Govern
